Chronic granulomatous disease (CGD) is an inherited disorder characterized by the inability of phagocytes to generate normal amounts of superoxide (O2-), leaving patients susceptible to life-threatening infections. It was previously assumed that once carriers of the X-linked form of CGD were found to have 30% or more of functionally normal neutrophils, they would be free of risk for infection because the lyonization ratio was believed to be constant. Our report strongly contradicts this assumption. A 45-year-old X-CGD carrier had approximately 40% of normal neutrophils in her peripheral blood at age 21 years. Recently, she contracted a life-threatening pulmonary infection with Aspergillus fumigatus. After recovery, the ratio of normal-to-nonfunctional neutrophils was re-evaluated. She was found to have only 6-8% of normal neutrophils, suggesting that a striking decrease in the number of normal cells over the past 25 years was the reason for an increased susceptibility to Aspergillus infection. We conclude that age-related acquired skewing of the lyonization ratio can result in an increased susceptibility to life-threatening infections in X-CGD carriers.
Zusammenfassung
Hintergrund Die Aufmerksamkeit für ökologische Aspekte als Teil des Gesundheitsschutzes nimmt bei Ärzten und Patienten zu. Dazu gehört auch der CO2-Fußabdruck durch die Verordnung von Medikamenten. Das Treibhauspotenzial inhalativer Therapien hängt von der Art des Inhalationsgeräts ab. Die noch häufig verwendeten treibgashaltigen Dosieraerosole (pMDI) haben einen 10- bis 40-fach höheren CO2-Fußabdruck im Vergleich zu treibgasfreien Pulverinhalatoren (DPI).
Zielstellung Ziel der vorliegenden Arbeit war es, die Umsetzbarkeit und die Relevanz der Umstellung von pMDI auf DPI hinsichtlich der Verringerung des CO2-Fußabdrucks in einer pneumologischen Praxis unter Alltagsbedingungen zu untersuchen und eine Kostenabschätzung zu treffen.
Methode Ausgehend von exemplarischen Therapieregimen verschiedener
Intensität für drei Patienten einer pneumologischen Gemeinschaftspraxis (pGP) wurden der
CO2-Fußabdruck und die Tagestherapiekosten untersucht. Auf Basis der Verordnungen
des 1. Quartals 2020 (I/2020) und des 1. Quartals 2021 (I/2021) in dieser pGP wurde im
Vergleich zum Verordnungsverhalten der Pneumologen in Sachsen und deutschlandweit ermittelt,
in welchem Umfang und mit welchen Effekten auf den CO2-Fußabdruck eine Umstellung
der Behandlung von pMDI auf DPI möglich ist.
Ergebnisse Für exemplarische Therapieregime bei Asthma- bzw.
COPD-Patienten wurde bei Umstellung einer pMDI-basierten auf eine DPI-basierte Therapie in
Abhängigkeit von der Therapieintensität sowie des verwendeten Treibgases ein Einsparpotential
von 115–480 kg CO2-Äquivalent (CO2e) pro Jahr und Patient ermittelt. Bei
2.610 (I/2020) und 2.693 (I/2021) Behandlungsfällen wurden in der Praxis 184.297 bzw. 164.165
definierte durchschnittliche Tagesdosen (DDD) rezeptiert. Der Anteil der DPI konnte von 49,2%
(I/2020) auf 77,8% (I/2021) erhöht werden. Insbesondere in der Monotherapie mit inhalativen
Kortikosteroiden (ICS) war die Zunahme der DPI-Verordnungen (von 19,8 auf 74,1%) deutlich. Es
konnten in der pGP im Vergleich der beiden ersten Quartale 2020 und 2021 35.000–40.000 kg
CO2e eingespart werden. Eine Kostensteigerung war im Vergleich zum
Bundesdurchschnitt nicht zu beobachten. Bei den Pneumologen in Sachsen und deutschlandweit
fand keine wesentliche Veränderung im Verordnungsverhalten statt. Wenn die ambulant tätigen
Pneumologen bundesweit 75% DPI verordneten, wäre eine Einsparung von 11.650 Tonnen
CO2e pro Quartal bzw. 46.600 Tonnen CO2e pro Jahr möglich.
Schlussfolgerung Die inhalative Behandlung von Patienten mit Asthma bronchiale und COPD kann unter Alltagsbedingungen weitgehend von pMDI auf DPI umgestellt werden. Eine wesentliche Einsparung von Treibhausgasemissionen ist ohne relevante Kostensteigerung möglich.
Background: In agricultural meat production, adding enzymes such as phytase to animal feed is widespread, but there is little awareness of the allergenic potential and health risks of these fungal enzymes. Patients and Methods: We report on eight patients working in a plant producing phytase granulates. All patients complained about work-related rhinitis occurring within six months of the onset of exposure to phytase dust. Asthmatic symptoms and contact urticaria also occurred. To detect sensitizations to phytase, skin prick-, patch-, and basophil activation test were carried out with the factory product. Levels of IgE and IgG against phytase were also measured. Results: There was a positive reaction to phytase with skin prick testing in seven of the eight patients. IgE specific to phytase was detectable in four of the eight patients, and IgG specific to phytase was detectable in six of the eight patients. The basophil activation test was positive in four out of seven patients tested, but the patch test was negative in all patients tested. Transfer to a different workplace with no exposure to phytase completely eliminated the symptoms. Conclusions: Mold enzymes such as phytase are highly potent occupational allergens. Occupational safety measures must be strictly implemented in order to protect the health of workers.
The performance of pressure- and volume controlled ventilators used for invasive and non-invasive ventilation in the home were tested on a patient lung model. In order to determine the influence of tidal volume preset, breathing rate, resistance, compliance and leakage to the variability of delivered tidal volume and peak airway pressure a factorial plan with adapted analysis of variance was used. The influence of tidal volume preset, compliance and leakage to the delivered tidal volume is significant. The peak airway pressure depends hardly on the influence factors. All tested ventilators meet the legal demands. But in some clinical situations there are considerable deviations of the breathing parameters depending on the brand. In conclusion ventilators of different brands are not interchangeable.
