El quiste branquial (QB) se considera una malformación congénita cuando existe obliteración incompleta de los arcos branquiales. El tipo de quiste a desarrollarse depende directamente del arco afectado. Este tipo de anomalías supone la segunda causa más frecuente de tumores congénitos en cabeza y cuello. Se presenta el caso clínico de un adulto de 23 años que consulta por infección y aumento de volumen en región cervical, de evolución rápida, indurado y doloroso a la palpación. El estudio imagenológico indica lesión quística, encapsulada lateral a vaina carotídea. Se decide enucleación bajo anestesia general y se plantea cirugía en conjunto con otorrinolaringología (OTL). El estudio anatomopatológico sugiere presencia de quiste de segundo arco branquial. En el caso expuesto se discute la presentación clínica, diagnósticos diferenciales y alternativas de tratamiento de las anomalías de segundo arco branquial, considerando su hallazgo imagenológico posterior a la reacción inflamatoria provocada por una infección orofaríngea.Palabras clave: quiste, quiste branquial, quiste de segundo arco branquial.
