Despite the fact that patients and parents were informed about wear time recording, compliance was insufficient with regard to functional treatment (14 h/d prescription), while it was sufficient for retention purposes (8 h/d prescription). Objective measures are necessary to assess compliance with removable orthodontic appliances since patient compliance is a highly variable issue.
SUMMARY The effect of a lip bumper on the dimensions of the lower dental arch and on the inclination of the incisors and first molars was studied in 40 children, aged 9-12 years. The children wore their lip bumper full time for 7-10 months.The effects of the lip bumper therapy were recorded on dental casts and profile cephalograms made before and after the treatment. The positions and stages of development of the lower second molars were determined on pretreatment intra-oral radiographs.The lip bumper treatment resulted in an increase of the dental arch widths between the molars, premolars, and canines. The arch length increased through proclination of the incisors and uprighting of the first molars. The stages of development and the positions of the second molars had no influence on the effect of the treatment. Simultaneous treatment in the maxilla with a headgear, a transpalatal arch or a removable plate had no influence on the outcome of the lip bumper therapy.
The purpose of this study was to examine the success rate of paramedian palatal Orthosystem first- and second-generation implants used for anchorage in orthodontic treatment in patients treated by one experienced orthodontist. The records of 143 patients (90 female, 53 male, median age: 15.7 years, range: 10.2-50.9) receiving 145 palatal implants of the first or second generation (Orthosystem, Straumann AG, Basel, Switzerland) were examined. All the palatal implants were placed in a paramedian palatal location by three experienced surgeons. Stable implants were orthodontically loaded after a healing period of 3 months. Out of the 145 inserted paramedian palatal implants only seven implants (4.8%) were not considered stable after insertion. All the successfully osseointegrated implants remained stable during orthodontic treatment. Paramedian palatal implants are highly reliable and effective devices to obtain skeletal anchorage for orthodontic treatment. This study has shown that the paramedian location is a good alternative to the median location.
▼ Ziel: Das Ziel dieser Studie ist die Vielfalt der Anwendungsmöglichkeiten von paramedianen Gaumenimplantaten zur Verankerung in der Kieferorthopädie aufzuzeigen. Methoden: Es wurden die Unterlagen von 143 Patienten untersucht, die von einem erfahrenen Kieferorthopäden mittels fi xen Apparaturen und paramedianem Gaumenimplantat behandelt wurden. Zudem wurde eine umfassende Literatursuche durchgeführt, um weitere mögliche klinische Indikationen festzuhalten. Resultate: Es gibt ein breites Spektrum von Behandlungen, wo paramedian gesetzte Gaumenimplantate klinisch eingesetzt werden können. Die Anwendungsmöglichkeiten können in 2 Hauptkategorien eingeteilt werden: passive und aktive. Passive Anwendung beinhaltet die Fixierung von Zähnen für maximale Verankerung, temporärer Ersatz von fehlenden oder geschädig-ten Zähnen im Oberkiefer, Retraktion der Oberkieferfront, Korrektur eines off enen Bisses und Lückenschluss nach Extraktionen von Oberkieferzähnen (uni-oder bilateral). Aktive Anwendung beinhaltet die Distalisierung von Bukkalsegmenten (uni-oder bilateral), Verankerung für Distalisierungsgeräte (wie Pendulum oder Distal Jet) und Intrusion oder Extrusion von Oberkieferzähnen. Schlussfolgerung: Paramediane Gaumenimplantate sind sehr verlässliche und eff ektive Hilfsmittel um skelettale Verankerung für kieferorthopädische Behandlungen zu generieren. Ihr grosser Anwendungsbereich kann das derzeitige Behandlungsspektrum der Kieferorthopädie erweitern, indem es dem Behandler eine komplette Kontrolle der Verankerung in allen 3 Dimensionen und eine Reduktion der unerwünschten Nebenwirkungen garantiert. Abstract ▼ Aim:The purpose of the present study was to demonstrate the various clinical applications of paramedian palatal implants used for anchorage in orthodontic treatment. Methods: Records of 143 patients treated with orthodontic fi xed appliances and paramedian palatal implant by an experienced orthodontist were retrieved. A comprehensive literature search was also conducted in order to identify further possible clinical indications. Results: Paramedian palatal implants can be used in a large spectrum of clinical applications. These applications can be divided in 2 major categories: passive and active. Passive applications include their utilization for temporary replacement of missing or traumatised teeth in upper jaw and anchorage reinforcement in order to achieve clinical goals such as: retraction of the anterior segment, open bite correction, space closure after extraction of upper teeth (uni-or bilateral). Active applications include distal movement of buccal segments (uni-or bilateral), anchorage for distalizing devices (such as Pendulum or Distal Jet) and teeth intrusion or extrusion. Conclusions: Paramedian palatal implants are highly reliable and eff ective devices to obtain skeletal anchorage for orthodontic treatment. Zusammenfassung Die Behandlung erwachsener Patienten unterscheidet sich in einigen wesentlichen Punkten von jener adoleszenter Patienten. Fehlendes Kieferwachstum, Kiefergelenksprobleme, Systemerkrankungen mit entsprechender Dauermedikation, Zahnsubstanzverlust, ausgedehnte Restaurationen, Zahnimplantate und reduziertes Parodont mit oder ohne aktive Parodontitis sind einige dieser Faktoren, die spezielle Ber?cksichtigung in der Therapieplanung verlangen. Sie beeinflussen sowohl die pr?therapeutische als auch die kieferorthop?dische Phase. Vor allem die Ber?cksichtigung der parodontalen Verh?ltnisse ist eine Grundvoraussetzung f?r eine erfolgreiche Therapie. Eine aktive Parodontitis muss vor Therapiebeginn behandelt sein und reizlose Verh?ltnisse w?hrend der gesamten aktiven Behandlung aufrechterhalten werden. Ein reduzierter Zahnhalteapparat ver?ndert die biomechanischen Verh?ltnisse des Zahn-Parodont-Alveolarknochen-Systems und erfordert eine ad?quate Reduktion der wirkenden Kr?fte. Unter Beachtung dieser Faktoren ist auch bei parodontal kompromittierten Patienten eine kieferorthop?dische Behandlung ohne zus?tzlichen Attachmentverlust m?glich, wie der vorliegende Fallbericht zeigt.
