Tracheal intubation is performed as part of daily routine in the operating room, rarely with complications. However, management of airway for cases such as a penetrating neck trauma case might constitute exceptions, in which cases the stabilization of the neck to prevent any further neural damage is a significant source of concern for the anesthesiologist. Generally, intubation techniques for penetrating neck trauma were planned according to the initial position of patients. To our knowledge, this is the first case report of alterated the position of the patient during the anesthesia induction for direct laryngoscopy. We report a case of successful airway management of a patient with penetrating neck trauma, by endotracheal intubation with direct laryngoscopy (DL) technique.
West syndrome (WS) is an epileptic encephalopathy usually occurring during the first year of life and is characterized by severe electroencephalography (EEG) derangement. Most of these patients may develop cerebral palsy, facial malformations, and skeletal deformities. The anaesthesiologist should make the preoperative assessment carefully due to epileptic seizures and should consider the possibility of difficult intubation because of coexisting anatomic malformations during the anaesthesia management of patients with WS. This report presents a case of general anaesthesia management in a left femoral fixation operation in an 11-year-old, 18 kg male patient.
ÖZ Amaç: İntraabdominal basınç (İAB) yüksekliği yoğun bakım hastalarında morbidite ve mortaliteyi artıran önemli bir etmendir. Bu çalışmada yoğun bakım hastalarında İAB artışı ile ilgili risk faktörlerinin araştırılması amaçlanmıştır. Gereç ve Yöntem: Çalışmaya cerrahi ve reanimasyon yoğun bakım ünitesinde 24 saatten fazla kalan 125 hasta dahil edildi. Bütün hastaların yaş, cinsiyet, vücut kitle indeksleri, APACHE II ve SOFA skorları kaydedildi. Yatış süresi boyunca İAB ölçümü yapıldı, devamlı veya tekrarlayan 12 mmHg üzerinde ölçülen İAB değerleri intraabdominal hipertansiyon (İAH) olarak kabul edildi. Yeni organ disfonksiyonuna eşlik eden İAB değerinin sürekli olarak 20 mmHg üzerinde olması Abdominal Kompartman sendromu (AKS) olarak kabul edildi. Tüm hastaların mekanik ventilatör, yoğun bakım kalış süresi ve prognozları saptandı. İAB risk faktörleri olan travma, sepsis, çoklu kan transfüzyonu, ileus, asidoz ve pnömoni durumları kaydedildi. Bulgular: Çalışmada 45 hastada İAH saptanırken, 5 hastada AKS saptandı. Çalışmaya katılan hastaların cinsiyet ve yaşlarına göre İAH ve AKS açısından farklılık saptanmadı. Vücut kitle indeksi yüksek olan, çoklu transfüzyon uygulanan, sepsis ve pnömoni tanılı hastalarda İAH daha yüksek olarak bulunurken (p<0,05), AKS bakımından fark saptanmadı. Travma geçiren hastalarda İAH ve AKS açısından anlamlı farklılık saptanmadı. İleus görülen hastalarda İAH ve AKS anlamlı derecede yüksek bulundu. Çalışmaya katılan hastalarda asidoz için AKS açısından anlamlı farklılık saptandı (p<0,05). İAH saptanan hastalarda daha yüksek APACHE II ve SOFA skorları, daha uzun yoğun bakım ve mekanik ventilatörde kalış süreleri saptandı (p<0,05). Sonuç: Vücut kitle indeks yüksekliği, sepsis, çoklu transfüzyon, ileus, asidoz ve pnömoni İAH ve AKS gelişmesi için önemli risk faktörleri olup, bu risk faktörleri varlığında hastaların daha dikkatli takip edilmesini önermekteyiz. Anahtar Kelimeler: Yoğun bakım, intraabdominal basınç, Abdominal Kompartman sendromu ABSTRACT Objective: Elevated intraabdominal pressure (IAP) is an important factor that increases morbidity and mortality in intensive care unit patients. In this study, it was aimed to investigate the risk factors related to IAP increase in intensive care unit patients. Materials and Methods: One hundred twenty five patients who stayed more than 24 hours in surgical and reanimation intensive care unit were included into the study. All patiens age, sex, body mass index, APACHE II and SOFA scores were recorded. IAP measurements were performed during the intensive care unit stay, intraabdominal hypertension (IAH) was approved by a sustained or repeated pathological elevation in IAP ≥12 mmHg. Abdominal compartment syndrome (ACS) was accepted as a sustained IAP >20 mmHg that is associated with new organ dysfunction. All patients' duration of mechanical ventilator, intensive care unit stay and prognosis were determined.
