Cette étude qualitative exploratoire analyse l’impact d’un diagnostic d’autisme sur les réseaux de soutien social de parents originaires du Maghreb, nouvellement immigrants au Québec, et ayant un enfant diagnostiqué avec un trouble envahissant du développement. Des entrevues semi-structurées ont été réalisées avec dix parents et une observation participante a été effectuée lors de rencontres de soutien pour les mères. Nos résultats soulignent que le diagnostic d’autisme transforme le réseau familial et communautaire des parents, en créant des distances et des tensions mais aussi en générant de nouveaux liens, sources de soutien. Des rencontres de groupe entre mères originaires de la même communauté ayant un enfant autiste sont perçues par celles-ci comme étant particulièrement bénéfiques. L’analyse du rapport aux services professionnels montre que l’élaboration d’une alliance thérapeutique est souvent ardue à cause d’obstacles administratifs et de difficultés sur le plan de la communication interculturelle. Une meilleure compréhension, par les intervenants en santé et services sociaux, de l’interaction qui existe entre le contexte migratoire et la fragilisation des réseaux familiaux et sociaux associés à un diagnostic d’autisme chez un enfant de famille immigrante pourrait contribuer à diminuer la solitude et la souffrance de ces parents.This qualitative exploratory study examines the impact of an autism diagnosis on the social support networks of North African’s parents recently immigrated to Quebec and having a child diagnosed with pervasive developmental disorder. Semi-structured interviews were conducted with ten parents and participant observation took place during support meetings for the mothers. Our results highlight how the autism diagnosis transforms the parents’ family and community networks, creating tensions and losses while also generating new links, sources of support. Group meetings between mothers of the same community experiencing the same problem are perceived particularly beneficial. The analysis of the relationship between professional service providers and families underlines how the development of a therapeutic alliance is threatened by major problems caused by administrative obstacles and intercultural communication difficulties. A greater awareness by health and social professionals of the interaction between the migratory context and the social network challenges associated with autism diagnostic for immigrant families would help ease these parents’ loneliness and suffering
Cette étude qualitative ethnographique documente les systèmes de croyance et les choix thérapeutiques de dix parents immigrants originaires du Maghreb et ayant un enfant diagnostiqué avec un trouble du spectre autistique au Québec. Des entrevues semi-structurées des parents et une observation participante lors de rencontres de soutien pour les mères ont été effectuées. Nos résultats montrent un tableau polysémique où coexistent des systèmes explicatifs biomédicaux, religieux et traditionnels. Ces différentes croyances influencent les choix thérapeutiques des parents qui font appel à la fois aux approches modernes communément acceptées au Canada et aux thérapies spirituelles ou traditionnelles acceptées au Maghreb. La religion joue un rôle particulièrement important dans l’acceptation de la condition de l’enfant. Une ouverture et un respect face aux croyances des parents et leurs choix thérapeutiques sont des éléments-clés pour faciliter l’alliance avec ces derniers.
Le deuil compliqué demeure un sujet à controverse. Bien que ce concept se retrouve dans de nombreuses cultures, peu d’études scientifiques abordent les complications du deuil dans une perspective transculturelle. Cet article, qui repose sur la présentation d’un cas clinique et une revue de la littérature, décrit les particularités du deuil selon les cultures et en contexte d’immigration. Il offre un regard transculturel sur le deuil dit compliqué et discute les limites des classifications internationales pour diagnostiquer les complications d’un deuil selon les cultures. En influençant le processus même du deuil, ses manifestations cliniques et ses complications éventuelles, la culture et la religion peuvent rendre complexe la distinction entre un deuil normal et un deuil compliqué en contexte interculturel. Le concept de deuil compliqué est discuté en mettant en avant l’importance d’une approche culturellement sensible pour en permettre une évaluation clinique appropriée. Nous soulignons la question centrale du sens donné à la mort et aux symptômes vécus en période de deuil, et la pertinence d’un regard critique sur le risque de médicalisation du deuil et les limites des classifications internationales, DSM-5 et CIM-11. Les nouvelles nosographies devraient bénéficier d’une plus grande précision concernant les complications du deuil selon les cultures non occidentales, en incluant des déterminants spécifiques liés à la culture, comme les systèmes de croyances entourant une « mauvaise mort » et les interprétations culturelles des rêves où figure une personne défunte.
The field of psychodynamic psychotherapy would benefit from a comprehensive model that integrates its constructs with neurobiology. Research on the autonomic nervous system activity during the psychotherapeutic process is necessary because it is key in affective experiences and defensive behavior. The current case study reports physiological findings on heart rate dynamics in a patient suffering from panic disorder during two therapeutic sessions in which we used Davanloo's Intensive Short-term Dynamic Psychotherapy. We looked at various metrics of heart rate variability during the therapeutic process leading to breakthrough of unconscious feelings. The measurements included sympathetic and parasympathetic indices, vagal tone, and their responses. Our results suggest that the sympathetic system activates during defensive responses associated with anxiety and during the passage of unconscious-aggressive impulses. Following the experience of unconscious guilt, there is an increased vagal tone corresponding to the phase of reunification with the attachment figure. Findings are discussed integrating developmental neurobiology and clinical psychodynamics.
