Мета-ретроспективно оцінити перебіг пологів і стан новонароджених у жінок із пролонгованою та переношеною вагітністю, з метою визначення значущості цієї патології для перинатальних наслідків. Пацієнти та методи. Проведено ретроспективний клініко статистичний аналіз 197 історій вагітності й пологів жінок, які перебували на лікуванні та розродженні у відділенні патології вагітності і пологів ДУ «Інститут педіатрії, акушерства і гінекології імені академіка О.М. Лук'янової НАМН України» за 2011-2015 рр. Вивчено перебіг пологів і стан новонароджених у жінок із пролонгованою та переношеною вагітністю. Основну групу становили 156 жінок. З них пологи в 41 тиж.-110 (70%), 41-42 тиж.-33 (21%), у 42 тиж.-13 (9%). Групу контролю становила 41 вагітна, розроджена своєчасно. Результати. Наведені дані свідчать про значні перинатальні ризики в жінок із пролонгованою та переношеною вагітністю. Так, фізіологічні пологи відбулися лише в 58,9% жінок (контрольна група-80,5%; р<0,05). Відповідно, патологічними були 41,1% пологів в основній групі. Оперативно розроджено 32,7% вагітних, що майже втричі більше, ніж у контрольній групі (12,2%). Достовірно вищим було значення показника дистресу плода в жінок основної групи (23,7%) порівняно з вагітними контрольної групи (9,7%, р<0,05). Пологовий травматизм спостерігався у 37,2% вагітних основної групи та у 17,0% жінок контрольної групи. Усього обстежені жінки основної групи народили 157 новонароджених-80 хлопчиків і 77 дівчаток. Випадків перинатальної смертності не було. Висновки. У жінок із пролонгованою та переношеною вагітністю значно зростають перинатальні й акушерські ризики. Запізнілі пологи призводять до необхідності оперативних втручань при розродженні, а також до високого рівня неонатальної захворюваності та пологового травматизму. На підставі виявлених ускладнень слід формувати групи ризику щодо переношеної вагітності. Ключові слова: пролонгована вагітність, переношена вагітність, дистрес плода, пологовий травматизм.
