Tyrimo pagrindimas. Nors jau 1996 metais atlikta studijų metaanalizė, kurioje nurodomos galvospadėties pokyčių ir smilkininio apatinio žandikaulio sąnario (SAŽS) disfunkcijų sąsajos (Harrison etal., 1996), tačiau apatinio žandikaulio (AŽ) nukrypimai dažniausiai vis dar gydomi kaip vienintelėpatologija, o tai lemia menką klinikinį efektą (Lee et al., 2017). Manoma, kad sąkandžio problemosgali būti susijusios ne tik su žandikaulių ir kaukolės padėtimi, bet ir su pečiais, stuburu, kurie veikiakaip biomechaninė grandinė (Moreno, Aranza, 2013; Shousha et al., 2018). Kadangi vis dar trūkstaįrodymų, kaip koreguojant galvos padėtį keičiasi kramtomosios sistemos funkcijos, atlikome tyrimąnorėdami nustatyti, ar vienkartinė galvos padėties korekcija turi įtakos kramtomajai sistemai.Tikslas – nustatyti vienkartinės galvos padėties korekcijos poveikį smilkininio apatinio žandikaulio sąnario funkcijai.Metodai. Laikysenos, SAŽS judesių amplitudžių, AŽ nuleidimo judesio stereotipo vertinimas.Rezultatai. Po vienkartinės galvos padėties korekcijos pakito galvos pasvirimas pirmyn (MentėD 5,65°; MentėK 3,9°). Apatinio žandikaulio nuleidimo amplitudė reikšmingai sumažėjo (MentėD15,09 mm, MentėK 15,58 mm), nuokrypis nuleidimo judesio metu sumažėjo (MentėD 2,06 mm;MentėK 2,38 mm). Skirtumas tarp AŽ nuokrypio į šonus reikšmingai sumažėjo (MentėD iki 0,32mm; MentėK iki 1,60 mm).Išvados. Vienkartinė galvos padėties korekcija reikšmingai sumažina asmenų, turinčių viršutinįkryžminį sindromą, galvos pasvirimo pirmyn kampą, apatinio žandikaulio nuleidimo amplitudę irnuokrypį nuo centro linijos, nesimetrišką menčių padėtį.
Tyrimo pagrindimas. Šoninių čiurnos raiščių traumos – vienos iš dažniausiai pasitaikančių sporto srityje. Jos paveikia visų amžiaus grupių atletus. Nustatyta, kad vienas iš 10000 fiziškai aktyvių žmonių kasdien patiria šių raiščių traumą (White et al., 2015). Didelio meistriškumo sportininkams ir fizinę veiklą propaguojantiems asmenims ypač svarbu greitai atsigauti po patirtos traumos stengiantis išvengti fizinių, psichologinių ir ekonominių pasekmių. Ilgą laiką po šoninių čiurnos raiščių traumų buvo taikoma imobilizacija, trunkanti apie 6 savaites, tačiau šiomis dienomis pirmenybė teikiama ankstyvuoju laikotarpiu taikomai kineziterapijai (Kerkhoffs et al., 2013). Tikslas – atlikti sisteminę tyrimų analizę ir nustatyti ankstyvuoju laikotarpiu taikytos kineziterapijos poveikį čiurnos skausmui ir funkcijai po šoninių čiurnos raiščių pažeidimo. Metodai. Naudojantis Pubmed, Ebsco, Pedro, Google mokslinčių duomenų bazėmis, atrinkti klinikiniai tyrimai, kurių metu buvo taikoma ankstyvoji kineziterapija malšinant skausmą ir gerinant funkcinę būklę. Rezultatai. Kontrolinių grupių, kurioms taikyta tradicinė imobilizacija ir RICE metodas, skausmas sumažėjo iki 1,96 ± 1,33 balo, intervencijų grupėse, kurioms taikyta ankstyvojo laikotarpio kineziterapija ir RICE metodas, iki 1,53 ± 1,23 balo. Efekto dydis kontrolinėse grupėse – r = 0,55 balo, intervencijų grupėse – r = 0,54 balo. Kontrolinių grupių, kurioms taikyta tradicinė imobilizacija ir RICE metodas, funkcinės būklės įvertinimai pagerėjo iki 68,67 ± 16,79 balo, o intervencijų grupėse, kurioms taikyta ankstyvojo laikotarpio kineziterapija ir RICE metodas, iki 75,61 ± 13,27 balo. Efekto dydis kontrolinėse grupėse – r = 0,82 balo, intervencijų grupėse – r = 0,88 balo. Išvados. Analizuotų tyrimų duomenimis, po patirtos šoninių čiurnos raiščių traumos RICE metodika, derinama su ankstyvuoju laikotarpiu taikoma kineziterapija ir apsaugai naudojant minkštą, lankstų įtvarą arba judesio korekcijos juostas, padeda greičiau sumažinti skausmą, pagerinti funkcinę būklę ir grįžti į sportinę bei darbinę veiklą.
