<p>En Odontología se prescriben antibióticos de manera empírica para el tratamiento de procesos infecciosos, siendo los macrólidos una alternativa terapéutica para casos de hipersensibilidad y resistencia bacteriana. Sin embargo, su uso no está exento de riesgos. El objetivo de este estudio fue identificar y caracterizar los eventos adversos asociados al uso de claritromicina y azitromicina. Se realizó un estudio observacional-descriptivo- transversal de Farmacovigilancia Proactiva en pacientes de dos Centros de Salud de la Ciudad de Corrientes que recibían antibióticos macrólidos, azitromicina o claritromicina, durante un período de dieciocho meses. Variables analizadas: edad, sexo, tratamiento farmacológico actual según diagnóstico, características de los eventos adversos identificados según: órganos y tejidos afectados; gravedad, tipo de reacción (A o B según mecanismo de producción); imputabilidad y resultado de la reacción (recuperado, recuperado con secuelas, no recuperado, desconocido, riesgo de vida). Se registraron 97 pacientes, de los cuales 26 (27%) presentaron eventos adversos. Los órganos afectados con mayor frecuencia fueron el sistema gastrointestinal (14, 54%) y cardiovascular en segundo término (5, 19%). Tres fueron graves (11%) y requirieron suspender la medicación, 7 moderados (27%) y 16 leves (62%). El 100% se recuperaron ad in- tegrum. Según el mecanismo de producción 22 (85%) fueron tipo A y 4 (15%) de tipo B. Según la imputabilidad 18 (82%) fueron probables y 3 probadas (12%). Los datos obtenidos muestran un potencial de eventos cardiovasculares, cuya gravedad debiera ser tenida en cuenta para la toma de decisiones relacionadas con la práctica clínica odontológica a fin de evitar un uso innecesario.</p><p> </p>
Objetivo Caracterizar la dispensa de antibiÓticos a pacientes ambulatorios en un instituto de seguridad social universitario durante el año 2020. Método Se realizó un estudio observacional, descriptivo-correlacional de utilización de medicamentos. Para cuantificar la dispensa, se utilizó la dosis diaria definida (DDD) por 1 000 habitantes por día (DHD) y la unidad de dosis, recomendadas por la Organización Mundial de la Salud (OMS), y el valor intrínseco terapéutico potencial como indicador cualitativo, según Laporte y Tognoni. Resultados Se incluyeron 4 748 dispensaciones ambulatorias. Edad promedio: 42 años; rango: 0-94 años, con predominio del sexo femenino (55%). La mayoría pertenecían al grupo de las penicilinas (51,3%) con predominio de amoxicilina en asociaciones a dosis fija o como monofármaco (48,3%), seguida por los macrólidos (21,0%) y las fluoroquinolonas (17,0%). Se pudo apreciar un consumo potencial de 7,7 DHD, lo cual implica que, en promedio, 7 de cada 1 000 afiliados de la institución estuvo expuesto a estos agentes diariamente, con predominio de amoxicilina asociada a ácido clavulánico (2,2 DHD). Conclusión La población general de universitarios estuvo expuesta a una prescripción alta de amoxicilina en asociaciones a dosis fijas, algunas de valor inaceptable o relativa, así como también a los nuevos macrólidos y fluoroquinolonas de segunda y tercera generación. Esto requiere especial atención, porque el consumo indiscriminado a nivel ambulatorio de antibióticos de amplio espectro podría convertirse en un factor de riesgo para el desarrollo de resistencia bacteriana; implica, además, un riesgo en la aparición de eventos adversos serios y un incremento en el gasto sanitario.
Introducción: La automedicación ha sido una estrategia frecuentemente utilizada como respuesta a una situación de malestar o enfermedad, ha jugado y juega un rol importante como primer eslabón en la atención de la salud, aunque no se la reconoce como tal. Se trazó como objetivo analizar la automedicación desde la perspectiva de los propios actores sociales. Material y método: Investigación de metodología cualitativa entre estudiantes universitarios de la Facultad de Medicina de la Universidad Nacional del Nordeste de Corrientes, Argentina, durante el ciclo lectivo 2013. Se utilizó el Método Comparativo Constante de Glaser y Strauss. Los datos fueron recolectados a través de entrevistas semiestructuradas, en profundidad. Se procedió a un análisis en espiral, arribándose a la generación de categorías respaldadas por fragmentos textuales de las entrevistas. Resultados: Los estudiantes universitarios se sienten seguros y capaces de a automedicarse ante cuadros clínicos que les resultan de baja complejidad y los previamente vivenciados por ellos o por familiares y conocidos. Discusión: El sentirse enfermo es una vivencia inseparable de la personalidad y de la biografía de cada sujeto. La respuesta a esa enfermedad es un saber heredado de la historia epigénica o formativa que ha sido olvidada tras su caída en la inmediatez.
