Хронические заболевания вен (ХЗВ)-многокомпонентный процесс, разворачивающийся во времени, в основе которого лежат изменения гемодинамики на макро-и микроциркуляторном уровнях. Меняющаяся «сила сдвига» стенки вены приводит к активации лейкоцитов, макрофагов, секреции медиаторов воспаления, в том числе ICAM-1, VEGF, матриксных металлопротеиназ, изменению профиля гладкомышечных клеток на секреторный тип. Итогом становятся деградация и ремоделирование венозной стенки, реализующиеся через каскад воспалительных реакций. Проницаемость сосудистой стенки меняется, что приводит к экстравазации плазмы и макромолекул. Высвобождаемые в результате гемодинамических сдвигов и гипоксии провоспалительные медиаторы могут активировать ноцицепторы, расположенные в венозной стенке между эндотелием и гладкомышечными клетками средней оболочки. Разворачивающееся воспаление обусловливает появление венозных симптомов, в том числе венозной боли, отеков, тяжести в ногах, а также других, типичных для варикозной болезни. Современные представления о патофизиологических механизмах позволяют выделить потенциальные терапевтические мишени, которые могут быть эффективными в уменьшении и устранении симптомов и признаков ХЗВ. Имеющиеся в настоящее время на фармацевтическом рынке флеботропные препараты обладают рядом особенностей, обусловливающих их эффективность в отношении тех или иных симптомов ХЗВ. Многочисленные исследования, в том числе метаанализы, дают возможность определить эффективность действия флеботропных препаратов в отношении различных симптомов и присвоить им разные уровни рекомендаций, что лежит в основе индивидуализированного назначения флеботропных препаратов пациентам с симптоматическим течением ХЗВ. Модификация местных лекарственных средств с добавлением эссенциальных фосфолипидов, действующих как трансдермальная система доставки активных компонентов, повышает эффективность препаратов, позволяя включать современные мази в комплекс лечения ХЗВ вместе с системными препаратами.
