ZusammenfassungWir berichten über eine 27-jährige I. Gravida in der 31. Schwangerschaftswoche, die nach einem Sturz vom Fahrrad stationär aufgenommen wurde. Von geburtshilflicher Seite bestanden keinerlei klinische Auffälligkeiten. Sonographie, Kardiotokographie sowie Aufnahmeuntersuchung waren ohne pathologischen Befund. Als Unfallfolge bot die Patientin eine Olekranonfraktur links und einen mehrfachen Kieferbruch. 70 Minuten nach Aufnahme der Patientin in den Kreiûsaal trat eine fetale Bradykardie auf, die die sofortige Sectio caesarea erforderlich machte. Das primär asphyktische Frühgeborene verstarb am 1. Lebenstag unter intensivmedizinischer Betreuung im nicht beherrschbaren Kreislaufschock. Die Todesursache des Kindes lieû sich erst durch die Obduktion klären. Nachgewiesen werden konnten massive intraabdominelle Einblutungen und eine Fraktur des Os ileum links. Da Unfälle während der Schwangerschaft nur in 5 ± 7% auftreten und unterschiedlichster Ausprägung sein können, gibt es keine einheitlichen Empfehlungen. Generell steht neben der Versorgung von maternen Verletzungen die Überwachung der Schwangerschaft im Vordergrund. Der vorliegende Fall zeigt, dass auch bei optimaler Betreuung auf Maximalversorgungsniveau fetale Blutungen solange okkult bleiben können, bis ein irreversibles Schockgeschehen eingesetzt hat. AbstractWe report on a 27-year-old primagravida in the 31st week of gestation, who was injured in a bike accident. Apart from fractures of the left olecranon and the jaw the pregnancy seemed intact and unharmed. However, 70 minutes after admission continuous registration of fetal heart rate showed bradycardia. A cesarean section was performed immediately. The asphyxiated preterm baby had to be reanimated and was transferred to the neonatal intensive-care unit. The baby died at the age of 1 day. The reason for the death of the newborn was found by the pathologist: severe intraabdominal bleeding, fracture of the left os ileum and multiple shock signs such as shock lungs, anemia and brain hemorrhage. Severe traumas occur fairly rarely during pregnancy (5 ± 7%). Because of the variety of the traumas, there are no general guidelines for diagnostic and treatment procedures.
