La epilepsia refractaria tanto generalizada como focal, es una patología sumamente incapacitante, para el tratamiento de la misma se ha establecido a la callosotomía desde hace décadas como primera línea quirúrgica para su control, la cual puede presentar efectos secundarios importantes como síndrome de desconexión y pérdida de memoria, sin embargo, existen pacientes que no responden a la callosotomía y necesitan nuevas líneas de tratamiento, buscando en la estimulación de nervio vago una respuesta a su condición. Descripción del caso de estudio. Se presenta el caso de paciente masculino de 24 años de edad con antecedente patológico de convulsiones tipo tónico clónicas generalizadas confirmadas por video electroencefalograma de 24 horas, de predominio nocturno de 13 años de evolución, es sometido a 2 regímenes farmacológicos antiepilépticos diferentes en un período de 7 años de duración, posteriormente diagnosticado con epilepsia refractaria, por lo que se realiza callosotomía sin control de su cuadro clínico, el mismo año se realiza estimulación de nervio vago, presentando resultados favorables en su evolución. Conclusión. Luego de evidenciar el presente caso de estudio se concluye que el tratamiento de epilepsia refractaria con la colocación de un estimulador de nervio vago izquierdo asociado a un correcto régimen FAE es una alternativa muy eficaz para considerar en estos pacientes.
Objetivo: Descrever o impacto da infecção por COVID-19 em pacientes com doenças reumáticas autoimunes. Metodologia: Foi realizado um estudo descritivo não experimental do tipo revisão de literatura, utilizando artigos de pesquisa indexados publicados nos últimos 3 anos, em espanhol e inglês, em bancos de dados como o PubMed e Web of Science. Para a consulta nos diferentes mecanismos de busca, foram utilizadas palavras-chave como agente reumatológico, doenças autoimunes, COVID-19, doença reumática. Resultados: A infecção por COVID-19 em pacientes com doenças reumáticas autoimunes não é diferente da população em geral, e foi até mesmo demonstrado que existe um menor risco de infecção e doença infecciosa grave devido ao uso de drogas que reduzem a resposta sistêmica. Da mesma forma, a presença de comorbidades, o uso de corticosteroides ou rituximabe influenciam o curso clínico. Conclusões: O impacto da infecção COVID-19 em pessoas diagnosticadas com doença reumática autoimune são tão prováveis de serem infectadas quanto a população em geral. Foi até mesmo demonstrado que existe um risco reduzido de infecção e doenças infecciosas graves devido ao uso de drogas que reduzem a resposta sistêmica. Os principais sintomas encontrados foram febre, tosse, calafrios e dispneia.
Os ensaios pré-clínicos e clínicos com vírus oncolíticos para o tratamento de tumores sólidos difíceis de gerir, tais como tumores cerebrais, provaram ser melhores do que os tratamentos convencionais. Especialmente para o glioblastoma (GB), um tumor cerebral com prognóstico e tratamento sombrios, são investigadas diferentes variantes do herpesvirus simplex tipo I e utilizadas para melhorar a sobrevivência a longo prazo. Objectivo: descrever a imunoviroterapia oncolítica com vírus do herpesvírus simplex tipo 1 como um agente imunitário e perturbador lítico em tumores cerebrais malignos. Método: a investigação é uma revisão abrangente dos artigos publicados entre 2013 e 2022, utilizando a estratégia PICO que integrou a questão da investigação. A pesquisa de informação foi realizada a partir de fontes de dados Pubmed e ScienceDirect, utilizando descritores e os operadores Booleanos AND e OR. Resultados: O vírus Herpes simplex tipo I confere particularidades cognatas para a matança de células cancerosas e cria imunidade inata e adaptativa contra células tumorais em cancros cerebrais em crianças e adultos.
Objetivo: descrever as características da conjuntivite como manifestação oftalmológica do SARS-CoV-2 em indivíduos com COVID-19. Método: foi realizada uma revisão integrativa de artigos publicados entre 2020 e 2022, utilizando a estratégia PICO para integrar a questão de pesquisa. As buscas eletrônicas foram realizadas nas bases de dados Pubmed (https://pubmed.ncbi.nlm.nih.gov/) e ScienceDirect (https://www.sciencedirect.com/), em espanhol ou inglês, usando descritores e operadores booleanos AND e OR. Foi realizada uma busca eletrônica para descrever as características da conjuntivite como manifestação oftalmológica do SARS-CoV-2 em indivíduos com COVID-19. Resultados: Foi identificado um registro total de 96 artigos, incluindo 14 estudos de pacientes que manifestaram conjuntivite por SARS-CoV-2 como manifestação de COVID-19. A conjuntivite como manifestação oftalmológica do SARS-CoV-2 em indivíduos com COVID-19 apresenta prevalência de 0,8% a 31,6%. As características mais comuns são; hiperemia conjuntival, epífora, sensação de corpo estranho e fotofobia. Conclusão: A conjuntivite pode ser uma parte inicial do quadro clínico da COVID-19, ou pode até se manifestar como o único sintoma que aparece dias antes, a incidência e as características da conjuntivite como manifestação oftalmológica da COVID-19 são variáveis.
Introduction: Vitamin D is a hormone synthesized in the skin, by the presence of the "precursor molecule 7-dehicrocholesterol" whose action depends directly on the stimulus of ultraviolet rays for the production of cholecalciferol. It has several functions including a fundamental role in mediating the response of the immune system, plays modulatory actions, counteracts inflammatory, oxidative and fibrotic activity, and consequently regulates autoimmune tissue lesions. As it possesses an immunomodulatory property, is related to the appearance and progression of autoimmune disorders. Objective: To describe the role of vitamin D in autoimmune rheumatologic diseases. Methods: A narrative review was performed, collecting information from January 2019 to January 2023 in scientific databases such as: Pubmed, Scopus, Web of Science, and Google Scholar, using keywords. Results: Vitamin D in autoimmune rheumatic diseases influences the control and decrease of inflammatory activity, slows its speed of progression and improves clinical manifestations, although there is no total consensus on the mode of use, however, its supplementation has a great benefit. Conclusions: The role of Vitamin D in autoimmune rheumatic diseases varies according to the underlying pathology, in combination with bisphosphonates it can improve bone mineral density in spondyloarthropathies, prevent osteoporosis, improve the clinical picture of SLE and Systemic Sclerosis, but it can increase antibody levels in Connective Tissue Diseases.
