Pachyonychia congenita (PC) is a rare ectodermal dysplasia with variable expression. The condition is usually inherited as an autosomal dominant trait. Several classifications of PC have been proposed. Feinstein and colleagues suggested four clinical types of PC. Type II, the Jackson-Lawler-Syndrome, is characterized by multiple epidermal cysts, palmoplantar bullae and hyperhidrosis as well as natal teeth in addition to the main findings of pachyonychia, palmoplantar hyperkeratosis and follicular keratosis. We report two patients (father and son) with Jackson-Lawler-Syndrome and describe in detail pathogenesis, diagnostic criteria and treatment approaches as well as the different classifications of pachyonychia congenita.

Aktinische Keratosen, die im internationalen Konsens als Carcinoma in situ eingestuft werden, k?nnen in ein Plattenepithelkarzinom ?bergehen und sollten ad?quat und fr?hzeitig behandelt werden. Bedauerlicherweise wird die Erkrankung oftmals erst im sp?ten Stadium ernst genommen, sodass die Patienten dann zum Teil aufwendige Operationen ?ber sich ergehen lassen m?ssen. Die Lokalbehandlung mit niedrigdosiertem 5-Fluorouracil (0,5?%) in Kombination mit Salizyls?ure (10?%) bietet die M?glichkeit einer rechtzeitigen, einfachen und kosteng?nstigen Therapieintervention mit guter Effizienz und Langzeiteffekt. Am Fall eines 73-j?hrigen Patienten, der sein gesamtes Berufsleben mit hautgef?hrdenden Keramikglasuren gearbeitet hat, kann eindrucksvoll gezeigt werden, wie effektiv eine Behandlung mit 5-FU/Salizyls?ure auch bei einem Plattenepithelkarzinom sein kann. Unser Fall zeigt, dass eine ausreichend fr?he konservative Behandlung aktinischer Keratosen sp?tere aufwendige Operationen verhindern kann.
EinleitungBeim Morbus Crohn, einer chronischen, entzündlich-granulomatösen Erkrankung des Gastrointestinaltraktes unklarer Genese, sind kutane und mukokutane Manifestationen häufig. In der Mehrzahl der Fälle werden diese durch eine perianale bzw. peristomale Ausdehnung der intestinalen Erkrankung verursacht. Preterm birth affects nearly 10% of all pregnancies in
the United States, with 40% of those due, in part, to infections. Streptococcus agalactiae (Group B Streptococcus, GBS) is one of the most common perinatal pathogens responsible
for these infections. Current therapeutic techniques aimed to ameliorate
invasive GBS infections are less than desirable and can result in
complications in both the neonate and the mother. To this end, the
need for novel therapeutic options is urgent. Human milk oligosaccharides
(HMOs), an integral component of human breast milk, have been previously
shown to possess antiadhesive and antimicrobial properties. To interrogate
these characteristics, we examined HMO-mediated outcomes in both in vivo and ex vivo models of GBS infection
utilizing a murine model of ascending GBS infection, an EpiVaginal
human organoid tissue model, and ex vivo human gestational
membranes. Supplementation of HMOs resulted in diminished adverse
pregnancy outcomes, decreased GBS adherence to gestational tissues,
decreased colonization within the reproductive tract, and reduced
proinflammatory immune responses to GBS infection. Taken together,
these results highlight the potential of HMOs as promising therapeutic
interventions in perinatal health.
