Oral squamous cell carcinoma (OSCC) is the most common type of head and neck (H&N) cancers with an increasing worldwide incidence and a worsening prognosis. The abundance of tumour infiltrating lymphocytes (TILs) has been shown to be a key prognostic indicator in a range of cancers with emerging evidence of its role in OSCC progression and treatment response. However, the current methods of TIL analysis are subjective and open to variability in interpretation. An automated method for quantification of TIL abundance has the potential to facilitate better stratification and prognostication of oral cancer patients. We propose a novel method for objective quantification of TIL abundance in OSCC histology images. The proposed TIL abundance (TILAb) score is calculated by first segmenting the whole slide images (WSIs) into underlying tissue types (tumour, lymphocytes, etc.) and then quantifying the co-localization of lymphocytes and tumour areas in a novel fashion. We investigate the prognostic significance of TILAb score on digitized WSIs of Hematoxylin and Eosin (H&E) stained slides of OSCC patients. Our deep learning based tissue segmentation achieves high accuracy of 96.31%, which paves the way for reliable downstream analysis. We show that the TILAb score is a strong prognostic indicator (p = 0.0006) of disease free survival (DFS) on our OSCC test cohort. The automated TILAb score has a significantly higher prognostic value than the manual TIL score (p = 0.0024). In summary, the proposed TILAb score is a digital biomarker which is based on more accurate classification of tumour and lymphocytic regions, is motivated by the biological definition of TILs as tumour infiltrating lymphocytes, with the added advantages of objective and reproducible quantification.
BackgroundImmune dysfunction in breast cancer patients is well established. Indoleamine 2,3-dioxygenase (IDO) is an immunosuppressive enzyme that is linked with progression of cancer. IDO is overexpressed in triple-negative breast cancer (TNBC) cases.Materials and methodsWe conducted the first study to analyze IDO expression and overall survival in breast cancer cases in Pakistan. Expression of IDO, estrogen receptor (ER), progesterone receptor (PR), and human EGF receptor 2 (HER2) was evaluated by immunohistochemistry. Formalin-fixed paraffin-embedded breast cancer tissues of 100 (TNBC, n=49 and non-TNBC, n=51) patients were obtained from Shaukat Khanum Memorial Cancer Hospital and Research Centre. IDO expression was analyzed in association with clinicopathological features and overall survival. A total of 100 patients were classified based on the ordinal IDO score variables as low, medium, and high. In addition, overall mean age and SD of patients was 48.28±11.82.ResultsImmunohistochemical analysis showed that high IDO was observed in the TNBC patients (65.3%) compared to that in the non-TNBC patients (33.3%). Multivariable analyses showed that TNBC was an independent risk factor for high IDO expression. Overall survival was also significantly associated with IDO score.ConclusionOur study showed that IDO protein expression is higher in TNBC patients (P<0.01) and may suggest its role in disease pathogenesis. TNBC might be effectively treated with IDO inhibitors. Furthermore, high IDO expression is considerably associated with overall decreased patient survival. IDO might be utilized as a potential biomarker and immunotherapeutic target in breast cancer patients.
Iron deficiency anemia refers to the anemia caused by insufficient iron stores within the body. The main cause of anemia in obstetrics is iron deficiency, which has a worldwide prevalence estimated between 20%–80% and constitutes mainly female population.
Objective: To find out the determinants of iron deficiency anemia amongst pregnant women visiting Tertiary Care Hospitals, Lahore. Methods: A cross-sectional study was conducted at Sir Ganga Ram Hospital, Lahore. A sample of 245 pregnant female patients was selected through non-probability convenient sampling technique. Data were collected with the pre-tested questionnaires. SPSS version 21.0 was used for data analysis.
Results: Amongst social determinants, socioeconomic status had a signifcant effect on iron defciency anemia; as 82% women belonged to low socioeconomic status. Among dietary determinants, less consumption of red meat was a major factor affecting IDA as 95% women were not consuming red meat more than two times a day thus contributing to overall iron defciency. Amongst clinical factors, lack of thorough clinical checkup before conception was a major factor as 79% women did not have regular clinical checkup before pregnancy.
Conclusions: Major determinants linked with IDA were dietary habits and practices such as preferring vegetables over meat, less overall consumption of red meat and excessive consumption of tea and coffee. Other determinants affecting IDA include low socioeconomic status, little or no gap between pregnancies and lack of knowledge and education concerning dietary practices during pregnancy.
