RESUMO -Uma abordagem neurocirúrgica endoscópica transeptal transesfenoidal foi realizada em 30 casos de patologias diversas da região selar. O uso do endoscópio permite boa inspeção e diferenciação entre tumor e tecido normal, resultando em microdissecção do tumor, com preservação funcional da hipófise. A visão angular do endoscópio, auxilia na retirada das projeções tumorais para e supra selar. O uso da via transeptal transesfenoidal endoscópica, evita as complicações da via sublabial, como luxação dentária, perfuração de septo nasal, sinequias e formação de crostas. Os inconvenientes do uso do endoscópio são a diminuição da profundidade de campo, necessidade de constante controle do mesmo, e exige experiência com as técnicas endoscópicas.PALAVRAS-CHAVE: endoscopia, hipófise, seio esfenoidal, sela túrcica.Neuroendoscopic endonasal transseptal approach to the sellar region: study of 30 cases ABSTRACT -An endoscopic endonasal transsphenoidal approach to the sella was performed in 30 patients. The use of endoscope allowed close inspection and differentiation between tumor tissue and glandular remains, resulting in micro dissection of the tumor with maximum preservation of pituitary function. The angled view of the endoscope helps gross total removal of the tumor tissue, from the supra and para sellar extension. The endonasal transseptal endoscopic approach helps from dental problems, and lowers incidence of septal perforation, synechia, and crust formation. However the endoscopic approach has drawbacks. Com a introdução recente do endoscópio na cirurgia dos seios da face, e a excelente visualização das estruturas nasais com técni-cas atraumáticas, criou-se uma nova possibilidade para o neurocirurgião [3][4][5][6][7] . A combinação do abordo neurocirúrgico com a técnica otorrinolaringológica, resultou na via endonasal transeptal transesfenoidal neuroendoscópica da sela túrcica, evitando a extensiva e traumática dissecção da região buco nasal. RESUMO -Mucocele é uma lesão benigna cística dos seios paranasais, apresentando crescimento lento. É composta de material mucoso ou raramente mucopurulento, ocorrendo com frequência nos seios frontal e maxilar, podendo ser múltiplas, e causar erosão óssea. As mucoceles localizadas no seio esfenoidal (MSE) ou região posterior do seio etmoidal são raras, correspondendo a 1% dos casos de mucocele. Relatamos casos de MSE espontâneas submetidas a tratamento neurocirúrgico pela via transnasal transesfenoidal com uso do endoscópio.PALAVRAS-CHAVE: seio esfenoidal, mucocele, cirurgia transesfenoidal, cirurgia transnasal, endoscopia.Neurosurgical treatment of sphenoidal mucocele by endonasal transseptal endoscopic approach: report of two cases ABSTRACT -Mucoceles are benign, slowly expansive, mucous-filled cystic lesions that arise in the paranasal sinuses. They most commonly occur in the frontal and ethmoid sinuses and are rarely in the sphenoid sinus. The incidence of sphenoid sinus mucoceles is about 1% of paranasal sinus mucoceles. Almost 200 cases of sphenoidal and or intraselar mucoceles have been reported since the first description by Berg in 1889. Sphenoid sinus mucoceles result from accumulation of secretions due to obstruction of the draining sinus osteum. Clinical manifestations are dependent on the direction of the expansion toward adjacent structures, and are therefore variable. The diagnosis is based on CT and MRI. The neurosurgical treatment was marsupialization of the lesion under endoscopic vision by transseptal approach. We report two cases of sphenoidal mucoceles. As mucoceles dos seios paranasais, são lesões cís-ticas expansivas benignas, ocorrendo mais comumente nos seios frontal e etmoidal e em menor frequência no seio esfenoidal, podendo ser algumas vezes múlti-plas [1][2][3][4][5][6][7][8][9] . A mucocele do seio esfenoidal (MSE) foi descrita pela primeira vez em 1929 por Berg 2 . A sintomatologia é vaga e o diagnostico é facilitado pela tomografia computadorizada (TC) e ressonância nuclear magnética (RM). O tratamento é cirúrgico e, na maioria das vezes, pela via transesfenoidal com a utilização do endoscópio. Dois casos de mucocele do seio esfenoidal tratados por esta via são relatados. CASOSCaso 1. Homem de 36 anos, agricultor, procurou o Serviço de Neurologia em março de 1999 com quadro de cefaleia aguda intensa retro orbitária, não melhorando com analgésicos, com evolução de uma semana. O exame neurológico foi normal, assim como os exames hematoló-gico, bioquímico, oftalmológico e endócrino. A TC e a RM (Fig 1) mostraram volumosa mucocele esfenoidal. O paciente foi submetido a descompressão e marsupialização endoscópica da mucocele, pela via transnasal transseptal transesfenoidal. Dois anos após a cirurgia, o paciente encontra-se assintomático e o exame endoscópio atual, revela um esfenóide largo e permeável.Caso 2.
