Differential diagnosis of neck tumours puts precedence on diagnosing neoplastic lesions. In the case of neck tumours, these are unfortunately late signs, but in patients with a primary neoplastic focus within the head and neck, neck tumour is often the first sign of the disease. The authors describe a clinical case of neck tumour with initially unclear etiology. The preoperative diagnostics including ultrasonography, thin-needle puncture, MRI, carotid angiography and videostroboscopy was significant for surgical treatment planning; yet it was the intraoperative clinical picture which indicated that the tumour derived from the inferior parotid pole. The preoperative histopathological diagnosis using thin-needle biopsy: cellulae carcinomatosae and the clinical picture resulted in block operation with neck lymphatic system removal and tissue defect reconstruction by means of a pectoral flap. The histopathological examination confirmed non-cornifying basal cell epithelioma only in the essential lesion with no metastases to lymph nodes and surrounding tissue margins free of infiltrates. Two and a half years after the procedure, the patient presented with a tumour localized on the front thoracic wall and two rapidly enlarging tumours in the nape of the neck. In the collected specimen of the tumour on the front thoracic wall, a diagnosis of acinic cell carcinoma was made. The deteriorating general condition of the patient, his cancer emaciation, problems with movement and suspected liver metastases confirmed by abdominal USG as well as radiological bone lesions resulted in the abandonment of surgical treatment due to disseminated neoplastic process and the patient was referred for palliative care.
Cystic hygromas are very uncommon malformations in adults. In most of the cases, they present as a painless, fluctuant swelling, usually located in the neck region. They tend to grow extensively if not removed. Cystic hygromas can range from 1 mm to several centimeters in size and are filled with clear- to straw-colored fluid. Surgical treatment remains the gold-standard treatment for these tumors. In some cases they can be misdiagnosed with branchial cysts, swollen lymph nodes or other pathologies. When this malformation is present in the prevertebral fascia its surgical treatment is challenging, and some Authors advise its conservative treatment, which is in a great risk of its reoccurence. Because of potential malignant transformation in some percentage of branchial cyst cases, a similar situation was suspected. The present report describes a case of cystic hygroma of the left side of the neck in a 92-year-old female adult and its clinical, radiologic and operative features.

Pacjenci świadomi swojej wady gnatycznej starają się znaleźć jak najwięcej informacji dotyczących sposobów jej korygowania. Operacje wiążą się z dużą zmianą w życiu pacjenta na gruncie fizycznym i psychicznym. Informacje znalezione na forach internetowych często kształtują postępowanie pacjenta zarówno przed zabiegiem operacyjnym, jak i po nim. Cel pracy. Analiza postrzegania wad gnatycznych oraz indywidualnej oceny wpływu zabiegów ortognatycznych u pacjentów operowanych i przygotowujących się do operacji na podstawie korelacji obaw i oczekiwań pacjentów przed zabiegiem z opiniami na temat rezultatów operacji wyrażonymi przez grupę osób po zabiegu. Materiał i metody. Badane osoby były użytkownikami forum internetowego podejmującego zagadnienia róż-nych wad twarzoczaszki (www.progenia.org). Pacjentów podzielono na dwie grupy: I -pacjenci przed zabiegiem: 80 osób (69 K, 11 M) oraz II -pacjenci po zabiegu: 50 osób (43 K, 7 M). Do analizy wyników użyto elektronicznego, anonimowego badania ankietowego użytkowników internetowego forum, wykorzystującego autorskie kwestionariusze badawcze. Wyniki. Najbardziej negatywnym skutkiem posiadania wady gnatycznej jest deformacja twarzoczaszki w obrę-bie piętra dolnego (56% K, 71% M z grupy II), co jest główną przyczyną skłaniającą do poddania się zabiegowi. Większość pacjentów z wadami gnatycznymi przyznaje, że jest przez to mniej pewna siebie (28% K, 39% M). Ankietowani (65% K, 64% M) twierdzą, że leczenie zmieni znacząco ich życie. Po zabiegu zwiększa się poczucie atrakcyjności, samoocena. Dochodzi do wielu zmian w układzie stomatognatycznym. Wnioski. Pacjenci o swojej wadzie zgryzu dowiadują się głównie od lekarza. Od operacji oczekują poprawy wyglądu i funkcjonowania. Większość obawia się pooperacyjnych parestezji. Ponad 50% ankietowanych wskazuje na dobry przebieg rekonwalescencji. Oczekiwania odnośnie do zabiegu spełniły się u większości ankietowanych. Dominują pochlebne opinie na temat rezultatów leczenia. Aż 82% przyznaje, że warto było poddać się leczeniu i poleca je pacjentom z podobnymi wadami (Dent. Med.
