Die diabetische Fußerkrankung ist die komplexeste Diabetes-assoziierte Folgeerkrankung an der unteren Extremität bei Menschen mit Diabetes mellitus. Eine multifaktorielle Pathogenese ist der Hintergrund dieses Krankheitsbildes, welches häufig zu chronischen Wunden führt und hierüber zu Minor-und auch zu MajorAmputationen. Aus Sicht des Gesundheitswesens gibt es oft lange Wege, bis ein Patient mit einer diabetischen Fußerkran-kung eine qualifizierte Behandlung erhält (s. Infobox 1). Die Prävalenz eines diabetischen Fuß-syndroms (DFS) liegt bei bis zu 15% aller Diabetespatienten, die jährliche Inzidenz einer diabetischen Fußläsion zwischen 2% und 7,5% der Diabetespatienten, abhängig davon, ob Diabetespatienten ohne oder mit Polyneuropathie und/oder arterieller Verschlusskrankheit (AVK) als Gesamtgruppe zugrunde gelegt werden. Weiterhin werden etwa 70% aller Amputationen bei Menschen mit Diabetes mellitus durchgeführt -in Zahlen ausgedrückt werden jährlich bis zu 40.000 Amputationen durchgeführt, wovon etwa die Hälfte oberhalb des Sprunggelenks erfolgt. Kommen Fußulzera unter der Behandlung zur Abheilung, neigen diese Patienten häufig zu Rezidivläsionen: etwa 34% nach einem Jahr, 70% nach 5 Jahren. Aber auch nach Amputationen ist innerhalb von 5 Jahren bei etwa 50% der Betroffenen mit einer erneuten Amputation zu rechnen. > Etwa 70% aller Amputationen werden bei Menschen mit Diabetes mellitus durchgeführt Das sind Grundlage und Rationale zur Entwicklung spezialisierter Fußbehand-lungseinrichtungen für Menschen mit diabetischer Podopathie. 1989 wurde in der Deklaration von St. Vincente die Halbierung der Amputationszahlen bei Menschen mit Diabetes (innerhalb eines Zeitraums von 5 Jahren!) gefordert -es hat sich jedoch bis heute offensichtlich nichts geändert.Die wesentlichen Grundlagen der Ätio-logie (Polyneuropathie, periphere arterielle Verschlusskrankheit, Mischbild, Fußde-formierungen) wie auch der Pathogenese und der Behandlung sind bekannt. Auch Neuerungen im Bereich der Wundbehandlungen, der Effektivität der Gefäßinterven-tionen oder der positiven Entwicklung der ambulanten Diabetologie haben keinen dokumentierten Effekt auf die Therapieergebnisse beim Patienten mit seiner chronischen Fußerkrankung ergeben.
