Objetivo: Obtener datos actuales de la incidencia de Hipoacusia Neurosensorial (HNS) y determinar la fuerza de asociación de factores de riesgo en pacientes con HNS. Diseño del estudio: Observacional, analítico de casos y controles. Métodos: Pacientes con HNS, diagnosticados mediante potenciales evocados auditivos automatizados en la Fundación CINDA, entre los años 2002 y 2012, pareados por edad gestacional y peso para una relación caso-control 1:2. Resultados: De 11.774 historias clínicas,se detectaron 563 (4,78%) pacientes con HNS. Se encontró riesgo atribuible (odds ratio, OR) estadísticamente significativo para ventilación mecánica por más de cinco días (OR = 24,2), síndromes asociados a HNS (OR = 13,7), malformación craneofacial (OR = 7,9), antecedente familiar para HNS temprana (OR = 5,68), exposición a ototóxicos prenatales (OR = 3,13), exanguinotransfusión (OR = 2,02) e infecciones por el perfil Storch (OR = 1,60). Conclusiones: Nuestros resultados indican que la ventilación mecánica por más de cinco días, los síndromes asociados a HNS y las malformaciones craneofaciales presentan mayor fuerza de asociación para desarrollar HNS que otros factores de riesgo. Estos datos son fundamentales para organizar programas dirigidos a estos grupos de alto riesgo.)
Se realizó un estudio descriptivo comparativo para determinar las diferencias significativas de los Potenciales Evocados Auditivos de Latencia Corta que podría, a través del análisis de diversas variables, determinar la razón por la cual los pacientes mayores de 65 años, con pérdida auditiva neurosensorial promedio hasta 70 – 80 dB rechazan la amplificación binaural en el Instituto Para Niños Ciegos y Sordos en la ciudad de Cali, en el 2005. Se tomó una muestra intencional de 40 pacientes con adaptación binaural de mínimo tres meses, de los cuales 20 rechazaban los audífonos y 20 no. El estudio permitió detectar que en los resultados de la latencia de las Onda I, III y V, las latencias interpicos I – III , III – V, I – V y la diferencia interaural de la onda V, utilizando un estímulo click, variando la velocidad del estímulo de 11 p/seg y 77 p/seg, y la polaridad del estímulo en Rarefacción y Condensación en cada uno de los oídos presentaron valores que no se desvían de lo normal, para la población objeto de estudio; igualmente esto sucedió en la funcionalidad auditiva en los dos grupos de pacientes en diferentes situaciones diarias presentadas en el cuestionario HHIE modificado. El estudio mostró que no existen diferencias significativas entre los dos grupos de estudio, lo cual se comprobó utilizando la prueba U de Mann-Whitney, es decir que la prueba electrofisiológica del BERA, no aporta información que permita determinar si un paciente mayor de 65 años con pérdida auditiva neurosensorial entre 70 y 80 dB se adaptará a la amplificación binaural.
La posturografía permite evaluar la integración de los tres sistemas sensoriales que intervienen en el control postural y del equilibrio: el vestibular, el visual y el somatosensorial (Ronda, Galvañ, Monerris y Ballester, 2002). La presente investigación busca describir y analizar los resultados de la posturografía en niños entre seis y once años con pérdida auditiva quienes no han compensado su edad auditiva con respecto a su edad cronológica. Fueron evaluados dos grupos: el grupo control conformado por 32 niños con sensibilidad auditiva periférica normal y el grupo de estudio, conformado por 16 niños con pérdida auditiva, quienes usan amplificación auditiva y asisten a la Fundación CINDA para terapia auditiva con el fin de cerrar el GAP existente entre su edad cronológica y su edad auditiva. En la evaluación audiológica se aplicó anamnesis, revisión de historia clínica, audiometría tonal para la verificación de la sensibilidad auditiva periférica y posturografía, con un equipo de la marca NeuroCom, realizándose las siguientes pruebas: Integración sensorial en balance-modificado (CTSIB), límites de estabilidad (LOS) y distribución de peso (WHT). En los resultados se encontraron diferencias estadísticamente significativas entre los dos grupos de estudio para las tres pruebas en distintas variables. Como conclusión se pudo determinar que existen diferencias estadísticamente significativas entre el grupo control y el grupo de estudio. La intervención con terapia ocupacional influenció positivamente los resultados en la posturografía, sobre todo en el grupo de 10 a 11 años. Se observaron diferencias sucesivas en los tres grupos de edad, indicando un componente de neuromaduración.
