Objetivo: O objetivo deste estudo é fazer uma análise comparativa entre as duas modalidades técnicas para acesso cirúrgico em nefrectomias de doador vivo para transplante: nefrectomia laparoscópica e acesso anterior transperitoneal. Métodos: No período de janeiro de 2001 a outubro de 2003 realizamos 63 transplantes de rim com doadores vivos. O acesso anterior transperitoneal foi utilizado em 36 casos e a nefrectomia laparoscópica assistida com a mão em 27. Comparamos o tempo de internação, analgesia no pós-operatório e a qualidade do enxerto. Resultados: O tempo de internação médio foi de 4,7 dias no grupo submetido ao acesso anterior transperitoneal e 3,7 dias no grupo submetido à laparoscopia assistida com a mão (p< 0,005). A utilização de analgésicos no pós-operatório foi menos freqüente no acesso laparoscópico, com expressiva significância estatística (p< 0,001). As complicações cirúrgicas e a qualidade dos enxertos foram semelhantes. Conclusão: Concluímos que os pacientes submetidos à Nefrectomia Laparoscópica apresentaram menor tempo de internação, menos dor no pós-operatório, melhor resultado estético e qualidade dos enxertos equivalente quando comparados com o acesso anterior transperitoneal.
