Introducción: el manejo actual de la pseudoartrosis se ha convertido en un reto para los ortopedistas, ya que no existe un tratamiento que pueda garantizar la consolidación ósea, por lo que es necesario en estos casos el uso de ortobiológicos que, junto con las condiciones biomecánicas óptimas, son parte fundamental para el tratamiento de la pseudoartrosis. Reporte de casos: en este reporte se presentan dos casos, ambos masculinos de 25 y 23 años con fractura diafisaria de fémur, posterior a un accidente automovilístico, los cuales fueron intervenidos quirúrgicamente, pero con evolución hacia la pseudoartrosis. Ambos pacientes fueron tratados mediante retiro de material de osteosíntesis, desbridamiento de tejido óseo no viable y posterior enclavado centromedular, con colocación de autoinjerto de cresta iliaca más aspirado de médula ósea, usando como contenedor del injerto y andamio una esponja de gelatina, con lo cual ambos obtuvieron una regeneración y consolidación ósea total de manera más rápida. Conclusiones: resaltamos el uso de la esponja de gelatina como membrana de contención y medio ideal para el transporte de factores de crecimiento y diferenciación celular de osteoblastos provenientes del autoinjerto, que podrían actuar en sinergia al acelerar el proceso de consolidación ósea y mejorar la tasa de éxito en el tratamiento de esta patología.Palabras clave: ortobiológicos, injerto óseo autólogo, esponjas de gelatina, andamio, pseudoartrosis, regeneración ósea.
At first glance, the COVID-19 pandemic and the field of orthopaedics and traumatology do not appear to be related. Although orthopaedists are not considered front-line personnel in the fight against the pandemic, the role of the surgeon as part of the overall health care team is crucial. The specialty of orthopaedics and orthopaedic trauma, due to its extraordinary scope, affects individuals of all ages and timely care affects patients’ long-term function and quality of life. Therefore, positioning the type and timing of care for musculoskeletal injuries and conditions, while maintaining the safety of the patient and healthcare providers, is essential. This article reviews the initial approaches to orthopaedic trauma care during the COVID-19 pandemic as established by 4 representative countries in Latin America: Mexico, Argentina, Colombia, and Brazil.
Introducción: La gonartrosis es de las principales causas de dolor musculoesquelético que conlleva discapacidad. Gran porcentaje se acompaña de genu varo. La artroplastia total de rodilla (ATR) no es opción en pacientes jóvenes y la osteotomía tibial alta (OTA) aunque es aceptada en jóvenes, no está exenta de complicaciones. La diafisectomía de peroné (DP) es un método simple y seguro que puede aliviar el dolor y mejorar la función. Objetivos: Describir resultados clínicos y radiográficos en pacientes con gonartrosis grado II-III de Kellgren-Lawrence y genu varo tratados mediante DP. Material y métodos: Se incluyeron pacientes sometidos a DP para la gonartrosis del compartimento medial. Se evaluó dolor y funcionalidad prequirúrgico, postquirúrgico inmediato y a los tres, seis, 12, 18 y 24 meses. Radiográficamente la altura articular medial, lateral, proporción del espacio articular, así como ángulo fémoro-tibial y cóndilo meseta fueron evaluados en el prequirúrgico, a las 24 horas y a los 24 meses. Se consideró un valor de p < 0.05 como significativo. Resultados: Se analizaron 20 pacientes. El dolor vs 3 [3-3] prequirúrgico y postquirúrgico inmediato p ≤ 0.001), la funcionalidad vs 74 [66-78] prequirúrgico y postquirúrgico inmediato, p = 0.001 y WOMAC 59 ± 3 vs 12 ± 1 prequirúrgico y postquirúrgico inmediato, p ≤ 0.001). A los 24 meses tuvieron incremento de la altura del compartimento medial en comparación con el prequirúrgico (2.5 ± 0.5 vs 2.2 ± 0.6 mm, p = 0.004). El ángulo fémoro-tibial (174 ± 1 vs 178 ± 1 grados, prequirúrgico y postquirúrgico inmediato p ≤ 0.001). Conclusiones: La DP puede mejorar significativamente el dolor y la funcionalidad en pacientes con gonartrosis grado II-III de Kellgren-Lawrence y genu varo.
