L'hémangiome intramusculaire (HIM) est une tumeur vasculaire bénigne touchant surtout l'enfant et l'adulte jeune. La localisation aux membres inférieurs est la plus fréquente, mais l'atteinte du pied est exceptionnelle, avec seulement quelques cas rapportés dans la littérature. Nous rapportons le cas d'un HIM des muscles interosseux du quatrième espace intermétatarsien du pied chez un patient de 24 ans traité par exérèse chirurgicale.

Although lower extremities are the commonest localization, the localization at the foot is extremely rare since only a few cases have been reported. We report a case of cavernous hemangioma of the interosseous muscles in a 24-year-old man who was treated with surgical excision.
La prise en charge des fractures trimalléolaires nécessite une attention particulière pour le fragment marginal postérieur (FMP). Le but de notre travail est de montrer l'intérêt de la voie d'abord postérolatérale dans ce type de fracture à travers une série prospective de 22 fractures trimalléolaires à FMP volumineux colligées sur une période de deux ans. L'âge moyen des patients était de 39 ans, le sexe masculin était atteint dans 80 % des cas, les étiologies étaient dominées par les accidents de la voie publique. Nous avons noté une prédominance des fractures intertuberculaires. Les patients étaient installés en décubitus ventral ou latéral afin de pratiquer un abord postérolatéral pour la réduction et l'ostéo-synthèse respectivement de la malléole fibulaire puis du FMP. Selon les critères de Vidal, les résultats étaient bons dans 72,7 %, acceptables dans 18,1 % et mauvais dans 8,2 % des cas. L'ostéosynthèse de la malléole postérieure est nécessaire quand elle touche 25 % de la surface tibiale. L'utilisation de cet abord postérolatéral nous a permis une bonne exposition pour obtenir la réduction et l'ostéosynthèse des malléoles fibulaire et postérieure. Pour citer cette revue : Méd. Chir. The purpose of this paper is to study the effect of the posterolateral approach for this type of fracture. We performed a prospective study of 22 cases over 2 years. The average age of the patients was 39 years; men were affected in 80% of the cases. Traffic accidents were found to be the principal cause of the fracture. We noted a predominance of intertubercular type fracture. For the posterolateral approach, the patient was positioned in posterior or lateral position. This approach permits good exposition, reduction and osteosynthesis of the lateral malleolus and later for the posterior marginal fragment. According to the Vidal ankle score, clinical and radiological results were good in 72.7 %, acceptable in 18.1 % and bad in 8.2% of cases. To cite this journal: Méd. Chir. Pied 27 (2011).
