У статті представлено проблеми виникнення післяопе-раційної евентрації. Розглянуто питання стосовно її етіології, патогенезу, класифікації, запобігання розвитку та лікування. Особливу увагу приділено способам запобігання післяопераційної евентрації шляхом превентивної абдомінопластики із застосу-ванням сітчастих алотрансплантатів. Відмічено їх переваги та недоліки, а також зазначено питання, які потребують подальшо-го вивчення.Ключові слова: післяоперційна евентрація, сітчастий алотрансп-лантат.Евентрація -це гостро розвинутий дефект в очеревині та м'язово-апоневротичному шарі, внаслідок чого створюється умови для розгерме-тизації черевної порожнини та виходу нутрощів за її межі [7,19, 24, 26].Частота виникнення цього ускладнення, за даними вітчизняних та іноземних авторів, стано-вить від 0,5 до 2,35%, а повторної -12,5% [7, 8].Післяопераційна евентрація є одним із найбільш небезпечних ускладнень в абдомі-нальній хірургії, при виникненні якого відзначаєть-ся досить висока летальність, яка сягає до 20%, а за даними деяких авторів -до 65%. Особливо велика летальність у випадку виникнення евент-рації на тлі гнійної рани [8,19, 24, 26].Високий відсоток летальних випадків пояс-нюється супутньою патологією або післяопера-ційними ускладненнями такими, як перитоніт, кишкові нориці, непрохідність кишечнику тощо [21].На сьогодні існує досить значна кількість кла-сифікацій післяопераційної евентрації, проте вони мають ряд недоліків, що не дозволяє на їх основі вибрати оптимальну хірургічну тактику [6, 8].Основуючись на результатах досліджень ба-гатьох авторів, найбільш об'єктивною та прак-тичною слід вважати класифікацію, за якою роз-різняють чотири ступені післяопераційної евент-рації [8].При І ступені (підшкірна евентрація) має місце розходження всіх шарів черевної стінки, крім шкіри. При ІІ ступені (часткова евентрація) визна-чається повне розходження всіх шарів черевної стінки, але внутрішні органи залишаються в ме-жах черевної порожнини. При ІІІ ступені (повна евентрація) спостерігається повне розходження всіх шарів черевної стінки, при цьому рана запов-нена нутрощами. IV ступінь (справжня евентра-ція або евісцерація) -це вихід нутрощів за межі черевної стінки.Також усі післяопераційні евентрації умовно розділяють на дві групи -асептичні та септичні. Під асептичною евентрацією розуміють розход-ження країв рани та вихід нутрощів назовні, але за умови відсутності перитоніту та явних ознак ра-нової інфекції. Септична евентрація безпосеред-ньо пов'язана з первинним нагноєнням рани або вторинним її інфікуванням у зв'язку із розвитком перитоніту. Цей розподіл умовний, але саме він передусім визначає тактику лікування даної пато-логії [8, 18].Механізм розвитку евентрації складний. Будь-яка післяопераційна евентрація у своєму розвитку проходить стадію підшкірної. Ascites syndrome (AS) in chickens represents a major cause of mortality and the most frequent metabolic-related cause of abattoir carcass condemnation in broilers. Susceptibility of broiler chickens to ascites is determined by a complex interaction between genetically-controlled factors (as high-rate metabolism and peculiarity of the cardiovascular system) and exogenous factors such as nutrition, and temperature. Endocrine imbalances, especially thyroid-gland dysfunctions, are currently regarded as key-elements in the development of AS. The current case represents the first spontaneous case of hypothyroidism (goiter) associated with AS in broilers, bringing additional arguments to the theory which claims the role of thyroid dysfunction in this syndrome. Further studies of spontaneous cases which must include assessment of the thyroid hormones need be performed in order to better understand the link between thyroid gland dysfunction and the cardiopulmonary changes in the context of AS in birds.

A 2-year-old female llama (Lama glama) from a private zoological park was submitted to the Pathology Department of the Faculty of Veterinary Medicine from Cluj-Napoca (Romania) for necropsy. The animal had a history of depression, anorexia, progressive weakening, ataxia, decubitus ulcers, fever, dyspnea, and abdominal distension. Death occurred 6 weeks after the onset of clinical signs. Necropsy, and cytological, histological, and immunohistochemical examinations were performed. Grossly, the animal was emaciated. Subcutaneous edema of the posterior limbs, abdomen, and thorax, hydrothorax, hydro-pericardium, and severe ascites (approximately 15 liters of fluid) with fibrin clots adherent to the visceral peritoneum, and multiple, acute, gastric ulcers were observed.A firm, white-gray, multinodular mass, 25 cm in diameter was found in the liver parenchyma. Moreover, multiple small nodules, unencapsulated, with poorly defined margins, and ranging from 0.5 to 4 cm in diameter scattered throughout the liver parenchyma were present. The cut surface of the tumors varied from gray-white to red-brown (Fig. 1). Multiple areas of necrosis, a few microabscesses, and venous thrombosis were also found within the liver parenchyma. Similar nodular lesions affecting the mesenteric, tracheobronchial, and submandibular lymph nodes, lungs (Fig. 2), parietal pleura, pericardium, diaphragm, the serosal surface of the gut, and peritoneum were present. Lesions were umbelicated, firm, and gray, and ranged from 0.5 to 3 cm in diameter. The lymph nodes were markedly enlarged and multifocally replaced by metastatic lesions. No tumors were detected in other organs, including kidney, stomach, pancreas, and mammary glands.Samples from liver, lung, lymph nodes, diaphragm, and pericardium were evaluated by cytological, histological, and immunohistochemical examinations. For cytology, multiple smears from the hepatic masses were stained by Dia-Quick Panoptic.a For histology, the samples were fixed in 10% phosphate buffered formalin for 24 hr, embedded in paraffin wax, cut into 3-5 µm sections, and stained with hematoxylin and eosin and periodic acid-Schiff (PAS). Histological interpretation of lesions was done according to the World Health Organization Classification of Tumors of the Alimentary System of Domestic Animals.6 For immunohistochemistry, sections from liver mass, lung nodules, and normal liver and lung tissue (inner control) were selected. After dewaxing, heat-induced epitope retrieval pretreatment, and endogenous peroxidase inactivation (H 2 O 2 1% in phosphate buffered saline b [PBS]), slides were incubated with goat serum 10% in PBS for reducing background staining. Sections were incubated with the following mouse monoclonal primary antibodies: broad spectrum cytokeratins (panCK), c CK20, c CK19, c CK7, c thyroid transcription factor 1 (TTF1), c and carcinoembryonic antigen (CEA), b for 24 hr at 4°C. Annotation. One of the main factors of postoperative eventration is intra-abdominal hypertension, which occurs in various surgical pathologies of the abdominal cavity. Despite the presence of a large number of scientific papers on the negative effect of intra-abdominal hypertension on the morphological state of granulation tissue in the area of the laparotomy wound, there are no publications on the impact on the strength of the postoperative scar. Therefore, the study aimed to investigate in an experiment on small laboratory animals the effect of intra-abdominal hypertension on the mechanical strength of the postoperative scar of a laparotomy wound. The experiment was performed on 120 laboratory rats, which underwent a median laparotomy and brought together the edges of the musculoaponeurotic layer of the anterior abdominal wall with simple nodal sutures. The main group consisted of 72 animals who developed intra-abdominal hypertension by inserting a container (condom) with a certain amount of Furacilin into the abdominal cavity. The comparison group consisted of 48 animals who had an empty condom inserted into the abdominal cavity after laparotomy. The mechanical strength of the postoperative scar of the laparotomy wound was determined by the method of G. V. Petrovich (2010) on the 1st, 3rd, and 5th day after the creation of intra-abdominal hypertension, by measuring the level of intra-abdominal pressure at the time of rupture of the postoperative scar of the laparotomy wound. Statistical analysis of the results was performed using Microsoft Excel spreadsheets and a package of statistical processing software PAST. Differences between study groups were determined using Mann-Whitney criteria. The results of the study indicate that the created intra-abdominal hypertension leads to a decrease in the mechanical strength of the postoperative scar of the laparotomy wound. The degree of the negative impact of intra-abdominal hypertension on the strength of the postoperative scar is inversely proportional to the level of intra-abdominal pressure.
