Introdução: O câncer vulvar responde por 3 a 5% tumores malignos femininos, compreendendo dois subtipos: tipo queratinizante e tipo clássico, predominantemente associado aos papilomavírus humanos (HPV) 16, 18 e 33. Há evidências de que algumas neoplasias intraepiteliais vulvares e vaginais de alto grau são lesões monoclonais derivadas de lesões de colo do útero. A presença do colo do útero não parece ser necessária para que o HPV oncogênico infecte o trato genital. Relato de caso: M.P.D.V., 53 anos, admitida por lesão vulvar dois anos antes, com prurido e ardência locais. Menarca aos 14 anos; sexarca aos 21, com cinco parceiros. G1PC1. Passado de cirurgia de alta frequência em outro serviço, porém sem relatório de alta. Durante o seguimento apresentou exame citopatológico compatível com neoplasia intraepitelial de alto grau (HSIL). Realizou-se biópsia que confirmou o achado, e a paciente foi submetida a histerectomia total. Em anatomopatológico da peça, não se identificou malignidade. Não foram feitos controles posteriores. Ao exame: vulva edemaciada, com placas cruentas em fúrcula, face medial do grande lábio esquerdo e introito vaginal circundadas por máculas hipercrômicas. Apresentava nódulo de aproximadamente 2 cm em terço médio, deformando a anatomia do grande lábio esquerdo. Não foram identificadas alterações de superfície ou anatômicas sugestivas de líquen escleroso. Não foi feito exame especular por intolerância álgica. Ao toque, vagina com 4 cm, endurecida, de trajeto irregular e friável, sangrante. Em fossa ilíaca direita, constatou-se presença de massa anfractuosa, de cerca de 3 cm linfonodos aumentados e endurecidos à esquerda. Foi realizada biópsia vulvar com achado de carcinoma de células escamosas, ulcerado e invasor, pouco diferenciado, grau III. Conclusão: Trata-se de paciente com passado de HSIL cervical, sem seguimento citocolposcópico adequado. As lesões causadas pelo HPV podem ser multifocais, multicêntricas (acometendo mais de um órgão), sincrônicas (ao mesmo tempo) ou metacrônicas (em tempos distintos). No câncer de vulva as lesões são multifocais em 5% dos casos e uma segunda malignidade sincrônica (mais comumente a neoplasia cervical) é encontrada em 22% das pacientes. Acredita-se que o câncer de vulva desse caso se associe ao HPV e não a distrofias vulvares, seja pela apresentação clínica (sem sinais de liquenificação vulvar), seja pela epidemiologia (concomitância com HSIL do colo do útero). Portanto, pacientes portadoras de HSIL, independentemente do sítio, são de alto risco para o desenvolvimento de doença maligna nas outras estruturas do trato anogenital inferior, devendo ser monitoradas de forma rigorosa por pelo menos cinco anos.
Introdução: Estima-se que a prevalência de adenomiose varie entre 20 e 30%, acometendo preferencialmente mulheres entre 40 e 50 anos, 90% multíparas. Quando sintomática, a doença caracteriza-se por sangramento uterino anormal, dismenorreia intensa e progressiva, dispareunia e dor pélvica crônica, com significativo prejuízo de qualidade de vida e importante causa de absenteísmo. Até recentemente, o tratamento preconizado para adenomiose era a histerectomia. Não é, todavia, opção para aquelas mulheres que desejam manter a fertilidade, apresentam risco cirúrgico elevado ou quando os sistemas de saúde se preparam para atender demandas sem precedentes de pessoas com COVID-19, e os serviços de cirurgias requerem adaptações, incluindo o adiamento das histerectomias. Nesse sentido, o endoceptivo que contém 52 mg de levonorgestrel (sistema intrauterino liberador de levonorgestrel - SIU-LNG) ascende como terapia alternativa, de primeira linha, à histerectomia (NICE), uma vez que é considerado mais eficaz que os tratamentos orais e injetáveis no controle do sangramento uterino anormal, mantendo a capacidade reprodutiva após a sua suspensão. Objetivo: Avaliar a eficácia e satisfação do SIU-LNG em reduzir os principais sintomas da adenomiose. Métodos: Os dados foram coletados por meio de entrevistas realizadas com pacientes que foram submetidas à inserção do SIU-LNG, em um serviço público na cidade de Belo Horizonte, como tratamento para sangramento uterino anormal. Resultados e conclusão: Entre as participantes do trabalho, 84 foram submetidas à inserção do SIU-LNG em virtude de adenomiose no período de 2019 a 2020. Delas, 33 pacientes foram excluídas, pois expulsaram o dispositivo ou não foi possível o contato para avaliação da satisfação. Entre as que estão em uso do método, 30 obtiveram redução importante de fluxo menstrual (ou amenorreia) após seis meses de tratamento. Vinte e nove manifestaram completa satisfação com o método em relação à melhora sintomática. Uma ficou insatisfeita com o método. Apenas duas permanecem com indicação de histerectomia em decorrência de insucesso no tratamento com o uso do SIU-LNG. Manejar os sintomas decorrentes da adenomiose de modo mais conservador é fundamental, mas nem sempre possível. O SIU-LNG mostrou-se capaz de reduzir a perda sanguínea menstrual em 74 a 97% dos casos após três meses de uso, conforme estudos prévios. Huskainem et al. mostraram melhora equivalente na qualidade de vida e bem-estar entre pacientes submetidos a histerectomia e usuárias de SIU-LNG. Desse modo, há de se repensar a maior disponibilidade do SIU-LNG no Sistema Único de Saúde, bem como na saúde suplementar, para finsde tratamento de sangramento uterino anormal, objetivando a redução de dor pélvica e sangramento uterino em pacientes portadoras de adenomiose, ao mesmo tempo que se reduzem os riscos atribuídos à histerectomia.
