Vivimos una época de incertidumbres, marcada por grandes amenazas, pero también, y sobre todo, por enormes oportunidades. Cada crisis encierra la oportunidad de lo nuevo. Las tecnologías digitales modelan los distintos ámbitos de las relaciones sociales. La educación superior a nivel global enfrenta múltiples desafíos, tanto tecnológicos, pedagógicos, financieros, como organizacionales y sociales. La irrupción de la pandemia de la COVID-19 motivó el cese de las actividades presenciales del campus universitario en casi todos los países, obligando a migrar de forma abrupta, hacia soluciones digitales para mantener la continuidad del aprendizaje de sus estudiantes. El confinamiento social obligó a las Instituciones de Educación Superior a la innovación educativa, a generar nuevos espacios de aprendizaje y nuevas modalidades de educación. Por otro lado, la experiencia de la transición hacia la educación virtual ha puesto en evidencia las grandes desigualdades sociales y tecnológicas existentes entre países, instituciones educativas y estudiantes. Sin embargo, el COVID-19 puede ser acelerador de las grandes transformaciones que la educación superior, en el marco de la sociedad del conocimiento, demanda con urgencia. Es claro que la respuesta a todos estos desafíos educativos no radica en acciones individuales de personas o instituciones educativas aisladas, sino que se requiere colaborar con otras universidades públicas y privadas, así como los gobiernos de muchos países, para poder generar ideas y formular estrategias que permitan avanzar en la ruta de la mejoría de la calidad educativa. En este libro se aborda, desde la perspectiva del docente, la discusión de los tópicos más relevantes relacionados con la problemática de los cambios en la educación superior en el marco de la pandemia de la COVID-19: Desafíos, aprendizajes de calidad, virtualidad, desigualdades, escenarios futuros de la educación superior.
La siguiente edición de consultas medicas se hace una revisión sistemática de las patologías más comunes en el primer nivel de atención y su manejo clínico. Se incluyen también algunos casos clínicos relevantes para el estudio del medico en formación y personal de salud en general.
Como hacen ver los informes de la Organización Mundial de la Salud (OMS), la mitad de la población mundial, es decir, unos 3.600 millones de personas no tienen acceso a una cobertura plena de servicios de salud esencial. Por ello, ampliar las intervenciones en atención primaria en salud (APS) en los países más pobres e incluso en los que tienen ingresos medianos, podría salvar unos 60 millones de vida y aumentar la esperanza de vida en 3,7 años para 2030.
La infección de las vías urinarias es uno de los principales problemas que se presentan durante el periodo de gestación estas pueden ser causadas por diferentes microorganismos dentro de los cuales se destaca la Escherichia Coli , el diagnóstico y tratamiento es sumamente importante para evitar complicaciones que se pueden presentar tales como parto pretérmino, ruptura prematura de membranas, aborto y además de infecciones que se transmiten de madre a hijo(a) de manera directa a través del trabajo de parto, por estas razón es importante conocer acerca de esta problemática que tiene lugar en el Hospital del Niño y la Mujer de la Ciudad de Cuenca ya que hay un alta incidencia de esta patología a nivel de consulta externa , es también relevante que las madres conozcan acerca de medidas higiénicas que puedan dar una mejor visión global de lo que abarca toda esta problemática, el tratamiento debe ser oportuno para evitar cualquier complicación además del conocimiento de los medicamentos que se pueden utilizar durante el embarazo, ya que lo que se evita en lo mayoría de los casos es causarle daño al producto de la madre gestante, además de los exámenes que son los más utilizados y que tienen mayor eficacia al momento de realizar el diagnóstico y en base a estos dar el tratamiento necesario. Objetivo: Determinar las complicaciones y la frecuencia con las que se presentan las infecciones de vía urinarias en mujeres embarazadas. Materiales y Métodos: Es un estudio descriptivo, cuantitativo de corte transversal, cuantitativo de método probabilístico el mismo que se efectuó desde febrero a Julio de 2015, para la obtención de los datos se aplicó un formulario de recolección de datos previo consentimiento informado en los que constaron las variables sociodemográficas y sus modalidades de estudio, los resultados se presentan en tablas simples y de doble entrada, para lo cual se utilizó el programa estadístico SPSS 15.00 versión evaluación. Resultados: La presente investigación se realizó en el Área de Ginecología del Hospital Municipal del Niño y la Mujer de la Ciudad de Cuenca, este estudio estaba dirigido a mujeres embarazadas que acudieron a consulta en los meses de febrero a julio de 2015, relacionadas a los factores de riesgo asociados a la cistitis en mujeres embarazadas, previa a la recolección de los datos se les informo que los datos que se recopile se utilizará exclusivamente para fines científicos sin comprometer la intimidad de las personas, procedieron a la firma del consentimiento y procedieron a contestar el cuestionario de forma individual. El 53,3% de pacientes manifiestan haber tenido problemas durante el embarazo, de eso el 37,5% han sufrido de amenazas de aborto, que al ser tratadas a tiempo se ha podido evitar la pérdida de los niños. Conclusión: En los meses de febrero a julio que es el tiempo que duro la investigación se identificaron 63 pacientes con infección de vías urinarias, representa el 52,5% de las personas investigadas lo que determina que este es un problema de salud pública que se presenta durante el embarazo y que si no es tratado adecuadamente puede tener repercusiones en el futuro.
Antecedentes: Desde 1980, la anestesia epidural se ha propuesto para controlar el dolor post- operatorio, consiste en administrar un anestésico local a nivel del espacio peridural con el fin de bloquear la transmisión del impulso doloroso. Objetivo: Describir las ventajas acerca del uso de anestesia epidural en cirugía abdominal. Fuente de datos: Se trata de una revisión bibliográfica durante los meses de julio – agosto 2020, en las bases de datos Google Scholar, PubMed y Elsevier obteniendo 352 trabajos, que tras la eliminación de resultados duplicados y análisis de calidad de evidencia generaron 30 estudios que se incluyeron como fuente de datos. Resultados: La anestesia epidural puede ser indicada en cirugía vascular abdominal, cirugía renal, cirugía ginecológica, y cirugía emergente en el contexto de COVID-19, además de todos aquellos pacientes con disminución previa de la función pulmonar que tengan contraindicaciones para anestesia general. El uso combinado con la anestesia general se asoció con importantes beneficios de la ventilación mecánica y una disminución de la estancia hospitalaria en pacientes críticos. Sin disminuir la inmunosupresión y pudiendo continuarse para aliviar el dolor posoperatorio. Limitaciones: La evidencia estudiada pertenece a poblaciones no latinas. El tipo de investigaciones incluidos para la revisión bibliográfica no fue homogéneo en su modalidad ni en el procedimiento quirúrgico en el cual se indicó. Conclusiones: La anestesia epidural por si sola presenta varias ventajas en múltiples procedimientos quirúrgicos abdominales, especialmente en aquellos realizados en pacientes con deterioro previo de la función pulmonar o alto riesgo de complicaciones, posee altos niveles de seguridad y viabilidad de utilización, sin embargo, su uso deberá estar basado en un protocolo que permita seleccionar los pacientes que se beneficiaran de ella.
