Objetivo: Compreender as experiências de usuários em relação ao acesso a medicamentos após a ocorrência de alta hospitalar. Materiais e método: Estudo de caso qualitativo fundamentada na sociologia compreensiva. Os participantes foram nove pacientes internados em um hospital geral e sete acompanhantes responsáveis pelo oferecimento do cuidado. A coleta de dados ocorreu por meio de entrevistas utilizando roteiro semiestruturado, seguido da realização de análise de conteúdo. Resultados: Foram identificadas duas categorias denominadas “A descontinuidade da farmacoterapia" e "A desarticulação da informação como uma barreira no acesso aos medicamentos". As experiências relatadas pelos usuários demonstraram a existência de um pós-alta complexo permeado pela prescrição de medicamentos não disponíveis na rede ou o desabastecimento nas farmácias das Unidades Básicas de Saúde e outras necessidades no processo de cuidado que competem com a busca pelo medicamento. A não identificação do usuário e de suas necessidades como objeto central no processo de fornecimento das informações, além da promoção de informações incompletas e/ou não adequadas, também emergiram nos relatos. Conclusão: Apesar da existência de um sistema universal de saúde no Brasil, os achados apontam, no local em estudo, para fatores que fragilizam o acesso aos medicamentos e tornam a transição do cuidado um processo delicado.
