INTRODUÇÃO: O acidente vascular cerebral (AVC) é atualmente a primeira causa de morte no Brasil. O risco de recorrência de AVC é de aproximadamente 30% em cinco anos. Agentes antiagregantes plaquetários são frequentemente prescritos para a prevenção de recorrência do AVC, porém sem um parâmetro laboratorial de ajuste terapêutico. OBJETIVO: Determinar a aplicabilidade da curva de agregação plaquetária, no controle do antiagregante, na prevenção secundária do acidente vascular cerebral isquêmico (AVCi). MÉTODO: Estudo retrospectivo de janeiro de 1990 a janeiro de 2001; foram analisados 189 pacientes com diagnóstico de AVCi acompanhados durante este período com curva de agregação plaquetária. Foram excluídos os portadores de doenças cardioembólicas. Consideraram-se hipoagregados os doentes com agregação plaquetária ativada pelo ADP e/ou pela adrenalina menor ou igual a 50%. Os doentes tiveram acompanhamento por 1 a 6 anos e a taxa de recorrência de AVCi foi comparada entre os normoagregados e os hipoagregados. RESULTADOS: Nove doentes apresentaram novo AVCi no período do seguimento. Entre os hipoagregados ocorreu 2,6% de recidiva e entre os normoagregados, 15,1% (0,03<OR<0,69, p<0,05). A maioria dos doentes tiveram o novo episódio no segundo ano após o primeiro evento. CONCLUSÃO: A curva de agregação plaquetária mostrou ser um exame laboratorial eficiente como auxiliar para o controle da antiagregação na prevenção secundária do AVCi.
Há anos, a relação entre álcool e doença cerebrovascular (DCV) tem sido firmemente estabelecida, seja na forma oclusiva ou hemorrágica [1][2][3] . O efeito do álcool etílico sobre a aterosclerose, em especial das artérias carótidas extracranianas, é motivo de controvérsias, havendo poucos estudos que examinaram esta relação e seu mecanismo 4-6 . Décadas de pesquisas epidemiológicas têm demonstrado que a aterosclerose tem uma etiologia multifatorial, dificultando a análise quantitativa e qualitativa de um fator de risco isoladamente, os quais interagem entre si [7][8][9][10] .Parece existir uma associação inversa significante entre baixo consumo de álcool e aterosclerose carotídea 11,-15 , a qual é demonstrada em forma de curva tipo "U" 6,16 ou "J" 17 . Estes dados poderiam explicar o fato de que o baixo ou o moderado consumo de álcool tem ação protetora contra DCV oclusiva 18 . Porém, existem autores que não encontram esta associação 19 .O objetivo deste estudo é avaliar, através de exame de ultrassonografia colorida com Doppler, a presença de placas de ateroma e o grau de estenose em artérias carótidas extracranianas em abstêmios e etilistas com diferentes graus de consumo alcoóli-co. Diferentemente dos trabalhos citados na revisão acima, somente o consumo alcoólico deverá ser o fator de risco existente; doentes com outros fatores RESUMO -Existem fortes evidências de menor incidência de doença cerebrovascular oclusiva, de aterosclerose coronariana e de outros vasos em indivíduos com consumo leve ou moderado de álcool. Este estudo procura analisar o efeito do etanol, em diferentes doses no comportamento da aterosclerose carotídea extracraniana. Através do ultrassom Doppler colorido, foram investigadas 328 artérias carótidas extracranianas, de homens e mulheres brancos, com mais de 35 anos de idade, normotensos, não tabagistas e sem as principais doenças que constituam fatores de risco para doenças cardiovasculares. Foram divididos de acordo com o consumo de álcool por semana (em mililitros) em abstêmios, etilistas leves (1 a 100), moderados (101 a 300) e pesados (301 ou mais). Houve menor incidência de placas de aterosclerose e de estenose naqueles que ingeriram moderada quantidade. Conclusão: O estudo sugere uma ação protetora do álcool etílico para aterosclerose carotídea, quando ingerido em moderada quantidade. PALAVRAS-CHAVE: álcool, carótidas, aterosclerose. Interest in the function of subcortical structures in cognitive function has grown in the last decades. Such interest has increased after the advent of modern innovations in radiology techniques, which provide a better visualization of subcortical lesions, like computerized tomography (CT) and magnetic resonance imaging (MRI). Cortical blood flow and metabolism studies also brought a better understanding of diaschisis mechanisms in vivo and improved our knowledge about the pathophysiology of subcortical lesions. The purpose of this study was to identify language and/or speech alterations in patients with exclusively subcortical lesions and to establish a profile of such alterations. The author compared the profiles obtained from thalamic and non-thalamic lesions, trying to define some pathophysiologic mechanisms subjacent to the lesions.To achieve this purpose, the author studied a group of 16 patients (9 patients with basal ganglia and adjacent white matter pathways lesions and 7 patients with thalamic lesions), chosen on the basis of their CT scan findings, selected from the Emergency Room of Hospital das Clínicas of the University of São Paulo School of Medicine (USP) and Hospital Universitário (USP). All patients were submitted to CT scan in the acute stage of illness, 13 patients were submitted to MRI and 12 patients were submitted to single photon emission computerized tomography (SPECT), in order to evaluate cortical blood flow. Patients were also submitted to language evaluation, using the Boston Diagnostic Aphasia Examination, Boston Naming and Token Test. Seventeen normal subjects were evaluated by the same language batteries as a control group.Articulatory and motor disturbances predominated in the non-thalamic group, and language alterations were poor (only one third of the patients showed some language disturbance). There was no characteristic profile in this group.In the thalamic group, however, there was a higher incidence of language disabilities, specially naming disturbances (5 cases) and comprehension disturbances (4 cases). Articulatory disabilities were found in only one patient. There was a high prevalence of cortical flow disturbances in these patients, compromising classical cortical language areas (in the frontal and temporal lobes), making it difficult to exclude cortical dysfunction as a contributing factor to explain the alterations.In the thalamic aphasias, the author also concludes about the possibility of verbal memory and attentional mechanisms disturbances impairing primary language processing; such aspects deserve future studies.
