In family practice patients diagnosed with asthma, several treatment goals were better achieved with a lower dose of fluticasone and salmeterol in a combination product than with a higher dose of fluticasone. We found no differences between the two approaches for patients with COPD.
Achtergrond Er is eerder onderzoek gedaan naar de werkzaamheid van combinatiepreparaten met inhalatiesteroı¨den en langwerkende luchtwegverwijders (be`ta-2-agonisten) tegen astma en chronische obstructieve longziekte (COPD), maar dit onderzoek is niet representatief voor patie¨nten in de huisartsenpraktijk.Doel Wij wilden in de huisartsenpraktijk onderzoeken of een combinatie van salmeterol met een lage dosis fluticason beter werkt bij patie¨nten met astma of COPD dan een conventionele behandeling met een hogere dosis enkelvoudig fluticason.Methoden Ons onderzoek was een gerandomiseerd, gecontroleerd dubbelblind onderzoek in 41 huisartsenpraktijken, onder in totaal 137 patie¨nten met astma en 40 patie¨nten met COPD. De onderzoeksduur was 12 weken. Patie¨nten mochten naast hun onderzoeksmedicatie een kortwerkende luchtwegverwijder gebruiken. Onze primaire effectmaat was het geforceerde expiratoire volume in 1 seconde (FEV 1 ) als percentage van de voorspelde waarde. Secundaire effectmaten waren de expiratoire piekstroom (PEF) in de ochtend, het aantal symptoomvrije dagen, de kwaliteit van leven, exacerbaties, gebruik van kortwerkende luchtwegverwijders en bijwerkingen.Resultaten De FEV 1 als percentage van de voorspelde waarde nam toe met 2,6% (SD 8,3) bij patie¨nten die met fluticason/salmeterol behandeld werden en met 0,01% (SD 6,6) Huisarts en Wetenschap (2007) 50:846-855
