Backgrounds. The incidence of molar pregnancies has increased during the COVID-19 pandemic. Molar pregnancy can cause hyperthyroidism and if not treated immediately can worsen the patient's condition. Molar pregnancy with hyperthyroidism infected with SARS-CoV-2 pose a challenge for anesthesiologists in perioperative management to prevent complications of thyroid crisis and worsening of COVID-19. Case presentation. A 38-year-old woman was referred to our hospital with a gestational age of 11-12 weeks with vaginal bleeding. Since 2 weeks before being admitted to the hospital the patient complained of fever and the last 3 days had a cough and runny nose. She had no history of contact with COVID-19 patient. On physical examination, her blood pressure was 160/90 mmHg, heart rate was 114 beats per minute, respiratory rate was 18 times per minute, SpO2 was 97% on room air, and body temperature was 37.4oC. Laboratory tests revealed hemoglobin levels of 9.2 g/dl, hCG levels > 1,000,000 mIU/ml, and thyroid function tests showed hyperthyroidism. Obstetric ultrasound examination revealed a molar pregnancy. A Burch-Wartofsky score was 20. The results of the real-time transcription polymerase chain reaction (RT-PCR) test were positive for SARS-CoV-2. Preoperatively she was treated with 100 mg of propylthiouracil (PTU) orally every 8 hours and 20 mg of propranolol orally once a day. We performed the curettage suction on the next day under spinal anesthesia using 10 mg of 0.5% hyperbaric bupivacaine. Surgery lasted 90 minutes and performed uneventfully. Evaluation up to 30 days after curettage we found no complications of thyroid crisis and worsening of COVID-19. Conclusion. In patients infected with SARS-CoV-2, innate immune dysregulation due to hyperthyroidism can worsen the clinical course of COVID-19, and conversely, SARS-CoV-2 infection can cause thyroid hormone disorders. Spinal anesthesia is safe to perform in molar pregnancy patients with hyperthyroidism accompanied by COVID-19.
Hipertiroid yang tidak terkontrol selama kehamilan dapat menyebabkan preeklamsia. Preeklamsia merupakan salah satu penyebab utama morbiditas dan kematian maternal. Wanita hamil yang mengalami preeklamsia dan hipertiroid tidak terkontrol memerlukan pemantuaan ketat dan terminasi kehamilan. Manajemen perioperatif dan anestesi yang tepat diperlukan pada kasus ini untuk mencegah terjadinya badai tiroid dan perburukan pasien akibat preeklamsia. Wanita 26 tahun dengan preeklampsia berat dan hipertiroid. Pada pemeriksaan fisik terdapat pembesaran kelenjar tiroid, eksoftalmus, tremor dan telapak tangan teraba lembab. Tekanan darah 160/100 mmHg, nadi 145 kali permenit, temperatur 36,7°C, SpO2 99% dengan O2 simple mask 5 lpm. pemeriksaan laboratorium didapatkan hemoglobin 12,7 gr/dL, FT4 33,08 ug/dl TSH 0,002 uU/ml, dan protein urin positif. Skor indeks Wayne 24 dan skala Burch Wartofsky’s 35. Sebelum operasi, pasien mendapatkan MgSO4 40% intravena, thyrozol, propanolol dan lugol. Pasien menjalani seksio sesarea dengan anestesi spinal menggunakan bupivakain hiperbarik 0,5% 12,5 mg dan ajuvan fentanil 25 mcg. Pasien menjalani perawatan selama empat hari dengan hemodinamik stabil dan dipulangkan tanpa komplikasi. Manajemen perioperatif dan anestesi yang tepat pada wanita hamil yang disertai preeklamsia berat dan hipertiroid tidak terkontrol dapat mencegah terjadinya perburukan kondisi pasien. Pemilihan anestesi spinal memiliki keuntungan dan aman dilakukan pada kasus ini.
Peripartum cardiomyopathy (PPCM) adalah kardiomiopati idiopatik pada kehamilan dengan manifestasi klinis gagal jantung akibat disfungsi sistolik ventrikel kiri tanpa adanya penyakit jantung yang mendasari. Terjadi pada kehamilan trimester akhir atau 1–5 minggu pasca kelahiran. Gagal napas pada PPCM terjadi karena adanya edem paru yang berasal dari gagal jantung akut. Wanita 25 tahun dengan G2P0A1 hamil 36 minggu + intrauterine fetal death + preeklamsi berat + gagal napas ec PPCM. Sesak bertambah berat saat tidur terlentang, pink froaty positif. Tanda vital: tekanan darah 131/100 mmHg, laju nadi 141x/menit, laju nafas 37x/menit, SpO2 76% dengan non rebreathing mask 15 liter/ menit, auskultasi paru ronki diseluruh lapangan paru kanan dan kiri, terdapat suara jantung tambahan murmur dan gallop. Echocardiografi didapatkan katup jantung mitral regurgitasi ringan sedang, trikuspid regurgitasi ringan, dan ejection fraction (EF) 36%. Pasien didiagnosa PPCM berdasarkan gejala klinis dan pemeriksaan penunjang, pasien mengalami gagal napas tipe 1 berdasarkan hasil analisa gas darah didapatkan nilai PO2 57,0 dan PCO2 35 mmHg. Intubasi dilakukan pada pasien PPCM dengan target saturasi O2 >95%. Tatalaksana PPCM bersifat suportif/non spesifik dan kausatif/spesifik secara simultan. Penatalaksanaan non spesifik secara tidak langsung ditujukan untuk memperbaiki pertukaran gas, berupa: mengatasi hipoksemia dengan terapi oksigen, atasi hiperkapnia dengan memperbaiki ventilasi hingga melakukan ventilasi mekanik dan fisioterapi dada. Telah dilaporkan pasien gagal napas akibat PPCM yang dilakukan tindakan ventilasi mekanik dan terapi multidisiplin memberikan keberhasilan terapi yang baik.
Infeksi Human Immunodeficiency Virus (HIV) dan Acquired Immunodeficiency Syndrome (AIDS) adalah masalah utama dari kesehatan global. Menurut United Nations Programme on HIV/AIDS (UNAIDS), salah satu bagian dari WHO yang mengurus tentang AIDS menyebutkan, bahwa perkiraan jumlah penderita yang terinfeksi HIV/AIDS di seluruh dunia sampai dengan akhir tahun 2010 mencapai 34 juta. Kasus HIV pada anak paling sering ditemukan akibat transmisi dari ibu yang HIV positif ke anaknya. Pada laporan kasus ini dilaporkan penanganan anestesi pada penderita pasien wanita berusia 25 tahun berat badan 50 kg primigravida hamil aterm 38 minggu belum dalam persalinan dengan infeksi HIV belum mendapatkan terapi antiretroviral. Pasien diklasifikasikan ASA II dan dilakukan anestesi regional teknik blok subarakhnoid dengan obat bupivakin 0,5% hiperbarik 10 mg. Dilahirkan bayi perempuan berat lahir 2500 gram, dengan skor Apgar 9/10. Operasi berlangsung selama 1 jam dengan hemodinamik TD 90–120/60-80 mmHg, HR 65-100 x/mnt, SpO2 99–100%, perdarahan 400 cc, produksi urin 0,5 ml/kgBB/jam. Pasca operasi pasien diobservasi di ruang pemulihan hingga skor Bromage 0 sebelum dikembalikan ke bangsal. Anesthesia Management in Cesarean Section Patients with HIV (Human Immunodeficiency Virus) Abstract Human Immunodeficiency Virus (HIV) and Acquired Immunodeficiency Syndrome (AIDS) infections are the main problems of global health. According to United Nations Programme on HIV/AIDS (UNAIDS), one part of the WHO that deals with AIDS states that the estimated number of people infected with HIV/AIDS worldwide by the end of 2010 reached 34 million. HIV cases in children are most often found due to the transmission of HIV-positive mothers to their children. We reported a 25-year-old primigravida, 38 weeks, weighing 50 kg, not in delivery with HIV infection without antiretroviral therapy underwent caesarean section. Patient was classified as ASA II and performed subarachnoid blocks with hyperbaric 0.5 mg bupivacaine 10 mg. A baby girl was born, weighing 2500 grams, with Apgar score 9/10. The operation lasts for 1 hour with hemodynamics TD 90-120/60-80 mmHg, HR 65-100 x/min, SpO2 99-100%, bleeding 400 cc, urine production 0.5 ml/kgBW/hour. In postoperative period, patient was observed in the recovery room until the Bromage score was 0 before transferred to the ward.
