Introdução: Os dentes supranumerários podem apresentar-se em todas as regiões da mandíbula ou maxila, mas existe uma predisposição para localizarem-se na região anterior da maxila. Objetivo: Relatar um caso de localização incomum de dente supranumerário fusionado na região posterior da maxila. Relato de Caso: A radiografia panorâmica sugeriu uma alteração da forma do terceiro molar superior, porém não foi possível obter o diagnóstico final. Assim, a tomografia computadorizada de feixe cônico foi utilizada para determinar o diagnóstico de fusão e também auxiliar no planejamento cirúrgico.Foi realizada uma intervenção cirúrgica com o objetivo de remover o terceiro molar fusionado ao supranumerário.Conclusão:Neste relato, observamos que a tomografia computadorizada de feixe cônico mostrou detalhes da ocorrência rara, permitiu o estabelecimento do diagnóstico e possibilitou a correta abordagem do caso, evitando intercorrências e complicações cirúrgicas.
Objective: The aim of this study was to evaluate bone microdamage in sites prepared for implant placement by using an ex vivo model with three drilling rotation speeds. Methods: Bovine bone ribs were used for the creation of 18 osteotomy sites at different rotation speeds: 1200 rpm, 800 rpm, and 400 rpm. Specimens were stained with xylenol orange and prepared for histological analysis by using fluorescence and polarized light microscopies. Bone microdamage was evaluated by number and based on total bone area, as follows: microfracture density (Fr.D = n/mm2), microcrack morphology (diffuse or linear), and density (Cr.D = n/mm2), and presence of bone chips. To complement the analysis, linear microcracks were assessed by using confocal microscopy for three-dimensional visualization. Results: Bone microdamage on the osteotomy sites included microcracks, diffuse damages, microfracture, and bone chip formation. There was an association between bone microdamage and cancellous bone (p 0.0016), as well as a positive correlation between Fr.D and Cr.D (p 0.05, r 0.54). BM occurrence was not different between the three rotation speeds. In 3D, the height of the microcrack depth was 60.81 µm. Conclusion: Bone microdamage occurs during osteotomy, and the ex vivo model used was effective for the assessment of these biomechanical parameters. In addition, microdamage was not influenced by the drilling rotation speed in this experimental condition.
Objetivo: Demonstrar o uso da visualização tridimensional (3D) na avaliação dos microdanos no osso com imagens histológicas. Métodos: Foram utilizadas imagens histológicas obtidas de amostras de osso não desmineralizadas, incluídas em metilmetacrilato e coradas em Calceína e com Alaranjado de Xylenol. Imagens que continham microdanos (microtrincas, danos difusos e microfraturas), analisadas por microscopia de luz polarizada ou contraste de fase, foram processadas para criar uma imagem 3D. A imagem tridimensional foi gerada utilizando-se o plugin Interactive 3D Surface Plot para o software ImageJ. Resultados: A morfologia dos microdanos foi mais bem observada nas imagens 3D obtidas neste estudo, visualizada tanto em suas cores originais ou no esquema multicor (Spectrum LUT). O esquema multicor Spectrum LUT permitiu diferenciar melhor os microdanos em relação à profundidade, uma vez que a diferenciação por cores é baseada nas características da superfície analisada. Maiores valores de um ponto no eixo z é representando na imagem por cores mais quentes. Conclusões: O uso da visualização tridimensional de forma complementar à análise convencional dos microdanos aprimorou sobremaneira sua avaliação morfológica e permitiu avaliar estes microdanos em diferentes perspectivas manipulando a imagem em diferentes eixos e posições.
