The population in the world is aging dramatically, and therefore, the economic and social effort required to maintain the quality of life is being increased. Assistive technologies are progressively expanding and present great opportunities; however, given the sensitivity of health issues and the vulnerability of older adults, some considerations need to be considered. This paper presents DigiHEALTH, a suite of digital solutions for long-term healthy and active aging. It is the result of a fruitful trajectory of research in healthy aging where we have understood stakeholders’ needs, defined the main suite properties (that would allow scalability and interoperability with health services), and codesigned a set of digital solutions by applying a continuous reflexive cycle. At the current stage of development, the digital suite presents eight digital solutions to carry out the following: (a) minimize digital barriers for older adults (authentication system based on face recognition and digital voice assistant), (b) facilitate active and healthy living (well-being assessment module, recommendation system, and personalized nutritional system), and (c) mitigate specific impairments (heart failure decompensation, mobility assessment and correction, and orofacial gesture trainer). The suite is available online and it includes specific details in terms of technology readiness level and specific conditions for usage and acquisition. This live website will be continually updated and enriched with more digital solutions and further experiences of collaboration.
RESUMENLas muertes de personas que se encuentran en situación de privación de libertad, presos, detenidos y pacientes ingresados involuntariamente en establecimientos psiquiátricos, conocidas en la literatura médico legal anglosajona como "Deaths in Custody", han motivado numerosas publicaciones y capítulos exclusivos en los textos más prestigiosos de la especialidad en los últi-mos años. Proponemos la denominación de Muerte en Privación de Libertad (MPL) en lengua española en lugar de la traducción literal de Muerte en Custodia, así como una normativa legal que establezca la obligatoriedad de la autopsia e investigación judicial en todos estos casos. La autopsia de una persona fallecida en estas circunstancias es, en general, la más problemática que se puede plantear a un patólogo forense. Los datos existentes sobre la situación en España son incompletos, se analizan las causas más frecuentes de estas muertes, con especial atención al Delirium con agitación, y se hacen una serie de propuestas encaminadas a la prevención y mejor conocimiento de las mismas. Así mismo revisamos las publicaciones médico legales sobre tortura, los métodos utilizados y los protocolos internacionales para la investigación y documentación de la misma, tanto en el sujeto vivo como en el cadáver (Protocolos de Minnesota y Estambul). ABSTRACTThe deaths of people who find themselves deprived of liberty, for example, prisoners (inmates in prison), people who have been arrested, involuntary patients in psychiatric institutions, known in legal medical English publications as "Deaths in Custody" have been the focus of a lot of publications and special chapters in forensic medicine books and journals in the last few years. Our proposal in the Spanish language is the expression Death in a Freedom deprived state. We also propose the promulgation of a Law to perform compulsory autopsy and judiciary investigation in all these cases. These autopsies are among the most difficult and contentious deaths for a forensic pathologist to investigate. The current information about this problem in Spain is incomplete. We analyse the most frequent causes of these type of deaths, especially agitated delirium, and make some suggestions aimed at their prevention and a better knowledge of the same. Also we review the medico-legal publications about torture, the methods used, and international protocols on its effective prevention and investigation, bothin living persons as well as in corpses (Minnesota and Istanbul Protocols). RESUMENSe reúnen en este trabajo de forma sucinta los conocimientos más útiles en la resolución forense de los traumatismos craneoencefálicos (TCE) letales. El estudio está orientado hacia la identificación macroscópica, la causalidad y los mecanismos de producción de las lesiones, con apoyo iconográfico. Se hace especial hincapié en la valoración de las fracturas craneales, las hemorragias subaracnoideas por lesión de la arteria vertebral, el edema y la hiperemia cerebral postraumáticos, así como algunas recomendaciones para la práctica de la autopsia de los fallecidos por trauma cefálico, incluida la retención y fijación del encéfalo. Algunos de estos conocimientos son antiguos y aunque olvidados o desconocidos por muchos han resistido el paso del tiempo, manteniendo plena vigencia. Igualmente se incluyen las características clínicas esenciales de cada trastorno, los nuevos avances en el diagnóstico neuropatológico de los TCE, en su fisiopatología y sus sospechadas relaciones con la posterior aparición de enfermedad degenerativa tipo Alzheimer y las alteraciones genómicas que predisponen a una evolución fatal de TCE ligeros o moderados. This study is aimed at the macroscopic evaluation, causality and production mechanisms of the lesions, with imagebased support.Skull fractures, subarachnoid hemorrhage due to lesion of the vertebral artery, posttraumatic cerebral edema and hyperaemia as well as basic recommendations to carry out the autopsy procedure (including the retention and fixation of the brain) in bodies with a lethal traumatic brain injury (TBI) will be analyzed.Some of this knowledge has been developed a long time ago, and although sometimes forgotten, their meaning is still fully patent.This study also points out the essential clinical characteristics of each condition and recent advances in the neuropathological diagnosis of traumatic brain injury (TBI) and its physiopathology. Finally, the suspected relationship of head injuries with the late development of degenerative disorders (Alzheimer-like); as well as new insights relating genomics and fatal evolution of mild to moderate TBIs are included. INTRODUCCIÓN:En los países desarrollados los traumatismos constituyen la principal causa de muerte en menores de 40 años, los traumatismos craneoencefálicos (TCE) son responsables de la mitad de estas muertes. Observaciones llevadas a cabo durante los últimos 15 años por expertos forenses han dado lugar al reconocimiento de un nuevo síndrome que puede llevar a la muerte súbita e inesperada. La muerte por agitación psicomotriz generalmente se acompaña por un procedimiento de restricción llevado a cabo por fuerzas y cuerpos de seguridad del estado, cuerpos de seguridad privada o personal hospitalario. Ello conlleva que dicho fallecimiento sea catalogado dentro del grupo de muertes en privación de libertad, muertes en custodia o deaths in custody. La frecuencia de este cuadro, llamativo por las circunstancias extrañas en las que se produce, es cada vez mayor ya que uno de sus principales factores de riesgo como es el consumo de drogas, fundamentalmente la cocaína, aumenta de forma alarmante en la sociedad de nuestros días. Por tanto, partiendo de la hipótesis de que la agitación psicomotriz de origen tóxico o psiquiátrico puede acabar en la muerte del sujeto tras una retención prolongada así como que algunos de los sujetos que sufren un cuadro de agitación psicomotriz son ingresados en el hospital durante o tras dicho episodio, nuestro objetivo ha sido realizar un estudio descriptivo utilizando para ello una muestra de 31 casos que requirieron atención médica en el Hospital de Poniente entre los años 2003 y 2005. Esta muestra la dividimos en dos subgrupos: • Sujetos con agitación psicomotriz y análisis cualitativo de tóxicos positivo y, • Sujetos con agitación psicomotriz y análisis cualitativo de tóxicos negativo. Con esta división en dos grupos realizamos un estudio comparativo entre ambos analizando estadísticamente diferentes variables con el fin de determinar la influencia de las drogas psicoactivas en los cuadros de agitación psicomotriz. Los resultados obtenidos ponen de manifiesto la falta de datos en las historias clínicas, el escaso número de estudios relacionados con el tema en cuestión y la ausencia de diferencias estadísticamente significativas entre los pacientes con agitación psicomotriz y consumo de tóxicos de los que no han consumido recientemente.
