Elzem bir eser element olan ve birçok enzimatik aktivasyonun önemli bir parçası olarak görev yapan selenyum (Se) 1957 yılından itibaren önem kazanmıştır. Metabolizması çeşitli gereksinimlere göre organize edilmektedir. Yapısal komponent olarak Se, protein yapılarına katılmaktadır. Düzeyi, serum selenyum ve serum glutatyon peroksidaz aktivitesinden etkilenmektedir. Hayvansal dokularda “selenometionin” ve “selenosistein” olarak iki formda bulunmaktadır. Genel olarak proteinden zengin besinler iyi selenyum kaynağıdır. Plazmada albumine bağlı olarak taşınmaktadır. Yetersizlik olduğunda selenyumun emilimi artmaktadır. Vücutta toplam olarak 13-20 mg selenyum bulunmaktadır. Eritrositlerdeki selenyum miktarı, uzun dönem selenyum alımının bir göstergesidir; malnütrisyonda ve kanserde serum selenyum düzeylerinin düştüğü bilinmektedir. Organizmada oksidatif yaralanmalara karşı koruyucu olan glutatyon peroksidazın yapısında yer almaktadır. Selenyumun kanser ile ilişkisi üzerine birçok çalışma yapılmaktadır. Birçok kanser türü ile selenyum alım düzeyleri ve serum selenyum seviyeleri ilişkilendirilmiştir. Yapılan çalışmalar sonucunda bazı kanser çeşitlerinde selenyumun serum seviyesi düşük bulunmuştur. Selenyumun antioksidan enzim sistemindeki fonksiyonu ile dejeneratif durumların çoğu arasında bağlantı kurulmuştur. İmmün fonksiyonları arttırması, apoptozisi indüklemesi, hücre proliferasyonunu inhibe etmesi, karsinojen metabolizmasını değiştirmesi gibi birçok potansiyel mekanizmaya sahip olduğu bildirilmiştir. Epidemiyolojik çalışmalar diyetle selenyum alımı ve kolorektal kanser riski arasında ters ilişki olduğunu göstermektedir. Farklı selenyum bileşiklerinin prostat kanser hücrelerinde antiproliferatif yanıtı, apoptoz, hücre içi redoks durumunu değiştirme ve NF-KB sinyal yolunun blokajı ile sağladığını göstermektedir. Hayvan deneyleri, epidemiyolojik çalışmalar ve müdahale çalışmaları; selenyum bileşiklerinin, hem spesifik kanserlerin önlenmesinde hem de son faz kanserlerde antimutajenik etki gösterebileceğini bildirmektedir. Selenyum antikarsinojenik olarak görev alabilmekte ve immün fonksiyonları güçlendirmektedir. Bazı çalışmalar, vücuttaki selenyum düzeyi ile kanser riski arasında negatif korelasyon olduğunu bildirsede, çelişkili sonuçlar literatürde yer almaktadır. Selenyumun kanser ve kanser tedavisindeki etkinliğini belirleyebilmek için daha fazla klinik çalışmaya gereksinim duyulmaktadır. Bu derlemede vücut için elzem bir element olan selenyumun metabolizması ve işlevlerinden bahsedilerek, kanser ile ilişkisini inceleyen çalışmalar üzerinde durulacaktır.
Amaç: Deneysel ve karşılaştırmalı olarak yürütülen bu araştırma, bir anaokulunda sunulan yemekleri, çocukların tüketimine uygun hâle getirerek yemek israfını azaltmak amacıyla gerçekleştirilmiştir. Bireyler ve Yöntem: Kapadokya Üniversitesi Çocuk Gelişimi Uygulama ve Araştırma Merkezine bağlı kreşte öğle yemeği hizmeti alan çocukların tamamı çalışmaya dâhil edilmiştir. Çalışma, gözlem periyodu ve yemek örneği iyileştirme aşaması olmak üzere iki aşamalı planlanmıştır. Birinci aşamada artık miktarı en yüksek olan yemek örnekleri ve bu yemeklerin “Artık Miktar Yüzdesi” belirlenmiştir. Çalışmanın ikinci aşamasında artık miktarı yüzdesi yüksek olan yemeklerde grubun özellik ve tercihlerine göre iyileştirmeler yapılmış ve çocuklara “İyileştirilmiş Yemek Örnekleri” sunulmuştur. Bulgular: Çalışma 15 kız, 35 erkek öğrenci olmak üzere toplam 50 çocuk ile gerçekleştrilmiştir. Çocukların yaş ortalaması 50.09±11.75 ay, boy uzunluğu ortalaması 102.09 ± 7.32 cm, vücut ağırlığı ortancası ise 16.60 (14.67-18.42) kilogramdır. Çocukların yaşa göre beden kütle indeksi Z skoru 0.46, persentil değeri 67.7’dir. Çalışma kapsamına alınan sekiz kap yemek örneğinin dördünde (patates oturtma - ıspanak yemeği - barbunya yemeği (sıcak) - çiftlik kebabı) artık miktar yüzdesi istatistiksel açıdan anlamlı olarak azalmıştır (p<0.05). Sonuç: Çalışmanın sonucunda, ilgili grubun özellik ve tercihlerine göre düzenlenen yemek örneklerinin yemek israfını önlemeye yardımcı olduğu gözlenmiştir. Bu sonuç, yemek israfının azaltılması ve katılımcı tercihlerine yönelik besleyici menüler planlanmasında alanında eğitim almış uzman kişilere yetki verilmesinin önemini göstermiştir.
Diabetes Mellitus, is an important public health problem that causes increased morbidity and mortality. Hyperglycemia, specific sign of diabetes; tried to be controlled clinically exogenous insulin administration or various drugs however, despite developments treatment and follow-up, development of vascular complications hasn’t been completely prevented in many diabetic patients. Several epidemiological, comprehensive studies have shown that early-intensive control of hyperglycemia reduces risk of diabetes-related complications. These studies emphasize need for early glycemic control. Early control of hyperglycemia has recently been defined as "metabolic memory". Different mechanisms, such as overproduction of free oxygen radicals in mitochondria and endothelial cells, mitochondrial deoxyribonucleic acid (DNA) damage, protein kinase C activation, polyol-hexosamine pathway activation, increased production of advanced glycation end products (AGEs) and AGE receptor overexpression play an important role in metabolic memory pathogenesis. These mechanisms induce gene expression permanently and causing epigenetic changes. Metabolic memory occurs through epigenetic changes such as histone modification, DNA methylation and micro-ribonucleic acid (RNA)-related mechanisms. From a clinical point of view, metabolic memory theory emphasizes necessity of an early-intensive treatment regimen to achieve metabolic control as soon as possible. In addition to early intensive hyperglycemia control, therapeutic agents or epigenetic therapy can reduce reactive oxygen species and glycosylation also be used in order to minimize long-term diabetic complications. In this review, metabolic memory theory, definition and pathogenesis of metabolic memory, epigenetic mechanisms and therapeutic approaches are evaluated.
