Hệ thống gen kháng nguyên bạch cầu người (Human leukocyte antigen - HLA) mã hóa các protein thụ thể bề mặt tế bào, chịu trách nhiệm điều hòa hoạt động hệ miễn dịch. Đa hình đơn nucleotide (SNP) rs3077 của gen HLA-DP được cho là có vai trò trong nhiễm virus viêm gan B (HBV) và bảo vệ chống lại giai đoạn muộn của quá trình xơ gan. Nghiên cứu này nhằm mục đích xác định sự phân bố của SNP rs3077 trên bệnh nhân xơ gan nhiễm HBV và mối liên quan giữa SNP này với nguy cơ xơ gan. Kỹ thuật Realtime-PCR được sử dụng để xác định SNP rs3077 trên bệnh nhân được chẩn đoán xơ gan nhiễm HBV và viêm gan B mạn tính. Tỉ lệ kiểu gen AA, AG, GG của SNP rs3077 ở nhóm xơ gan và viêm gan B lần lượt là 5,1%, 39,7%, 55,1% và 11,2%, 38,8%, 50,0%. Tần số các alen và kiểu gen rs3077 của hai nhóm không khác biệt có ý nghĩa thống kê. Như vậy, chưa tìm thấy mối liên quan giữa SNP rs3077 với nguy cơ xơ gan trên nền viêm gan B mạn tính.
HLA - DQ là protein thụ thể bề mặt tế bào ở các tế bào trình diện kháng nguyên. Các đột biến hay đa hình gen HLA - DQ có thể tác động tới hệ miễn dịch và ảnh hưởng tới diễn biến của bệnh viêm gan B. Nghiên cứu này được thực hiện nhằm xác định đa hình đơn nucleotide (SNP) rs2856718 gen HLA - DQ và mối liên quan với tiến triển xơ gan ở bệnh nhân viêm gan B mạn tính. 108 mẫu máu của bệnh nhân xơ gan và 112 mẫu máu của bệnh nhân viêm gan B mạn tính đã được thu thập và xác định kiểu gen SNP rs2856718 bằng phương pháp TaqMan Realtime - PCR. Tỉ lệ các kiểu gen GG, GA và AA lần lượt ở nhóm xơ gan là 44,4%, 40,7% và 14,8% và nhóm viêm gan B mạn tính là 32,1%, 46,4% và 21,4%, khác biệt không có ý nghĩa thống kê. Tần số alen G ở nhóm xơ gan cao hơn nhóm viêm gan B mạn có ý nghĩa thống kê (p = 0,043). Đa hình rs2856718 gen HLA - DQ có thể liên quan với tiến triển xơ hóa gan trên bệnh nhân nhiễm HBV mạn tính.
Mô tả một số đặc điểm lâm sàng, cận lâm sàng của thai phụ nhiễm HBV và xét nghiệm máu cuống rốn tại Bệnh viện Trung ương Thái Nguyên. Tiêu chuẩn lựa chọn thai phụ có 01 thai sống, đủ tháng đến đẻ có HBsAg (+). Thiết kế mô tả cắt ngang, sử dụng SPSS 20.0 và R, phân tích tương quan Pearson và Anova Fisher. Kết quả cho thấy tuổi trung bình 27,6 ± 4,2, triệu chứng lâm sàng không đặc hiệu cho viêm gan bao gồm phù, chán ăn, mệt mỏi; tỉ lệ mổ lấy thai 66,7%; xét nghiệm máu mẹ HBeAg(+) chiếm 45%, HBV DNA ≥ 5E+07 bản sao/ml chiếm 33,3%; máu cuống rốn tỉ lệ HBsAg(+) chiếm 53,3%, HBeAg(+) chiếm 38,2%; PBMCs của máu mẹ và máu cuống rốn tương quan thuận ở nhóm thai phụ có tải lượng vi rút thấp hơn 5E+07 bản sao HBV DNA/ml.Kết luận các triệu chứng lâm sàng của vi rút viêm gan B ở thai phụ không đặc hiệu cho chẩn đoán viên gan B; ở nhóm thai phụ có tải lượng vi rút thấp hơn 5E+07 bản sao HBV DNA/ml thấy có mối tương quan thuận có ý nghĩa giữa PBMCs của mẹ và máu cuống rốn.
Ung thư gan nguyên phát là bệnh ung thư phổ biến và có tỉ lệ tử vong cao. Gen HLA-DQ mã hóa các chuỗi polypeptid của phân tử MHC lớp II tham gia hoạt hóa tế bào trình diện kháng nguyên trong đáp ứng miễn dịch. Đa hình đơn nucleotide (SNP) rs2856718 của gen HLA-DQ có thể liên quan tới nhiễm virus viêm gan B mãn tính và nguy cơ ung thư biểu mô tế bào gan. Nghiên cứu này được thực hiện nhằm mục tiêu xác định sự phân bố của SNP rs2856718, đồng thời đánh giá mối liên quan giữa SNP này với nguy cơ mắc ung thư biểu mô tế bào gan. Kỹ thuật RT-PCR được sử dụng để xác định kiểu gen SNP rs2856718 trên 93 bệnh nhân ung thư biểu mô tế bào gan và 93 người khỏe mạnh. Kết quả đã xác định được tỷ lệ kiểu gen GG, AG, AA ở nhóm bệnh là 39,8%, 46,2%, 14% và nhóm chứng là 30,1%, 53,8%, 16,1%. Chưa tìm thấy mối liên quan giữa SNP rs2856718 với nguy cơ ung thư biểu mô tế bào gan cũng như với một số yếu tố nguy cơ khác.
