The adrenal contribution to cardiac responses elicited by acute hypoxia was assessed in 16 piglets, 1-12 wks old, anesthetized with pentobarbital (30 mg/kg). External cardiac work was held constant and parasympathetic blockade was produced in each animal with atropine (1 mg). Hypoxia was produced by addition of N2 to the respirator. In a sham-adrenalectomy group (n = 6) left ventricular (LV) dP/dtmax increased significantly during hypoxia (PaO2 approximately 30 mmHg) to 3,680 +/- 414 mmHg/s from control values of 2,686 +/- 317 mmHg/s (P < 0.01). Heart rate rose from 171 +/- 6 to 186 +/- 7 beats/min (P < 0.02). These responses were not significantly altered by ganglionic blockade with trimethaphan camsylate (0.5 mg x kg-1 x min-1). Equally large increases of LV dP/dtmax appeared when heart rate was held constant by pacing. beta-Adrenoreceptor blockade with practolol (4 mg/kg) sharply reduced but did not eliminate the response. In contrast, no changes in LV dP/dtmax or heart rate were observed during hypoxia in adrenalectomized piglets (n = 6). These findings indicate that the increased cardiac contractility during acute hypoxia in piglets is dependent on the integrity of the adrenal glands and that there is minimal contribution from cardiac sympathetic nerves.
To analyze the dynamic properties of body temperature and effector mechanisms during endotoxin fever, both experimental and mathematical procedures were applied. Experiments were carried out on rabbits in a climatic chamber at various ambient temperatures. Salmonella typhosa endotoxin (0.1 microgram/kg) was injected into an ear vein. A biphasic core temperature increase evoked by different effector mechanisms depending on ambient temperature was observed. A mathematical model based on experimental results with nonfebrile rabbits predicts the effector behavior at all ambient temperatures. From a comparison of experimental results with the model prediction, it is concluded that the increase of core temperature during fever is essentially caused by a dynamic shift of the controller characteristics. The effect of the pyrogen may be simulated by a resultant fever-controlling signal that is biphasic but increases more steeply than does core temperature. The analysis suggests that the three possible fever-driving effectors, metabolism, ear blood flow, and respiratory evaporative heat loss, should be controlled by the same resultant signal, although the time courses of the effectors and of core temperature vary distinctly at different air temperatures. The model uses an additive controller structure.
EINLEITUNOIn der Therapie der fortgeschrittenen Herzinsuffizienz hat sich in jüngster Zeit neben der klassischen medikamentösen Behandlung gezeigt, dass durch zusätzliche elektrische Stimulation des Herzens im linken Ventrikel oder biventrikulär eine weitere hämodynamische Verschlechterung nicht nur verhindert werden kann, sondern eine deutliche Verbesserung erzielt wird [1,2]. Es ist jedoch nicht gänzlich geklärt, welcher Mechanismus zu dieser Verbesserung der linksventrikulären Funktion führt. So gibt es Hinweise dafür, das neben der biventrikulären Stimulation durch eine weitere multisite Stimulation eine bessere Performance und damit eine Abnahme des kritischen Zustandes solcher insuffizienter Herzen erreichbar ist [3]. In dieser Studie wurde das Kontraktions verhalten der Herzen bei verschiedenen Stimulationsarten untersucht. MATERIALIEN UND METHODEN Die Untersuchungen hierzu wurden an vier isolierten, mit Eigenblut perfundierten und oxygenierten Herzen von Hausschweinen durchgeführt. Es handelte sich um Akutversuche an nicht insuffizienten Organen, die nach Explantation in einem Experimentalaufbau reperfundiert und für den gesamten Untersuchungszeitraum in einem biventrikulären Arbeitsrhythmus gehalten wurden [4]. Die Stimulationen wurden mit Hilfe von epikardialen Saugelektroden und einem externen Stimulator durchgeführt. Es wurde nach einem Versuchsprotokoll oberhalb der intrinsischen Frequenz mit 100 und 120 bpm bei jeweils konstanten Frequenzen über eine Minute stimuliert. Es wurde die klassische Stimulation im rechten Ventrikel verglichen mit der LV-Stimulation im Bereich des linksposterolateralen Astes, der linken Ventrikelspitze sowie biventrikulär, d.h. RV-Spitze und LV posterolateral oder RV-Spitze mit LV-Spitze oder LV-Spitze mit LV-posterolateral, sowie "multisite", d.h. RV-Spitze mit LV-Spitze und LV posterolateral (Abb. 1). Die hämodynamischen Daten (LVP, RVP) wurden mit Tip-Kathetern (Fa. Millar), der AP mit Statham Drucksensoren erfasst. Das EKG wurde intrakardial über einen mehrpoligen rechtsventrikulär gelegenen Katheter abgeleitet. Zur Analyse wurden die Daten der letzten 10 sec einer Stimulationsphase herangezogen. ERGEBNISSE Aufgrund der durch den Versuchsaufbau bedingten Möglichkeit der Blutzufuhr zu den Vorhöfen wurde diese für die Versuche konstant gelassen. So wurden auch weitgehend konstante Werte für den LAP von 10±2mmHg und für den RAP von 3±2mmHg eingehalten. Die av-Zeit wurde für alle Stimulationsarten auf konstant 80 ms eingestellt. Der LVP (Abb.2) zeigte keine signifikanten Unterschiede innerhalb der Frequenzstufen von 100 und 120 bpm. Der RVP ergab einen fast kontinuierlichen Wert von 28 ± l mmHg. Die Druckanstiegsgeschwindigkeit (dP/dt max) für den linken Ventrikel ergab tendenziell höhere Werte für die biventrikuläre und die multisiteStimulation, als für die single-site Stimulation. EINLEITUNG Bisher veröffentlichte Herz/ Kreislauf-Modelle sind entweder zur Untersuchung kurzfristiger, weitgehend strömungsmechanisch und muskelphysiologisch bedingter Herz/ Kreislaufphänomene auf der einen Seite (Guyton [1], Hardy [2], Urbaszek [3]) und mittel-bzw. langfristiger Kreislaufregulationsprozesse (Guyton [4]) auf der anderen Seite geeignet Modelle der erstgenannten Kategorie machen keine Aussagen über metabolische Parameter, wie die Sauerstoffaufhahme und die gemischtvenöse Sauerstoff-Sättigung unter körperlicher Belastung. Modelle der zweitgenannten Kategorie dagegen betrachten den pulsatilen Charakter des Herzzyklus nicht zeitaufgelöst («beat to beat»). Für die Zielsetzung der Analyse von Schrittmachersystemen mit unterschiedlichsten Sensorsystemen erscheint ein gekoppeltes Modell notwendig, das Elemente beider Kategorien in sich vereint
