The new classification announced by the World Health Organization in 2016 recognized five molecular subtypes of diffuse gliomas based on isocitrate dehydrogenase (IDH) and 1p/19q genotypes in addition to histologic phenotypes. We aim to determine whether clinical MRI can stratify these molecular subtypes to benefit the diagnosis and monitoring of gliomas. The data from 456 subjects with gliomas were obtained from The Cancer Imaging Archive. Overall, 214 subjects, including 106 cases of glioblastomas and 108 cases of lower grade gliomas with preoperative MRI, survival data, histology, IDH, and 1p/19q status were included. We proposed a three-level machine-learning model based on multimodal MR radiomics to classify glioma subtypes. An independent dataset with 70 glioma subjects was further collected to verify the model performance. The IDH and 1p/19q status of gliomas can be classified by radiomics and machine-learning approaches, with areas under ROC curves between 0.922 and 0.975 and accuracies between 87.7% and 96.1% estimated on the training dataset. The test on the validation dataset showed a comparable model performance with that on the training dataset, suggesting the efficacy of the trained classifiers. The classification of 5 molecular subtypes solely based on the MR phenotypes achieved an 81.8% accuracy, and a higher accuracy of 89.2% could be achieved if the histology diagnosis is available. The MR radiomics-based method provides a reliable alternative to determine the histology and molecular subtypes of gliomas. .

Background: Radiomics-based non-invasive biomarkers are promising to facilitate the translation of therapeutically related molecular subtypes for treatment allocation of patients with head and neck squamous cell carcinoma (HNSCC). Methods: We included 113 HNSCC patients from The Cancer Genome Atlas (TCGA-HNSCC) project. Molecular phenotypes analyzed were RNA-defined HPV status, five DNA methylation subtypes, four gene expression subtypes and five somatic gene mutations. A total of 540 quantitative image features were extracted from pretreatment CT scans. Features were selected and used in a regularized logistic regression model to build binary classifiers for each molecular subtype. Models were evaluated using the average area under the Receiver Operator Characteristic curve (AUC) of a stratified 10-fold cross-validation procedure repeated 10 times. Next, an HPV model was trained with the TCGA-HNSCC, and tested on a Stanford cohort (N = 53). Findings: Our results show that quantitative image features are capable of distinguishing several molecular phenotypes. We obtained significant predictive performance for RNA-defined HPV+ (AUC = 0.73), DNA methylation subtypes MethylMix HPV+ (AUC = 0.79), non-CIMP-atypical (AUC = 0.77) and Stem-like-Smoking (AUC = 0.71), and mutation of NSD1 (AUC = 0.73). We externally validated the HPV prediction model (AUC = 0.76) on the Stanford cohort. When compared to clinical models, radiomic models were superior to subtypes such as NOTCH1 mutation and DNA methylation subtype non-CIMP-atypical while were inferior for DNA methylation subtype CIMP-atypical and NSD1 mutation. Interpretation: Our study demonstrates that radiomics can potentially serve as a non-invasive tool to identify treatment-relevant subtypes of HNSCC, opening up the possibility for patient stratification, treatment allocation and inclusion in clinical trials.

Fully convolutional neural networks like U-Net have been the state-of-the-art methods in medical image segmentation. Practically, a network is highly specialized and trained separately for each segmentation task. Instead of a collection of multiple models, it is highly desirable to learn a universal data representation for different tasks, ideally a single model with the addition of a minimal number of parameters steered to each task. Inspired by the recent success of multi-domain learning in image classification, for the first time we explore a promising universal architecture that handles multiple medical segmentation tasks and is extendable for new tasks, regardless of different organs and imaging modalities. Our 3D Universal U-Net (3D U 2 -Net) is built upon separable convolution, assuming that images from different domains have domain-specific spatial correlations which can be probed with channelwise convolution while also share cross-channel correlations which can be modeled with pointwise convolution. We evaluate the 3D U 2 -Net on five organ segmentation datasets. Experimental results show that this universal network is capable of competing with traditional models in terms of segmentation accuracy, while requiring only about 1% of the parameters. Additionally, we observe that the architecture can be easily and effectively adapted to a new domain without sacrificing performance in the domains used to learn the shared parameterization of the universal network. Automatic detection of atrial fibrillation (AF) for AF diagnosis, especially for AF monitoring, is necessarily desirable for clinical therapy. In this study, we proposed a novel method for detection of the transition between AF and sinus rhythm based on RR intervals. First, we obtained the delta RR interval distribution difference curve from the density histogram of delta RR intervals, and then detected its peaks, which represented the AF events. Once an AF event was detected, four successive steps were used to classify its type, and thus, determine the boundary of AF: 1) histogram analysis; 2) standard deviation analysis; 3) numbering aberrant rhythms recognition; and 4) Kolmogorov-Smirnov (K-S) test. A dataset of 24-h Holter ECG recordings (n = 433) and two MIT-BIH databases (MIT-BIH AF database and MIT-BIH normal sinus rhythm (NSR) database) were used for development and evaluation. Using the receiver operating characteristic curves for determining the threshold of the K-S test, we have achieved the highest performance of sensitivity and specificity (SP) (96.1% and 98.1%, respectively) for the MIT-BIH AF database, compared with other previously published algorithms. The SP was 97.9% for the MIT-BIH NSR database.
