Ultrasound is able to have a multifaceted therapeutic effect in periodontitis. In therapeutic doses, it affects a variety of local tissue reactions, stimulates compensatory, restorative, protective and adaptive mechanisms.Ultrasound in dosages used in clinical practice does not have an independent, direct antibacterial effect. In this regard, in the treatment of periodontitis, where the microbial factor plays an important role, it is advisable to use antiseptic solutions as a contact medium for ultrasonic effects. The study of the comparative effectiveness of low-frequency and high-frequency ultrasonic effects in combination with the domestic antiseptic preparation miramistin, which, with high antibacterial effectiveness, is characterized by the absence of adverse side effects, in the treatment of periodontitis, is of significant scientific and practical importance.
Changing the vestibulo-oral inclination (torque) of the frontal teeth is an important component of orthodontic treatment. Cone-beam computed tomography is recommended as an accurate diagnostic tool allowing the orthodontist to estimate the safety of frontal tooth movement and inclination. This tool is helpful in measuring bone thickness at different levels of the tooth root and estimating incisal inclination and position. The aim of our study was to analyze bone thickness in patients with pathologically and normally inclined teeth using CT images and to create a universal table that will provide useful information about the thickness of the bone around different segment of the root required for a safe change in tooth inclination. Using the proposed table, the orthodontist can assess the feasibility of the planned tooth movement in the setting of changed tooth inclination, with due account of critical bone deficit regions. This will ensure the safety of tooth movement, stable retention and a positive treatment outcome.
Epidemiological studies have reported the prevalence of inflammatory periodontal diseases of 85%90% in individuals aged 30 years. Predicting the possibility of preserving teeth is an important integral part of treatment planning for periodontitis. Traditionally, the prognosis is based on the likelihood of tooth loss. The article presents a review of the literature characterizing the factors that affect the prognosis of the service life of teeth in case of periodontitis. For the literature review, a literature search was performed on the MedLine, EMBASE, CyberLeninka, and eLibrary databases. The following factors should be considered when assessing the long-term prognosis in patients with the highest risk for tooth loss: severity of periodontitis, smoking, bruxism, number of abutment teeth, age, type of tooth, depth of probing the periodontal pocket, bone loss, furcation involvement, root mobility and proportions, and tooth crowns. Indicators for molar loss include furcation lesion, bone loss progression, endodontic treatment, persistent periodontal pocket, age, female sex, smoking, and diabetes mellitus. A favorable treatment result and preservation of teeth depend on the entire group of teeth, with the following values: 85% in single-rooted anterior teeth, 78% in premolars, and 47% in molars. The low accuracy of standard predictive tooth loss indices can be explained by the point that the indices are mainly based on factors related to teeth and do not take into account the influence of patient-related factors. Thus, predicting the service life of teeth with periodontitis is a difficult task and requires taking into account both individual indicators of periodontal status and general factors.
Аннотация. В статье проводится обзор и сравнительный анализ существующих видов и методов диагностики зубочелюстных аномалий на этапе подготовки и планирования ортодонтического лечения. Определяется и доказывается целесообразность проведения комплексной диагностики на ранних этапах подготовки к проведению ортодонтического лечения. Устанавлива-ется связь между методами различных видов диагностики и процентом рецидивов в ретенционном периоде, делается вывод об отсутствии единой методики проведения диагностики, недосточной ее полноте, что в свою очередь оказывает существен-ное влияние на ход лечения, а впоследствии на возникновения рецидивов. В рамках определения и анализа установленных взаимосвязей предлагаются подходы к снижению процента рецидивов на основе оптимизации процессов и методов долечеб-ной диагностики. Ключевые слова: зубочелюстные аномалии, виды диагностики, лучевая диагностика, ортодонтическое лечение, ретенцион-ный период, рецидивы, компьютерная томография.В настоящее время в структуре стоматологических заболеваний аномалии положения зубов занимают тре-тье место после кариеса зубов и болезней пародонта и имеют тенденцию к росту. Среди обследованного населения, аномалии положения зубов отмечены, по данным разных авторов в 38,0 − 75,0% случаев [1].Согласно статистике, сегодня среди всех зубоче-люстных аномалий около 35% составляют аномалии положения отдельных зубов. Скученное положение зу-бов, по данным ряда авторов, колеблется от 5,5% до 47,7%. Особенно важным является тот факт, что со-гласно данным после проведенного ортодонтического лечения у 18,9 % взрослых и у 36,8% детей наблюда-ется развитие рецидива [2]. В качестве решения данной проблемы сегодня предлагается проводить длитель-ную ретенцию для стабилизации достигнутого резуль-тата. Однако это не всегда гарантирует достижение по-ставленной задачи по сохранению результата ортодон-тического лечения у пациента.Зубочелюстные аномалии чаще выявляются в смен-ном прикусе и старших возрастных группах детей. Преимущественно формируются во фронтальном от-деле верхнего и нижнего зубного ряда. В результате чего возникают проблемы различного характера: ску-ченное положение резцов верхней и нижней челюсти, нарушение линии улыбки, асимметрия лица, (эстети-ческие жалобы), так же отмечаются функциональные нарушения − нарушение захвата и откусывания пищи, звукообразования и другие.Разнообразие зубочелюстных аномалий очень ве-лико и большое значение приобретает их классифика-ция, использование которой помогает в краткой фор-мулировке охарактеризовать сущность патологии. Классификация необходима для учета отклонений от нормы, систематизации, выбора плана лечения, меж-дународного общения. This study aimed to analyze social, clinical, and laboratory indicators that contribute to the maintenance of oral sanitation based on current literature. This article analyzed data from foreign and domestic studies indexed in PubMed, CyberLeninka, MEDLINE, Web of Science, Embase, and Scopus. Literature data from the last 15 years were used. Epidemiological data have shown that dental caries affects 90% of the adult populations in the Russian Federation and European countries. The most important role in maintaining oral health care is determined by the complex composition of the microbial community. To ensure caries-free oral cavity from childhood, individual oral hygiene, nature of nutrition, and mineralization of tooth enamel must be emphasized. Caries-specific markers can be saliva proteins. Caries resistance is also caused by differences in the composition of the crystal lattice of enamel apatites and is characterized by an increase in the degree of substitution of the PO4 3group by CO3 2. Maintaining oral hygiene and healthy teeth is influenced by a combination of factors such as the microbial composition of saliva, peculiarities of the metabolism in biofilm, genetic factors, structure of the crystal lattice of enamel apatites, nature of nutrition and peculiarities of individual oral hygiene, local use of fluorides, and educational level.
