Aspects endocriniens de la fatigue chronique J Mirouze et E RenardLa fatigue, sous ses formes neuropsychique ou neuromusculaire, est une composante sémiologique de nombreuses pathologies. L'expérience clinique des maladies endocriniennes permet de retenir le rôle de perturbations hormonales dans sa constitution. Différents mécanismes physiopathologiques hormonodépendants sont impliqués, tant au niveau musculaire qu'au plan du système nerveux central pour concourir à la fatigue. Inversement, la fatigue chronique de nature endogène telle qu'observée dans les états dépressifs est à l'origine de modifications neuro-endocriniennes, confirmant ainsi l'étroite intrication des processus. La boucle d'interdépendance entre les désordres endocriniens et l'état de fatigue chronique conduit à envisager la possibilité de terrains propices à l'installation d'un tel cercle vicieux. L'approche génétique et l'analyse moléculaire de la biochimie cellulaire peuvent s'avérer des voies nouvelles pour appréhender la fatigue sur un mode uniciste plutôt qu'à travers ses différents aspects sectoriels attachés à un organe ou à un appareil.
