It was the objective of the DAL study HD-85 to examine the possibility of a reduction of chemotherapy, compared to the study HD-82 which had applied a combined treatment strategy resulting in a relapse rate of only 2.5% after 4 years, and a proportion of intercurrent deaths of 1.5%, in a total of 203 patients. Procarbazine was eliminated in the OPPA-therapy (OPA) and replaced by methotrexate in the COPP-cycles (COMP). Chemotherapy consisted in 2 cycles of OPA for stage I/IIA (group 1), 2 cycles of OPA and 2 cycles of COMP for stage IIB/IIIA (including IEA/IIEA) (group 2), and 2 cycles of OPA and 4 cycles of COMP for stage IIIB/IV (group 3). The subsequent radiotherapy was limited to the initially involved fields, the dosage of 35, 30 or 25 Gy depending on the extent of chemotherapy. Regions showing incomplete tumor regression after chemotherapy received another 5 Gy, in patients of groups 2 and 3. Between Jan. 1985 to Nov. 1986, 103 children with Hodgkin's disease from 42 participating hospitals were enrolled in this study. 98 patients were treated according to protocol (59 boys, 39 girls). A specific strategy for the selective indication of laparotomy and splenectomy was applied. With this strategy no laparotomy was performed in 39/98 patients (40%). 67 patients (68%) retained their spleen. 4 patients showed progression of the disease under chemotherapy, all others achieved remission. 16 patients relapsed before Dec. 31, 1987. Sekretorische Diarrhö als Leitsymptom eines Ganglioneuroblastomsbolischer Azidose und Osteopenie ursächlich ein VIP-sezernierendes Ganglioneuroblastom der Nebenniere diagnostiziert werden konnte. FallberichtDie Patientin fiel erstmalig mit 2 1 / 2 Jahren durch Müdigkeit, Knochenschmerzen, hohe Trinkmengen bis 3 l/Tag sowie eine Wachstumsverzögerung auf. Im Alter von fast 4 Jahren kam es im Rahmen eines fieberhaften Infekts zur akuten lebensbedrohlichen Dehydratation. Es zeigten sich eine ausgeprägte Dyselektrolytämie, metabolische Azidose und renale Insuffizienz (K + 1,4 mmol/l, Na + 131 mmol/l, Ca 2+ 1,0 mmol/l; pH 7,0, Base excess −22,6 mmol/l, Kreatinin 3,2 mg/dl). Nach initialer Rehydratation und Ausgleich der Dyselektrolytämie betrug die täglich notwendige Flüssigkeitszufuhr durchschnittlich ca. 5 l, wobei für den hohen Flüssigkeitsbedarf wäßrige Durchfälle verantwortlich waren, die mit ausgeprägten Elektrolytverlusten einhergingen (täglicher Bedarf an Na + und K + jeweils ca. 25 mmol/kg, Cl − ca. 15 mmol/kg, Ca 2+ ca. 2 mmol/kg, Bikarbonat ca. 10 mmol/kg, wobei die Elektrolyte parenteral zugeführt wurden). Es lag Verner u. Morrison [28] beschrieben im Jahr 1958 wäßrige Durchfälle mit Hypokalämie und Dehydratation bei erwachsenen Patienten mit Inselzelltumoren. Das aus 28 Aminosäuren bestehende vasoaktive intestinale Polypeptid (VIP) wurde 1970 von Said u. Mutt [19] aus dem Dünndarm von Schweinen isoliert. 1973 etablierten Bloom et al. einen Zusammenhang zwischen einer Erhöhung des VIP und dem Auftreten des sog. WDHA-Syndroms [15] (wäßrige Durchfälle, Hypokalämie, A-oder Hypochlorhydrie) bei erwachsenen Patienten [3]. Der erste Bericht über ein Kind mit VIPassoziierter Diarrhö stammt aus dem Jahr 1975 [23]. Bisher wurden insgesamt etwa 75 Fälle beschrieben, bei denen sich ein VIP-sezernierender Tumor im Kindesalter manifestierte [27].Während das WDHA-Syndrom im Erwachsenenalter vorwiegend im Rahmen von VIPsezernierenden Tumoren des Pankreas beobachtet wird, ist im Kindesalter bei dieser Symptomatik vorwiegend an Tumoren des sympathischen Nervensystems zu denken [8].Als zentraler pathophysiologischer Mechanismus wurde eine VIP-induzierte, cAMP-vermittelte Stimulation der Elektrolyt-und Flüssigkeitssekretion in den Darm bei Hemmung der Magensäuresekretion nachgewiesen.Wir berichten über ein 4jähriges Mädchen, bei dem im Rahmen der diagnostischen Abklärung einer sekretorischen Diarrhö mit Hypokalämie, meta-
