Objetivo: Analisar as principais complicações relacionadas à infecção urinária durante o período gestacional. Métodos: Trata-se de uma revisão integrativa da literatura, de caráter qualitativo e nível exploratório. Foram utilizados os seguintes descritores e operadores booleanos: Urinary tract infections AND Pregnancy, nas bases de dados eletrônicas PubMed, Biblioteca Virtual em Saúde e Scielo. Resultados: Demonstrou-se que os quadros de Infecção do Trato Urinário (ITU) sobretudo nas pacientes com dificuldade de instalação terapêutica por conta de resistência aos antibióticos, podem predispor diversas complicações materno-fetais, como parto prematuro, baixo peso ao nascer, deficiência em desenvolvimento motor e microcefalia. Considerações finais: O período gestacional é um momento de muitas alterações fisiológicas e anatômicas no organismo feminino, com mudanças sobretudo no trato urogenital, que podem predispor toda a via urinária a processos de infecção, sobretudo por Escherichia coli uropatogênica, por conta do processo de dilatação ureteral e atuação da progesterona na musculatura lisa, e a sua precaução e intervenção imediata com tratamento eficaz, pode ajudar a atenuar as possíveis complicações advindas com a ITU prolongada e não tratada.
