Portuguese / English: www.scielo.br/reeusp RESUMO O estudo teve por objetivos: descrever o processo de desenvolvimento do projetopiloto de assistência a idosos acamados, da Unidade Básica de Saúde IAPI; e identificar aspectos demográficos, sociais e de saúde desses idosos, bem como aspectos relevantes relatados pela equipe, na implantação da assistência domiciliar. A pesquisa teve características descritivas e avaliativas. Foram revisadas as fichas cadastrais e os prontuários dos pacientes atendidos, assim como os registros de avaliação do projeto. A experiên-cia-piloto permitiu o desenvolvimento de habilidades na equipe, foi enriquecedora e de grande responsabilidade para profissionais e cuidadores. Apontou para a continuidade da assistência domiciliar, fazendo-se necessários ajustes na sua organização, com a finalidade de ampliar os espaços de assistência e a qualidade do que vinha sendo ofertado à população. DESCRITORES ABSTRACTThe objectives of this study were to describe the development of a pilot-project in home care to bedridden aged patients at a Basic Health Unit, and identify demographic, social and health aspects of these patients, as well as relevant aspects reported by the health team that implemented the home care. The study had descriptive and evaluative characteristics. The patients' enrollment forms and health records and the project's records were analyzed. The pilot-experience permitted to develop the team's skills, in addition to being enriching and of great responsibility for the professionals and caregivers involved. The results indicated the need for continuous home care and adjustments in its organization with the purpose of increasing the areas for health care and improving the population's quality of life.
Objective: To describe the process of adaptation of an instrument that analyzes the health literacy of older people. Methods: The adaptation consisted of steps to determine conceptual equivalence of items, operationally and semantically. Results: The stages of conceptual and item equivalence showed that the items and concepts of this Canadian instrument was suitable for use in Brazil. The steps of initial translation, back translation, expert committee review and pre-test performed during evaluation of semantic equivalence have resulted in alterations to some items and rewording of some questions. Regarding operational equivalence, the interview was appropriate to local realities. Conclusion: The steps of transcultural adaptation, generally adopted for validation of measurement tools, were used and allowed researchers to adequately conduct this research study. O objetivo do estudo foi verificar a utilização dos serviços ambulatoriais de saúde e fatores associados pelas mulheres de 20 a 60 anos, residentes na zona urbana de São Leopoldo, Rio Grande do Sul, Brasil. Foi um estudo transversal de base populacional, incluindo 1.026 mulheres, no ano 2003. A associação entre as variáveis e o desfecho foi avaliada por meio das razões de prevalência e dos intervalos de 95% de confiança (IC95%). A regressão de Poisson foi utilizada segundo modelo hierarquizado. Entre 1.022 mulheres, 136 (13,3%; IC95%: 11,2-15,4) não utilizaram os serviços ambulatoriais de saúde nos últimos 12 meses que precederam a entrevista. Demonstrou-se que as mulheres inseridas nas classes econômicas C, D e E, com menos anos de estudo e com renda mais baixa apresentaram menor utilização dos serviços de saúde. Assim, as mulheres com maiores necessidades e mais vulneráveis apresentaram menor utilização dos serviços de saúde, sugerindo condições desfavoráveis no sistema de saúde local, apontando para a presença de falta de universalidade e iniqüidade no acesso.
The main objective of this case study was to understand the construction of care in the Homecare Program for Bedridden Patients (PADA) of a basic health unit (UBS) in Porto Alegre, RS. Data were obtained from 13 caretaker groups in the UBS, and participating observation recorded in field diary in the patient's homes. An analysis of the discourse practices was performed inspired in Foucault's discourse concepts speeches and in studies on ethics and self-care. In the groups, the caretaker occupied a space that we metaphorically called the Delphic Oracle, a place for acceptance, listening and support. Hearing the dialogues that took place between the team and the caretakers made us face the contradiction present in the institutional discourse, which both stimulates self-care, and imposes rules, duties, tasks. Care as a citizenship right opposite to care as submission and subjugation created tension inside the group several times. Foucault's notion of care comprises a synthesis of the exercise of a person over his- or herself, making him or her better as a human being and, at the same time, capacitating him or her to become a better citizen.
Estudo de Caso, cujo objetivo principal foi de compreender a construção do cuidado no Programa de Atendimento Domiciliar ao Acamado, de uma Unidade Básica de Saúde, em Porto Alegre - RS. Dados coletados em 13 encontros de grupo com 29 cuidadores e nos domicílios, através de observação participante, de maio a dezembro/2006 e analisados através da análise do discurso. As cadeiras destinadas ao conforto do paciente no domicílio e a sonda nasoentérica foram identificadas como dispositivos de poder, nas relações dos cuidadores. A cadeira simboliza o tensionamento de poder cambiante, ora presente naquele que realiza o cuidado, ora naquele que está postado na cadeira. A sonda nasoentérica significa sofrimento e limitação imposta ao outro, mas também a preservação da vida. Referenciais pautados em Foucault, que remete à questão do homem como o que faz e exerce poder, porém sem dúvida, aponta que o poder pode produzir liberdade.
