RESUMO -A transferência tendinosa do músculo tibial posterior é intervenção cirúrgica bastante utilizada na ortopedia para correção do pé caído por seqüela de hanseníase. Poucos modelos propuseram investigações mais significativas sobre os fenômenos plásticos cerebrais nas transferências tendinosas. O presente estudo teve como objetivo analisar a coerência espectral (CE) na Eletroencefalografia (EEG) em pacientes submetidos a transferência do tendão do tibial posterior pela técnica de Srinivasan através da EEG quantitativa (EEGq). A amostra foi composta de quatro sujeitos com pé caído devido a seqüela de hanseníase. Os parâmetro s de CE do EEG foram quantificados no momento pré e pós-operatório. Os resultados mostraram que houve efeito principal para o fator momento, significativo, no par de eletrodos C3-CZ, com aumento da coerência. E n t retanto, os achados da ANOVA não revelaram interação significativa entre bandas versus momento. However, few models have proposed a t h o rough investigation of the brain plasticity phenomenon during tendon transfer. Thus, the present study aimed at analyzing EEG spectral coherence (SC) in patients submitted to tendon transfer of the tibiliais posterior muscle by Srinivasan's technique and quantitative EEG (EEGq). The sample consisted of four subjects with drop foot caused by lepro s y. The SC parameter was evaluated in two experimental moments: p re and post-surg e ry. Results demonstrated a main moment effect for the C3-CZ electrode pair. Specifically, a significant increase in coherence values was observed. However, the ANOVA did not indicate a significant band/moment interaction. Objectives:To report a case series of squamous cell carcinoma (SCC) in the foot, describing previous risk factors, surgical treatment, histopathological findings, and functional and oncological results. Methods:Nine consecutive patients diagnosed with SCC of the foot were treated at a single institution and prospectively analyzed for risk factors related to the disease, surgical outcome, and histopathological, functional and oncological results. All patients had identifiable risk factors for SCC. Results:Definitive treatment consisted of partial (6 patients) or total (3 patients) amputation of the foot. The functional score was good or excellent in the surviving patients. Early identifiable risk factors are present in most patients. Biopsy when this diagnosis is suspected, in association with oncological principles, avoids diagnostic and treatment errors. Conclusion:Despite delayed diagnosis and surgical treatment with partial and total amputations of the foot in our series, we observed good oncological outcomes that avoided systemic spread of the disease and achieved expected functional results. The posterior tibial tendon transfer for foot drop in leprosy was efficient in restoring normal function of the foot and gait without changing foot posture. In the absence of a standardized method for assessing the results of posterior tibial tendon transfer, the Stanmore system seems to be a good tool for an objective evaluation.
A 42-year-old man presented with synovial osteochondromatosis (SO) of the retrocalcaneal bursa with intraosseous lesions in the calcaneum, leading to significant pain and consequent disability. Simple x-rays, computed tomography, and magnetic resonance imaging were performed preoperatively. The patient has remained asymptomatic, clinically and radiographically, without recurrent lesions for 18 months postoperatively.
-the aim of this study is analyze possible modifications in the cerebral cortex, through quantitative electroencephalography (qEEG) in patients submitted to a tendon transfer procedure (posterior tibialis) by the srinivasan's technique. Four subjects (2 men and 2 women), 49.25 age average (sD±21.4) were studied. All subjects have been through surgical procedure due to leprosy and had, at least, two years of drop foot condition. the qEEG measured the electrocortical activity (relative power) between 8 and 25 Hz frequencies pre and post surgery. A paired t test analyzed all data (p≤0,05). the results show significant alterations in the alpha relative power, electrodes F7 (p=0.01) and F8 (p=0.021). Altogether, based on findings of the current literature, we can conclude that the tendon transfer procedure suggests electrocortical alterations sensitive to specific qEEG bands.KEy worDs: qEEG, tendon transfer, relative power, drop foot, leprosy.Alterações na potência relativa cortical em pacientes submetidos a transferência de tendão: estudo pré e pós-cirurgico rEsumo -o objetivo deste estudo é analisar possíveis modificações no córtex cerebral, através da electrencefalografia quantitativa (EEGq), em pacientes submetidos a um procedimento de transferência de tendão (tibial posterior) pela técnica de srinivasan. Quatro sujeitos (2 homens e 2 mulheres), com média de idade de 49,25 anos (±21,4 DP) foram estudados. todos os sujeitos realizaram o procedimento cirúrgico devido a hanseníase e tinham, pelo menos, dois anos de pé caído. o EEGq mediu a atividade electrocortical (potencia relativa) entre freqüências de 8 e 25 Hz, no pré e pós-operatório. um teste t pareado analisou todos os dados (p≤0,05). os resultados mostram alterações significativas na potência relativa em alfa, nos elétrodos F7 (p=0,01) e F8 (p=0,021). baseados em recentes achados na literatura, podemos concluir que o procedimento de transferência de tendão sugere alterações eletrocorticais sensíveis às freqüências específicas do EEGq.PAlAvrAs-cHAvE: EEGq, transferência tendinosa, potencia relativa, pé caído, hanseníase.
Neste trabalho foram monitorados, por meio de radiografias simples e ultra-sonografias, dez pacientes submetidos a alongamento ósseo com o uso do fixador externo de Ilizarov, no período de março de 2001 a março de 2002. Os pacientes submetidos ao método de Ilizarov foram principalmente crianças e adolescentes, para correção de anomalias congênitas (seis casos). A correção do encurtamento se deveu a trauma complicado com osteomielite em três pacientes e a ressecção tumoral em um paciente. O fêmur esteve envolvido em 60% dos casos, a tíbia em 30% e a fíbula em 10%. Os achados radiográficos encontrados na evolução normal do regenerado ósseo foram um centro radiotransparente, ossificação linear, reação periosteal, aumento das partes moles e ossificação completa. Os achados da formação do osso novo na ultrasonografia foram o regenerado anecóico, ecos esparsos no sítio de distração, ecos lineares alinhados longitudinalmente, ecos grosseiros e amorfos e calcificação completa. Ocorreram 15 complicações em seis pacientes: cisto no regenerado (cinco casos), infecção adjacente aos pinos (quatro casos), coleção em partes moles (três casos), consolidação precoce (dois casos) e retardo na consolidação (um caso). Ten patients treated with Ilizarov external fixator for limb lengthening were monitored with radiographs and sonography examinations between March, 2001 and March, 2002. Most of the patients were children and adolescents who were submitted to surgery for correction of congenital deformities (six cases). In three patients shortening was secondary to post-traumatic osteomielitis and in one patient due to tumor resection. The femur was affected in 60%, tibia in 30% and fibula in 10% of the cases. The main radiographic findings of bone regeneration during the normal process of limb lengthening were lucent areas, linear ossification, periosteal reaction, soft tissue abnormalities and complete consolidation. Sonographic findings of new bone formation included anechoic areas, sparse echogenic foci within the distraction site, linear longitudinally oriented echogenic foci, amorphous and coarse echogenic foci and complete calcification. There were 15 complications in six patients: fluid collection at the distraction site (five cases), pin site infection (four cases), soft tissue collection (three cases), premature consolidation (two cases) and delayed consolidation (one case).
