Introdução: Cirurgiões dentistas e médicos-veterinários são legalmente habilitados a prescrever medicamentos sob controle especial com indicação dentro de suas áreas de atuação. No entanto, existe uma lacuna de estudos que descrevam a completude de prescrições realizadas por profissionais não médicos conforme legislações, normas sanitárias e éticas estabelecidas no Brasil. Objetivo: Avaliar erros de prescrição em notificações de receita e receitas brancas de medicamentos sujeitos a controle especial pela Portaria 344/1998, as quais tenham sido realizadas por profissionais não médicos, cirurgiões-dentistas e médicos-veterinários. Metodologia: Estudo transversal, realizado com base em análise documental de prescrições arquivadas em uma farmácia comunitária de um município no estado da Bahia, com aplicação de um formulário padronizado. Os softwares EpiData 3.1 e SPSS 21.0 auxiliaram na tabulação e análise dos dados, descritos em frequência absoluta e relativa e com média e desvio padrão. O nível de significância adotado foi 5%. Resultados: Foram analisadas 235 prescrições, das quais 67,7% (159) foram realizadas por cirurgiões-dentistas e 32,3% (76) por médicos-veterinários. A associação paracetamol com codeína predominou em 69,8% das receitas de cirurgiões-dentistas, e os antiepilépticos prevaleceram em 75% das receitas de médicos-veterinários. Houve um total de 1.529 erros de prescrição entre médicos-veterinários e cirurgiões-dentistas, com média superior para os médicos-veterinários. Conclusão: Realizar prescrições com dados completos evita erros e promove uma adequada e segura terapêutica para a população usuária.
To analyze the errors of prescription and administration of antimicrobials powder for solution for injection in a public hospital. This is a cross-sectional study carried out in a public hospital, in which antimicrobials prescriptions and administrations were analyzed for patients admitted to the Intensive Care Unit (ICU) and Medical Clinic (CM) from November 2015 to February 2016. The tabulation and data analysis were done in Epidata software version 3.1 of 2008 and IBM Statistical Package for the Social Sciences (SPSS). In the statistical analysis, chi-square tests or Fisher’s exact test were applied when necessary. The level of significance was 5%. Among the statistically significant results, the following are the errors related to medical prescription with the variables: age at 57% and medical specialty at 67%, both at the ICU; bed with 30% and hospitalization unit with 37%, both in CM. In the administration of antimicrobials, statistically significant differences were observed only in the failure to identify the patient (30% in CM). Regarding the use of antimicrobials, Cefepime was the most prescribed with 65.1%. In view of these aspects, it is extremely important that errors arising from an incomplete and misleading prescribing are identified, to propose improvements in the medication system, in order to prevent errors, and to promote a more rational antibiotic therapy, avoiding infections.
A prescrição é um documento que deve permitir a comunicação entre prescritor, dispensador e usuário, tendo como objetivo o uso correto e racional de medicamentos. Com o objetivo de avaliar a completude e indicadores de prescrição de notificações de receita B e receitas de controle especial arquivadas em uma unidade básica de saúde em Jequié-BA, realizou-se um estudo transversal, com base em análise documental de notificações de receita B e receitas de controle especial de medicamentos constantes da Portaria 344/98, ao qual foi realizado entre os meses de dezembro de 2013 a maio de 2014 com análise da totalidade da amostra diária, realizada em dias aleatórios durante a semana, no total foram analisadas 200 notificações de receita B e 202 receitas de controle especial. Os resultados demonstraram incompletude em alguns requisitos de preenchimento obrigatórios para a dispensação de medicamentos, o item que apresentou maior ausência de informação foi o endereço do paciente, assim como um percentual significativo de nomes comercias dos medicamentos. Dessa forma constata-se ainda que há uma necessidade urgente dos profissionais médicos em aderirem e seguirem as legislações vigentes quanto à prescrição de medicamentos no sistema único de saúde.
Este estudo teve como objetivo compreender a cultura de segurança do paciente e notificação de eventos adversos de equipe multiprofissional de saúde de um hospital público de ensino. Estudo transversal com coleta de dados entre dezembro de 2015 a março de 2016, utilizando questionário Hospital Survey on Patient Safety Culture (HSOPSC). A amostra foi intencional constituída de profissionais de saúde envolvidos na assistência. Dos participantes (331) 44,0% eram técnicos/auxiliares de enfermagem, 28,4% enfermeiros, 17,1% médicos e 10,5% outros. Considerando as 42 questões do instrumento um total de 5385 (39,7%) respostas foram positivas, 3122 (23,0%) neutras e 5066 (37,3%) negativas. Observou-se que 78,3% profissionais não comunicaram nenhum evento adverso (EA) nos últimos 12 meses e 52,8% consideraram a nota de segurança do paciente como “regular”. A pontuação mais positiva esteve associada com profissionais que tinham menos de um ano de trabalho no hospital (p=0,021), trabalham menos de 80 horas semanais (p=0,048), exercem cargo ou função de nível técnico (p=0,010), segundo grau completo (p=0,006) e aqueles que realizaram notificação de EA nos últimos 12 meses (p=0,016). A cultura de segurança do paciente no hospital de estudo ainda não está estruturada, no entanto aspectos positivos destacados podem ser utilizados como ponto de partida para a implementação de mudanças.
Introdução: As sondas, dispositivos utilizados para nutrição enteral, podem consistir em uma via alternativa para a administração de medicamentos orais para pacientes em estado grave, como os internados na Unidade de Terapia Intensiva (UTI). No entanto o uso da sonda nasoenteral (SNE) e nasogástrica (SNG) para a administração de medicamentos não está isenta de riscos como às obstruções da sonda, incompatibilidades entre fármacos e nutrientes, aumento de efeitos adversos e diminuição da eficácia dos fármacos. Estudos realizados em três hospitais de ensino relatam a prescrição de medicamentos de uso oral para administração por sondas gástricas, prescritos na forma farmacêutica sólida em 6,6%, 94%, e 60,61% respectivamente. Considerando o uso empírico de medicamentos por essa via em hospitais, mas precisamente em UTI, e a escassez de estudos com informações que direcionem um correto preparo e administração de formas farmacêuticas orais, pesquisas com esses pontos são importantes para promover ações que tenham por fim a elaboração de protocolos clínicos em hospitais que não dispõem de farmacotécnica hospitalar. Objetivo: Analisar prescrições de medicamentos administrados por sonda a pacientes internados em uma UTI. Métodos: Estudo transversal, descritivo – analítico, realizado no mês de novembro de 2017, por meio da análise de prescrições de medicamentos utilizados via SNG ou SNE por pacientes internados na UTI de um hospital público no estado da Bahia. Foram considerados como critérios de inclusão, todas as prescrições recebidas pela farmácia do hospital, advindas da UTI, no mês de novembro de 2017 com indicação de uso de medicamentos VSNG ou VSNE. A UTI foi selecionada nesta avaliação por ser o setor hospitalar com maior número de prescrições de medicamentos envolvendo esse tipo de via de administração. Amostra e coleta de dados: Foram avaliadas a totalidade das prescrições do mês de novembro de 2017, totalizando uma amostra de 138 prescrições contendo medicamentos por VSNG e VSNE. Foram avaliadas todas as prescrições, via da farmácia, de pacientes internados na unidade de terapia intensiva durante o período de novembro de 2017, que continham medicamentos utilizados via sonda nasogástrica ou nasoenteral. Para avaliação do uso adequado ou inadequado do medicamento por VSNG ou VSNE foi consultada a bula do profissional referente ao medicamento disponível no bulário eletrônico da Agência Nacional de Vigilância Sanitária. Análise de dados: A tabulação dos dados foi feita nos softwares Epidata versão 3.1 de 2008 e StatisticalPackage for the Social Sciences (SPSS), versão 23.0 de 2015 (IBM Corp, Armonk Estados Unidos da América) para a realização das análises estatísticas. Foram calculadas as frequências absolutas (FA), relativas (FR) das variáveis categóricas e para as variáveis continuas a média e desvio padrão (DP). Para a análise estatística, considerou-se como variável dependente o 3o nível ATC dos medicamentos e todas as outras variáveis como independentes. Foi feito o teste Qui-quadrado de Pearson e para valores observados abaixo de cinco a razão verossimilhança. O nível de significância adotado foi de 5% (α = 0,05). Considerações éticas: O estudo foi realizado seguindo todos os preceitos éticos vigentes, Resolução do Conselho Nacional de Saúde - CNS no 466/12, sendo aprovada pelo Comitê de Ética e Pesquisa da Universidade Estadual do Sudoeste da Bahia com no de protocolo 29780014.8.0000.0055 e número de parecer favorável 703.37. Resultados: O estudo foi elaborado com base na análise de 138 prescrições, as quais continham 410 medicamentos para serem administrados via sonda nasoenteral e nasogástrica, tendo uma média de 2,92 (DP=1,53) medicamento por prescrição. Em relação ao tipo, 94,9% dos medicamentos foram prescritos para administração via SNE e 5,1% VSNG. De acordo a classificação Anatomical Therapeutic Chemical (ATC), as classes de medicamentos mais prescritas foram: para o sistema cardiovascular 64%, sistema nervoso 22,9%. Formação de sangue órgãos (antitrombóticos) 4,4% e trato alimentar e metabolismo, 3,2%. Conclusão: Com base nos resultados do referido estudo, é perceptível a alta frequência de medicamentos prescritos por sonda nasoenteral e nasogástrica, sendo em maior porcentagem os pertencentes as classes de medicamentos para o sistema cardiovascular, e nervoso, uma vez que, esses dados podem ser justificados pela vasta utilização dessas classes de medicamentos.
