Intussusception is a rare cause of bowel obstruction in adults, and has generally an organic etiology. However, adenomatous polyp of the small bowel is an uncommon etiology. Moreover, there’s a great difference with childhood intussusception in its presentation, etiology and management. We describe herein a case of adult ileocecal intussusception due to an adenomatous ileal polyp with a preoperative diagnosis made on computed tomography. We performed a right hemicolectomy, without attempting to reduce the intussusception, and an end-to-end ileotransverse anastomosis. The pathological examination of the surgical specimen revealed an adenomatous polyp with a high grade dysplasia on the terminal ileum, being the cause of the ileocecal intussusception.
Lithopedion is defined as the evolution and calcification of an unrecognized intra-abdominal (usually ectopic) pregnancy, following a fetal death after 3 months of gestation.The term lithopedion comes from the Greek words "lithos" meaning "stone" and "paidion" meaning child [1].The fetus is retained in the maternal abdominal cavity or genital tract. It may then undergo a process of calcification, its tissues being more or less infiltrated with calcareous salts. The placenta, free membranes, and umbilical cord may also be calcified. It may be manifested by clinical signs such as abdominal pain [3]. It may also be discovered during surgery for a completely different indication or during imaging or autopsy examinations [4].
La grossesse abdominale (GA) se définit comme la nidation et le développement de l'œuf fécondé dans la cavité péritonéale. Elle reste une variété rare de grossesse ectopique, dont la découverte peut être fortuite simulant une urgence chirurgicale. Nous rapportons un cas rare d'une occlusion intestinale aiguë fébrile révélant une grossesse abdominale chez une patiente âgée de 27ans sans antécédents pathologiques particuliers, reçue pour la prise en charge d'un syndrome occlusif évoluant depuis une semaine. L'examen physique retrouvait un abdomen distendu luisant, sensible dans son ensemble et météorisé. Le bilan biologique montrait des globules blancs à 20300, le taux d'hémoglobine à 7,2g/dL. L'exploration chirurgicale avait objectivé un hémopéritoine de 2000ml, une masse retro-utérine encapsulée, adhérent fortement au méso-sigmoïde et au sigmoïde responsable d'une sténose du colon descendant. Il a été réalisé une exérèse monobloc dont l'effraction montrait un placenta accolé au méso-sigmoïde relié au fœtus, une annexectomie droite, une colostomie.
Le lipome est une tumeur bénigne des parties molles. Il s´agit d´une prolifération bénigne d´adipocytes matures. Il est qualifié de géant lorsque son poids dépasse 1 kg ou son diamètre supérieur à 5 cm. La gêne fonctionnelle et esthétique peut être un motif principal d´exérèse chirurgicale. Ils peuvent se localiser partout, mais surtout au niveau de la face postérieure du thorax. Nous rapportons un cas de lipome géant de la face postéro-supérieure gauche du thorax.
The consequence of a monstrous and diffuse evolution of the usual epithelial hyperplasia of pregnancy, gravidic gigantomastia is a disabling pathology. Rarely, it poses a problem for management and its etiology is still undetermined. In spite of a benign condition, it is nonetheless a serious one because of the complications associated with excessive growth, the weight of the mammary gland and the compromise of breastfeeding function (through destruction of the nipple-areolar plate and surgery). We report a case of bilateral gravidic gigantomastia in a 31-year-old woman to discuss the diagnostic and therapeutic aspects.
