Het door Kris voorgestelde onderscheid tussen divergerende en convergerende intrapsychische conflicten verheldert de rol van steun bij inzichtgevende psychoanalytische therapievormen. Bij patie¨nten die worstelen met een dilemma, bestaat er bij divergerende conflicten een karakteristieke bron van weerstand tegen de therapie en verandering. In tegenstelling tot convergerende conflicten komen divergerende conflicten niet tot oplossing door het opheffen van de verdringing door duiding, maar door een proces dat lijkt op rouw waarbij een steunende, actieve therapeutische houding vaak noodzakelijk is. InleidingHet doel van psychoanalytische psychotherapie is het bewerkstelligen van inzicht en structurele verandering. Dit kan voor de patie¨nt een moeilijk en pijnlijk proces zijn, omdat oude zaken moeten worden losgelaten om ruimte te maken voor nieuwe. Zoals Judith Herzberg (1986) in een gedicht schreef: 'Zoveel wordt bij het winnen ook verloren'.Soms wordt men als therapeut indringend geconfronteerd met de pijn van de patie¨nt. Dit is bijvoorbeeld het geval wanneer zich bij de patie¨nt in de loop van de therapie een dilemma ontwikkelt; het gevoel van een onmogelijke keuze tussen het oude bekende waar hij gehecht aan is en het nieuwe onbekende waar hij naar verlangt. De affecten waarmee dit gepaard gaat kunnen leiden tot grote weerstand tegen de therapie, en meer in het bijzonder tegen de klassieke psychoanalytische grondhouding die in een dergelijk geval als onvoldoende steunend en betrokken ervaren wordt. Deze weerstand kan zo groot zijn dat de therapie in een impasse raakt.In een dergelijke situatie kan bij de therapeut de vraag opkomen of er met het volhouden van een abstinerende, afwachtende therapeutische opstelling op dat moment niet meer verloren dreigt te gaan dan ermee gewonnen kan worden. Aan de andere kant, op grond waarvan valt een goed therapeutisch resultaat van een meer actieve en steunende benadering te verwachten?Voorbeeld 1 »Sinds een jaar is de heer A. in inzichtgevende psychoanalytische psychotherapie. Tijdens de behandeling ontstaat een moeilijk probleem: hij is verliefd op twee vrouwen, Sophie en Marie. De eerste is zijn ex-vriendin, waar hij zeven jaar lang een moeizame relatie mee had en die hij nu eens per week ziet. Marie is een jonge collega met wie hij enkele malen gevrijd heeft. De heer A. vindt dat hij moet besluiten met welke vrouw hij zijn toekomst op zal bouwen, maar hij weet niet met wie. Het probleem wordt binnen enkele weken een dilemma dat hem geheel in beslag neemt.
