Objective:To analyze nurses' knowledge and practices regarding pain management of newborns admitted to Neonatal Intensive Care Units. Method: A descriptive and crosssectional study. Data were collected from 51 nurses based on an adapted questionnaire aimed at evaluating knowledge and practices regarding the management of neonatal pain in six hospitals in Curitiba and its Metropolitan Region. Results: For most nurses (86.0%), neonates feel pain. A total of 34.7% of the nurses reported never using pain assessment scales. Pain management was recorded by 84.3% of the nurses. Administered pharmacological measures were Paracetamol and Fentanyl (47.1%) and Morphine (17.6%); while non-pharmacological measures adopted were sweetened solution (68.6%), non-nutritive sucking (58.8%) and positioning (56.9%). Conclusion: Nurses considered neonatal pain a real event; however, they do not perform pain assessment or treatment of newborns in a systematized way. It is necessary to implement knowledge translation strategies in order to improve pain management in newborns.
Objective: To report a case of neonatal appendicitis in a children's hospital in southern Brazil, demonstrating the impact on neonatal survival. Method: Case study with data collection from medical records, approved by the Institution and Ethics Committee for Research with Human Beings. Results: The clinical picture is initially characterized by food intolerance, evolving to hypoactivity, alteration of vital signs and septicemia due to intestinal perforation. Management is exclusively surgical, since no case described in the literature was diagnosed preoperatively and the findings usually point to acute abdomen. Conclusion: A focused clinical surveillance should be established when the infant presents peritoneal irritation. Follow-up of the evolution and the worsening of the symptoms by nurses, as part of the care team in partnership with the medical team, enables an early surgical intervention, thereby avoiding complications such as septicemia and death. Peripherally inserted central catheter: nursing practices in neonatal intensive careObjective: To identify nursing knowledge regarding the insertion and maintenance of the peripherally inserted central catheter in a Neonatal Intensive Care Unit. Method: Exploratory descriptive quantitative study carried out in a pediatric reference hospital in May and June 2019. Data analysis using the SPSS program (21.0). Results: Seventy-eight percent of nurses received training at the institution, 100.0% performed access flushing with saline, positive pressure and pulsating technique and all considered as critical points erythema, pain, secretion at the insertion site and obstruction of the device. Conclusion: The role of the nurse is fundamental in the diligence with the peripherally inserted central catheter, acting as an educator of the nursing team to promote qualification and adherence to institutional protocols continuously.Descriptors: Newborn; Central venous catheterization; Neonatal intensive care units; Neonatal nursing. Objective: To describe the implementation of Modified Seldinger Technology for percutaneous catheterization in critically ill newborns. Method: A quasi-experimental before- and-after study, carried out with neonatologist nurses in a Neonatal Intensive Care Unit. Results: Seven nurses participated in the research. Catheter pre-insertion, insertion and maintenance were assessed using the conventional and modified Seldinger technique. Reliability was satisfactory in pre-test, 5.40 (Md = 6.00), and post-test, 5.94 (Md = 7.00), and perfect in the items about device insertion and maintenance. There was low assertiveness in the items on indication, microintroduction procedure via ultrasound, limb repositioning and disinfection of connections/connectors. Conclusion: Despite the Modified Seldinger Technique expanding some stages of execution over the traditional method of percutaneous catheterization, nurses were more assertive after theoretical-practical training. The technology was implemented and is in the process of being implemented in the health service.
