<p>En Oaxaca, la agricultura es la principal actividad en el sector primario y en las comunidades rurales. Lamentablemente, la producción agrícola está expuesta al uso indiscriminado de insumos agrícolas, en particular, a plaguicidas. Dichos insumos, representan un potencial de riesgo para la salud pública y el ambiente. La presente investigación tuvo como objetivo documentar el manejo, conductas de uso e intoxicaciones agudas por plaguicidas utilizados en la agricultura de la comunidad de San Baltazar Chichicápam, Oaxaca, México. Mediante encuestas a 50 productores, se obtuvo información de los tipos de cultivo y plaguicidas utilizados, las conductas de uso, así como los signos y síntomas de intoxicación aguda por plaguicidas (IAP). Los cultivos más relevantes fueron el maíz y frijol (78%), alfalfa (68%), garbanzo (28%), tomate (20%) y flores (20%). Se identificaron 37 Ingredientes Activos (IA) de plaguicidas, 17 son utilizados en maíz, 15 en frijol, 15 en alfalfa, 23 en tomate y 12 en flores. El insecticida organofosforado metamidofos de Categoría Toxicológica II (CT II), se aplica principalmente en maíz, frijol y alfalfa. En el resto de los cultivos, se utilizan insecticidas, herbicidas y fungicidas de diferentes CT. La mayoría de los campesinos no utiliza medidas de seguridad al utilizar los plaguicidas. El 66% de la población presenta por lo menos un síntoma de IAP. El principal plaguicida asociado a dichos síntomas fue el metamidofos. La salud de la población en estudio, está en riesgo debido a la exposición a la diversidad de plaguicidas identificados.</p>
SUMMMARYLeukoreduction is the process of removing white blood cells contained in blood products of a Blood Bank to rule out adverse reactions in the receptor. The objective of this study was to test the leukoreduction effectiveness of the "Top and Bottom System" centrifugation method in canine and horse blood units. Blood was obtained in a transfusion system, Optipac® cuadruplex CPD/Adsol, Baxter®, from healthy animals, 10 dogs and 10 horses. 450 ml of blood was extracted from each donor and collected in the total blood unit (ST) of the system and centrifuged. After centrifugation, the ST unit was processed in the Optipress ® , Baxter ® to obtain the leukoreduced red blood cells concentrate (CE) into a separated plastic unit. Blood samples of 1 ml were collected from each animal during the blood extraction, a second from the ST unit and a third from the CE unit. Samples were analyzed to determine red cell count, haemoglobin concentration, packed cell volume, mean corpuscular volume, mean corpuscular haemoglobin concentration, and leucocyte count (WBC) using an haematological analyzer, Sysmex KX-21N. Descriptive statistics and "t" Student matching group test were used to analize data. The CE obtained at the end of the process had a Packed Cell Volume (PCV) = 57% in both species and the erythrocyte characteristics remained unchanged after the leukoreduction process. The CE of the canine blood was efficiently leukoreduced (mean = 95.5%) with a remaining WBC content of 0 to 3100 µL (mean 790 µL). The CE of the horse blood was also efficiently leukoreduced (mean = 93.4%) remaining a WBC content between 200 to 800 µL (mean 450 µL). These results show that the Top and Bottom System is efficient in obtaining leukoreduced erythrocyte units to be used for transfusion in dogs and horses.Palabras clave: leucorreducción, transfusión, sangre, caninos, equinos.
La enfermedad injerto contra hospedero asociada a la transfusión (EICH-AT) es una reacción postransfusional grave debido al injerto de linfocitos residuales (LR) presentes en los componentes sanguíneos. El objetivo de este trabajo fue evaluar la eficacia del irradiador terapéutico THERATRON® Phoenix™ (cobalto 60) del Centro de Oncología y Radioterapia de Oaxaca, México, para eliminar la capacidad de proliferación de los LR en concentrados eritrocitarios (CR). Para este fin, se obtuvieron siete concentrados eritrocitarios de donadores de sangre seleccionados según los criterios de las normas oficiales nacionales, los cuales fueron irradiados a 2520 cGy; y se cuantificaron parámetros para evaluar la conservación del potencial terapéutico de los concentrados eritrocitarios (hemoglobina, hematocrito, hemólisis e hiperkalemia) y de efectividad para inactivar a los LR (número, viabilidad y capacidad proliferativa). El irradiador preservó la calidad e incrementó la seguridad transfusional de estos componentes sanguíneos al eliminar la capacidad proliferativa de los LR, y el análisis de costo-efectividad demostró su eficiencia para este proceso. Tomando como base estos resultados, se proponen estos parámetros como control de calidad para la validación de irradiadores terapéuticos y/o de rayos X para la irradiación de concentrados eritrocitarios, así como la difusión de este servicio en todo el país, con la finalidad de reducir la incidencia de la enfermedad injerto contra hospedero asociada a la transfusión.
