RESUMENSe reúnen en este trabajo de forma sucinta los conocimientos más útiles en la resolución forense de los traumatismos craneoencefálicos (TCE) letales. El estudio está orientado hacia la identificación macroscópica, la causalidad y los mecanismos de producción de las lesiones, con apoyo iconográfico. Se hace especial hincapié en la valoración de las fracturas craneales, las hemorragias subaracnoideas por lesión de la arteria vertebral, el edema y la hiperemia cerebral postraumáticos, así como algunas recomendaciones para la práctica de la autopsia de los fallecidos por trauma cefálico, incluida la retención y fijación del encéfalo. Algunos de estos conocimientos son antiguos y aunque olvidados o desconocidos por muchos han resistido el paso del tiempo, manteniendo plena vigencia. Igualmente se incluyen las características clínicas esenciales de cada trastorno, los nuevos avances en el diagnóstico neuropatológico de los TCE, en su fisiopatología y sus sospechadas relaciones con la posterior aparición de enfermedad degenerativa tipo Alzheimer y las alteraciones genómicas que predisponen a una evolución fatal de TCE ligeros o moderados. ResumenSe presenta un caso de muerte súbita e inesperada en una mujer joven con antecedentes de esclerosis múltiple y espondilitis anquilosante que es encontrada muerta tras un breve cuadro de estreñimiento, dolor y distensión abdominal. En la autopsia, la apertura del abdomen mostró una extraordinaria dilatación del estómago repleto de secreciones. Este cuadro es un trastorno poco frecuente, de causa y patogenia desconocidas, que se relaciona con numerosas situaciones clínicas. AbstractWe report a case of sudden and unexpected death in a young woman with a history of multiple sclerosis and ankylosing spondylitis who was found dead after constipation, abdominal pain and bloating. At the autopsy the opening of the abdomen showed a largely distended stomach full of secretions. This condition is a rare disorder of unknown cause and pathogenesis, it is related to many clinical situations. The disorder has considerable mortality and the finding in the autopsy table is pathognomonic. IntroducciónLa dilatación aguda del estómago es un cuadro de escasa incidencia que se caracteriza por su potencial gravedad, con notable mortalidad, así como por su etiología y patogenia poco conocidas 1 . Se ha relacionado con postoperatorios de cirugía abdominal y pelviana con anestesia general, después de vagotomías y con varias situaciones patológicas como traumatismos contusos, quemaduras extensas, lesiones raquimedulares, corsé de yeso (cast syndrome), intoxicaciones por fenotiazinas y antidepresivos, diabéticos y sujetos con trastornos de la conducta alimentaria (anorexia nerviosa, bulimia nerviosa, trastorno por atracones, síndrome de Prader-Willi) y malnutridos que inician la nutrición por vía oral, entre otras 1-3 .El cuadro de dilatación gástrica aguda guarda relación con el conocido como síndrome de Wilkie o síndrome de la arteria mesentérica superior. Éste es también un trastorno poco frecuente y se atribuye al pinzamiento de la tercera porción duodenal por la arteria mesentérica superior, descrito por primera vez por Rokitansky en 1842 4 . Es especialmente frecuente en sujetos muy delgados, en quienes la desaparición de la grasa intraabdominal hace que el ángulo aórtico-mesentérico se haga más agudo y se produzca una especie de pinza aórtico-mesentérica sobre el duodeno 5 . ResumenEl taponamiento cardíaco es una complicación devastadora de los catéteres venosos centrales. Se debe en general a que la punta del catéter alcanza indebidamente las cavidades cardíacas derechas. Para la correcta identificación en la autopsia por el patólogo forense es importante que a los pacientes fallecidos en los hospitales que van a ser sometidos a autopsia no se les retiren los dispositivos que tengan insertados, e igualmente que no se les retiren en la sala de autopsias hasta que no se haya comprobado su correcta inserción. Se presenta un caso estudiado por nosotros y se analizan otros factores de riesgo de dicha complicación. AbstractCardiac tamponade is a devastating complication of central venous catheters. It is due to perforation of the atrial or ventricular wall during or after insertion. In order to identify this complication it is important that patients deceased in Hospitals and referred to Forensic Pathologists be sent with all the catheters and devices inserted. Also they should not be removed in the autopsy ward until their correct position has been addressed. A dramatic case is presented and the risk factors for these complications are analyzed. Se debe a que la punta del catéter queda indebidamente en el interior de la aurícula o del ventrículo derechos y perfora la pared cardíaca. Todos los casos de muerte por esta causa deberían ser objeto de autopsia médico-legal, tanto si se presentan como una supuesta mala praxis profesional médica o como una muerte inesperada en un paciente hospitalizado². Para el patólogo forense que se enfrenta a uno de estos casos es fundamental el adecuado reconocimiento y la objetivación de dicha complicación en la autopsia.Hay dos medidas muy importantes en la autopsia médico-legal que venimos practicando desde la puesta en funcionamiento del Servicio de Patología Forense del Instituto de Medicina Legal. La primera es comunicar insistentemente a los hospitales de la provincia que cuando fallezca un paciente que ha de ser sometido a autopsia médico-legal no se le retiren los dispositivos que tenga insertados en el cuerpo (tubos orotraqueales, sondas de todo tipo, drenajes, catéteres intravenosos, etc.); esta petición se hace también a los servicios médicos de emergencias para el caso de pacientes sometidos a medidas de reanimación avanzada y que fallezcan antes de la hospitalización.
