A number of memory-strategy training interventions have been developed to target and reduce the impact of normal age-related memory decline. Most outcome studies to date have used objective memory measures to evaluate the efficacy of such interventions. Participant-reported outcomes, well-suited to capture the extent to which such interventions address the expressed concerns of aging persons, have been less closely examined. We conducted a systematic review and meta-analysis to examine whether memory-strategy training interventions improve participant-reported outcomes in healthy older adults with age-related memory changes. Of 536 identified articles, a total of 18 randomized controlled trials met the inclusion criteria for the review, and 15 of these provided relevant data to include in the meta-analyses. A systematic risk of bias assessment identified common problems with recruitment and masking procedures. Results from the meta-analyses indicate that memory interventions produce positive effects on several participant-reported outcomes, including improvements in perceived memory ability, memory self-efficacy, strategy use, memory-related affect, psychological well-being, and quality of life. Memory-strategy training interventions have a meaningful perceived impact on healthy older adults experiencing age-related memory changes. Future research utilizing stronger study designs (e.g., masking participants and study staff) and more closely investigating everyday functional impact is needed.
RÉSUMÉLa perte auditive représente une problématique répandue chez la population âgée. Cela peut poser des défis pour les neuropsychologues, étant donné que les méthodes d’intervention doivent être présentées oralement et donc entendues avec précision. Dans cette étude, le statut auditif de 20 clients (âge moyen= 71 ans) a été évalué au sein d’une clinique de neuropsychologie en milieu hospitalier et l’impact de cette évaluation sur la pratique des neuropsychologues a été étudié. Un assistant de recherche a administré un bref test de dépistage des troubles auditifs à chacun des clients. Nous avons demandé à quatre neuropsychologues de commenter le statut auditif de leurs clients avant et après leur avoir présenté les résultats des tests de dépistage. Le dépistage a révélé que la majorité des clients présentait au moins une perte auditive légère et que les neuropsychologues étaient relativement précis (60 %) dans leur estimation du statut auditif de leurs clients. Enfin, les neuropsychologues ont utilisé l’information sur le statut auditif de leurs clients en vue de les diriger vers des services en audiologie, ainsi que pour renseigner les clients et les familles sur la perte auditive et la communication.
Online interventions for older adults should be tailored to their unique needs to increase the efficacy of and adherence to the intervention. The agile development cycle is a dynamic model to solicit and incorporate feedback from older adults during the design process. We combined this approach with the framework of Harvard University’s clinical and translational phases that provide a clear structure for evaluating new health programs before they are offered in the community. We based our online memory program on the empirically validated in-person Memory and Aging Program. The aim of the present study was to combine the agile development cycle with the clinical and translational phases framework to develop and pilot an online memory program tailored to the unique needs of older adults. Study 1 involved piloting individual program modules on site and integrating participant feedback into the program’s design to optimize usability. Study 2 involved two sequential pilots of the program accessed remotely to evaluate preliminary clinical outcomes and obtain feedback for iterative modifications. Plans for further validation and limitations are discussed. The successful application of the agile development cycle implemented in this series of studies can be adapted by others seeking to offer online content for targeted end users.
Clinician-led memory interventions have been shown to increase knowledge, reduce anxiety, promote memory-strategy use, and increase brain-healthy lifestyle behaviours in older adults with normal age-related memory changes. A self-guided, e-learning version of the Baycrest Memory and Aging Program® was recently developed to increase accessibility to memory interventions. The objectives of the current study were to assess program feasibility (retention rate), acceptability (satisfaction), and participant-reported impact (memory concerns, behaviour change, goal attainment). As part of a larger study, participants were 139 healthy older adults (mean age: 73±7, 73% female). Ninety-two individuals completed the program (retention rate=66%). Anonymous feedback data indicated a high level of satisfaction with the program overall (98%), the pace and clarity of the learning modules (100%), and the organization and navigation of the interface (92%). Suggested improvements included offering more interaction with others and addressing minor platform glitches. There was a decrease in the level of concern about memory change, with 64% expressing concern at a level consistent with the Jessen et al. (2014) criteria for Subjective Cognitive Decline at baseline, and 23% expressing the same at post-test. The majority of participants reported increases in using memory-strategies (63-97%) and lifestyle-promoting behaviours (40-72%). All participants reported moderate to high satisfaction with personal goal attainment. Results support feasibility, acceptability, and impact of a self-guided e-learning adaptation of memory intervention. E-learning tools may be a promising avenue to deliver accessible brain health promotion in later life, especially in the context of the shift to virtual care during and beyond COVID-19.
Age-related memory changes pose considerable concerns for aging adults, and can adversely affect their daily living and cause worry even when changes experienced are not clinically significant. The Memory and Aging Program® is a validated psychoeducation and memory strategy-training program that teaches the public about memory changes during aging and trains them to use evidence-based strategies to support brain health. The program has been offered in-person for over 20 years, and a self-guided eLearning version was recently developed to improve program accessibility. This study evaluated the self-reported impacts of memory changes in older adults who completed this eLearning against a control group. We randomized 202 older adults, without neurological or psychiatric diagnoses (71.6 years; 69 % female; 15.6 years of education), into the eLearning program or a control group that received no intervention. All participants reported their perceived impact of memory changes using the Memory Impact Questionnaire at pre-, post-, and 6-8 weeks follow-up. A significant reduction in negative impact of memory changes on daily living and a significant improvement in positive coping with memory changes relative to controls was observed at post-test (13.4 versus 2.5 points reduction and 7.4 versus 0.1 point improvement, respectively, both p < 0.05), but these did not persist at follow-up. The adoption of digital tools has hastened across all ages. Our study showed that self-guided digital tools, such as the eLearning Memory and Aging Program®, may be a promising avenue to help aging individuals reduce the impact of memory changes on daily living.

Аннотация: В статье рассматривается возрастная инволющия познавательных процессов у пожилых людей: мышления, кратковременной зрительной памяти, устойчивости произвольного внимания. Актуальность изучения обусловлена стареющим населением страны. Квалифицированные работники умственного труда, достигнув пен-сионного возраста, по воле работодателей оказываются исключенными из активной трудовой жизни. В то же время их опыт нередко оказывается востребованным. Психологическая проблема заключается в выявлении когнитивного ресурса, позволяющего им пролонгировать свою продуктивную работоспособность. Эмпирическое исследование выполнено на общей выборке 322 человека в возрасте от 18 до 89 лет; в их числе 144 мужчины и 178 женщин с высшим и средним образованием, работающие в настоящее время. Методом возрастных срезов изучалась про-дуктивность высших психических функций в разные возрастные периоды жизни при естественном старении без патологических изменений. Использовались общеизвестные валидные и надежные психодиагностические методи-ки. Полученные данные представлены в графиках, отражающих изменение продуктивности познавательных про-цессов (Y) в зависимости от возраста (X). Обнаружен гендерный сдвиг в трендах и характере возрастной инволю-ции когнитивных функций. Установлено, что регресс познавательных процессов согласно закону Т. Рибо протекает в порядке, обратном их генезису. На частной выборке 70 лиц с высшим и 70 лиц со средним образованием, рабо-тающих в возрасте 60-85 лет, проведены корреляционный и факторный анализы. Выявлены психосоциальные факторы, влияющие на сохранность продуктивной мыслительной деятельности в пожилом возрасте. Наибольшие веса принадлежат факторам семейной поддержки, умственной активности и образованности субъекта. Эмпириче-ски подтверждено, что у специалистов интеллектуального труда регресс познавательных функций протекает менее интенсивно и стабилизируется в возрастном периоде 65-70 лет. В пожилом возрасте на сохранность мыслитель-ных операций позитивно влияет предшествовавшая регулярная интеллектуальная деятельность в сферах профес-сий «человек -человек» и «человек -знак». Образованность субъекта, не занятого интеллектуальным трудом, ока-зывает лишь скрытое тормозящее влияние на регресс когнитивных функций.
