Palomino (6) RESUMEN Objetivo: el presente trabajo pretende evaluar el efecto de los aparatos ortodóncicos fi jos linguales en la articulación de los fones, en pacientes que hablen español chileno, y la adaptación a estos efectos dentro del primer mes de uso de los aparatos. Métodos: la muestra consistió en 13 pacientes que acudieron para instalación de aparato ortodóncico fi jo lingual. Se realizó un examen fonoarticulatorio en condiciones estandarizadas, en 5 momentos diferentes: previo a la instalación de los aparatos fi jos (E0), inmediatamente después de realizada esta (E1), a las 24 horas posteriores (E2), a los 7 días (E3) y un mes después de la instalación (E4). Se determinaron los fones afectados en los distintos momentos de examen respecto a la línea base dada por el examen previo, y se analizó su resolución. Resultados: se observaron cambios signifi cativos en el punto de articulación de los fones [d], [s] y [r]Los fones [d] y [s] mostraron una resolución favorable dentro del primer mes de uso de los aparatos. El fone vibrante múltiple [r] no mostró una recuperación de la alteración una vez cumplido un mes de uso de los aparatos ortodóncicos. Conclusiones: la instalación de aparato ortodóncico fi jo lingual produce modifi caciones en el punto de articulación de los fones consonánticos, las cuales tienden a mejorar dentro del primer mes de uso de los aparatos, con excepción del fone vibrante múltiple [r].
Objectives To present early teeth extractions as a treatment option in severe dental crowding in patients with generalized recessive dystrophic epidermolysis bullosa (RDEB). Materials and methods Three patients with generalized RDEB were treated with early teeth extractions to prevent severe dental crowding. Results Two patients had bilateral upper first premolars extraction, and the third patient had permanent maxillary canine extraction. Crowding was avoided, and no further orthodontic treatment was necessary. Conclusion Considering the challenges of severe mucosal fragility and microstomia in patients with generalized RDEB, early teeth extractions are a reasonable option as an orthodontic management. This approach reduces the severity of dental crowding as the child gets older and reduces the need for orthodontic appliances. Individual factors such as access to dental care, general health, and oral health have an important impact on the decision‐making process. Orthodontic treatment planning should include a multidisciplinary team.
The difficulty experienced by patients with fixed orthodontic appliances in maintaining adequate oral hygiene poses a risk for dental caries. The use of probiotics has been proposed as a means of prevention. The following systematic review aims to determine the effectiveness of probiotics in the prevention of dental caries during treatment with fixed orthodontic appliances. This review was carried out according to the PRISMA guidelines. A comprehensive electronic search was conducted in the Cochrane Library, EMBASE, PubMed and Google Scholar Beta databases. The inclusion criteria included randomized controlled clinical trials involving the use of probiotics, caries and patients under fixed orthodontic treatment. The methodological quality of the articles was evaluated according to risk of bias. Of the five included studies, three reported significant differences compared to the control group. Of the others, one article reported significant decrease in the final count of microorganisms compared to the beginning of treatment. Only one study reported no differences of any kind. It was estimated that the eligible studies were of moderate methodological quality and had an unclear risk of bias, without affecting key domains for the research. It is concluded that the daily consumption of probiotics can be effective in the prevention of carious lesions in patients under fixed orthodontic treatment. However, this should be interpreted with caution and corroborated by clinical trials of better methodological quality.
A class II/2 malocclusion with considerable amount of crowding was treated non-extraction by distalization. The upper molars were moved posteriorly with the use of superelastic wires. The orthodontic therapy-mode is presented and the results are discussed. This treatment approach shows another use of superelastic wires.
