Legros Un pavé dans la mare… C'est très certainement ce que la plupart des endocrinologues traitant le déficit androgénique masculin auront comme « sensation » en lisant cette excellente étude supervisée par le célèbre et très sérieux Shalender Bhasin de Boston ! En effet, plusieurs travaux parus ces dernières années démontraient, à des degrés divers, l'effet favorable d'un traitement par androgènes sur la fonction cardiaque, en particulier chez des patients âgés présentant une décompen-sation cardiaque chronique associée à un hypogonadisme (Caminiti, et al. (2009) J Am Coll Cardiol 54:919-27). Cette démonstration pouvait constituer un argument de plus pour traiter le late onset hypogonadism (LOH), parfois appelé « déficit androgénique lié à l'âge » (DALA) en bon français, voire à faire de la testostérone (T) « …a novel therapeutic approach in chronic heart failure », selon l'éditorial de Aukrust et al. dans le même numéro de la revue (J Am Coll Cardiol (2010) 54:928-9) ! L'étude du groupe de Bhasin est réalisée en double insu sur 209 patients de plus de 65 ans en « maison de repos » présentant un déficit androgénique (testostérone libre [fT] inférieure à 50 pg/ml, soit 173 pmol/l) et soumis pendant une période programmée de six mois, soit à un traitement quotidien par 10 g d'un gel transdermique contenant 100 mg de T, éventuellement portés à 15 g pour atteindre le niveau de T visé, soit un placebo. Les auteurs ont dû arrê-ter prématurément l'inclusion des patients à la demande du comité de supervision suite à l'apparition d'un nombre significativement plus élevé « d'événements cardiovasculaires et dermatologiques » dans le groupe traité que dans le groupe témoin ! Dans le même numéro, le groupe du non moins célèbre et sérieux Frederick Wu (Manchester) (Wu et al. (2009) New Engl J Med 363:123-35) précisait la définition clinique (au moins trois symptômes) et biologique du LOH : T infé-rieure à 11 nmol/l (3,2 ng/ml) ou fT inférieure à 220 pmol/l (64 pg/ml). La simultanéité de la publication de ces deux travaux a amené William J. Bremmer à écrire un éditorial dans le même numéro (New Engl J Med 363:189-91) s'interrogeant sur l'intérêt et surtout l'innocuité du traitement androgénique du DALA ! Le but initial de l'étude de Basaria et al. était de tester l'efficacité d'une supplémentation en T chez ces patients
