Arterioportal fistulas are a rare complication of abdominal interventions. They can remain asymptomatic for a long time and manifest with symptoms of portal hypertension. We present the case of a 65-year-old man admitted to the emergency room with increasing fatigue and melena. He had a history of partial gastrectomy with Billroth II reconstruction 40 years earlier for peptic ulcer perforation. On physical examination, he was pale and presented a machinery-type murmur in the epigastric area. Blood tests revealed iron deficiency anaemia of 5.6 g/dL. During hospitalisation, he required several blood transfusions and maintained melenas. An abdominal CT angiography showed a fusiform aneurysm of the left gastric artery draining to the venous portal-splenic confluent. He was successfully treated with transarterial coil embolisation of the left gastric artery aneurysm. The correct diagnosis of arterioportal fistulas can be very challenging, whereas the standard treatment with coil embolisation offers a low morbidity resolution of symptoms.
Introdução: Literacia em saúde é um tema de popularidade crescente face à relevância e impacto que tem nos re- sultados clínicos. Existem diversas ferramentas de avaliação de literacia em saúde sendo o European Health Literacy Sur- vey Questionnaire (HLS-EU) o mais recentemente aplicado e validado. Graus de literacia consideradas problemáticas ou insuficientes estão associadas a incidência e gravidade pior de doenças, taxas de hospitalização e morbi-mortalidade superior. O objetivo foi aferir o grau de literacia em saúde relativamente aos cuidados de saúde.Material e Métodos: Aplicação da versão portuguesa do questionário HLS-EU (HLS-PT) aos doentes internados ao cuidado da medicina interna. Incluídos todos os doentes internados ao cuidado da medicina interna, tendo-se excluído de doentes cognitivamente e clinicamente incapazes de responder ao questionário. Resultados: Foram inquiridos 64 doentes. A amostra era maioritariamente do sexo masculino (57%) com média de idade 71 anos, reformado (71,8%) e com grau de escolaridade baixa (56,2% com formação académica até 1o ciclo). A maioria (68,7%) tinha grau de literacia em saúde problemática ou insuficiente quanto aos cuidados de saúde. Conclusão: Apesar de se reconhecer que, tanto o ambiente da aplicação do questionário como as características demográficas deste estudo serem diferentes do estudo nacional e europeu, revela-se um resultado preocupante da literacia em saúde de indivíduos hospitalizados com doenças crónicas.
Prostate cancer is the most frequent malignancy seen in males, with about 15% of cases showing metastatic disease at diagnosis, most commonly in bones (vertebrae, ribs, long bones). However, peritoneal metastasis with malignant ascites is extremely rare and may be overlooked by physicians. We report the case of a 77-year-old man presenting with symptoms of abdominal distension, nausea and weight loss. Abdominal ultrasound and computerised tomography confirmed ascites, peritoneal carcinomatosis and an enlarged prostate. Serum and ascitic prostatic-specific antigen were both elevated. Prostate biopsy identified a prostate adenocarcinoma, while biopsy of a peritoneal implant identified a prostate cancer metastasis, which responded well to hormonal therapy.
Os autores relatam o caso clínico de um homem de 77 anos com história de etilismo crónico que residia em condições socioeconómicas precárias numa região suburbana, admitido no hospital por hematoma espontâneo na coxa esquerda sem compromisso neurovascular. A avaliação complementar reve- lou pancitopenia (hemoglobina 6,4 g/dL; hematócrito 19%; leucocitos 2600/L; plaquetas 126000/L), macrocitose (VGM 113fL), hiperbilirrubinemia (total 2,64 mg/dL), tempo de pro- trombina prolongado (INR 1,47), hipoalbuminemia (2,8 mg/dL) e alterações fibróticas na arquitetura estrutural do fígado visíveis na ecografia abdominal. No internamento, verificaram-se alterações semiológicas sugestivas de escorbuto (lesão purpúrica nos membros inferiores, hemorragia perifolicular, pápulas hemorrágicas, pêlos em saca-rolha e hipertrofia gengival). A hipótese diagnóstica de hipovitaminose C foi confirmada perante doseamento < 1 mg/L, para além de déficit de vitamina B12 e ácido fólico. Iniciou-se suplementação vitamínica e otimização dietética, resultando numa evolução favorável das alterações clínicas e laboratoriais associados ao escorbuto. Com a descrição deste caso, os autores pretendem alertar para a associação entre má nutrição, etilismo e carência económica com subsequente hipovitaminose C. O reconhecimento das manifestações clínicas de escorbuto e diagnóstico atempado permite evitar investigação desnecessária e tratamento eficaz de uma entidade que acarreta grande morbilidade e mortalidade.
