Primary intestinal tuberculosis is unusual in European and North American countries today. Its diagnosis is often surprising and differentiation from inflammatory bowel diseases is difficult. The authors present a rare case of severe stercoral peritonitis caused by multiple intestinal perforations in a patient with primary ileocecal tuberculosis. Initial clinical and laboratory investigations led to the suspicion of inflammatory bowel disease. The subsequent diagnostic workup included colonoscopic examination of the cecal and terminal region of the ileum with multiple biopsies. After the pathologist had assessed the specimen as indicating Crohn’s disease, appropriate therapy was initiated. Several days later, however, the patient was readmitted to a surgical intensive care unit with clinical signs of peritonitis and immediately operated on. The final diagnosis from a resection specimen confirmed the diagnosis of primary intestinal tuberculosis. The follow-up was complicated by a subhepatic abscess formation with the necessity for surgical drainage. The patient’s recovery was uneventful, she underwent intensive antituberculotic therapy and is asymptomatic at present. Surgeons caring for patients with acute abdomen should be aware of tuberculous perforation peritonitis even in non-risk groups of patients.

Background/Introduction We are presenting a 33-year-old patient with huge pseudoaneurysm formation of the ascending aorta, developing after type A acute aortic dissection repair.
Cílem této studie bylo posoudit výsledky dialyzovaných pacientů, kteří podstoupili operaci srdce. Retrospektivně jsme hodnotili 36 dialyzovaných pacientů s průměrným věkem 63 ± 9,4 roku, kteří podstoupili srdeční operaci. Operace typu našití aortokoronárního bypassu (CABG) byla provedena u 27 pacientů (75 %), izolovaná operace srdeční chlopně u dvou (5,5 %), kombinovaný výkon CABG a operace chlopně v pěti případech (13,8 %), kombinace operace chlopně a metody MAZE u jednoho pacienta, operace chlopně + CABG a metoda MAZE u jednoho pacienta, výkon na ascendentní aortě v jednom případě, sutura poraněné pravé komory u jednoho pacienta a exstirpace infi kovaných trombů z pravé síně u jednoho pacienta. Nemocniční mortalita dosahovala 11,1 %. Všechna úmrtí nastala u pacientů, kteří podstoupili urgentní výkon. Ke dvěma úmrtím došlo u pacientů, kteří podstoupili srdeční operaci za použití mimotělního oběhu (náhrada vzestupné aorty a odstranění infi kovaného trombu), dále zemřel jeden pacient, který prodělal suturu poraněné pravé komory a jeden pacient, který podstoupil konvenční revaskularizaci myokardu. Jednoleté přežívání bylo 77,8 %. Obecně navrhované prediktory zvýšené pozdní úmrtnosti jsou srdeční selhání, akutní operace, složitost chirurgických zákroků (chirurgie chlopní, kombinované výkony a výkony na ascendentní aortě) a pooperační syndrom nízkého srdečního výdeje. U pacientů závislých na dialýze má CABG přijatelné riziko. Výsledky u pacientů s postiženou chlopní v kombinaci s ischemickou chorobou srdeční jsou lepší při včasném načasování operace, a to ještě před nástupem příznaků srdečního selhání.
